Hepatitis E

Infeksi zoonosis yang ditularkan melalui makanan

Acute self-limiting disease, bisa menjadi kronis 

sirosis hepatis

Seroprevalensi di Eropa 0,6% - 52,5%

Autoimmune Hepatitis

Penyebab pasti belum diketahui

Penyakit hati inflamasi kronis yang jarang

terjadi, ditandai kadar transaminase dan IgG
yang tinggi, dominan perempuan, autoantibodi
positif, interface hepatitis, dan respons cepat
terhadap pengobatan steroid.

Autoantigen (yang belum teridentifikasi) disajikan
pada sel naïve T-helper (Th0) di hepar.

Virus diduga sebagai pemicu AIH melalui

mekanisme mimikri molekuler.

Studi di Jerman, seroprevalensi HEV lebih tinggi

pada pasien dengan AIH.

Laporan kasus infeksi HEV akut dengan

autoantibodi positif.

Etik
•Deklarasi Helsinki 1975.
•Komite Etika setempat.
•The Scientific Committee of the Swiss AIH
cohort study.

Bahan
•Serum dari 50 pasien usia > 16 tahun.
•Diagnosa HEV akut.
•Ticino Canton Mei 2015 - Mei 2017.

Informasi klinis :
•Usia dan jenis kelamin
•Pre-existing infeksi HCV, HBV atau HIV
•Riwayat penyakit autoimun
•DM tipe II, Sirosis hepatis
•Obat imunosupresi
•Keganasan
•Penyalahgunaan alkohol dan narkoba
•Perjalanan keluar Eropa dalam 6 bulan terakhir

Kriteria Eksklusi :
Pre-existing AIH dan infeksi kronis HCV, HBV atau
HIV

Infeksi HEV akut :
•HEV RNA positif dalam serum/plasma atau feses,
dan atau
•Serum IgM anti-HEV positif atau gabungan serum
IgM dan IgG anti-HEV positif.

HEV viral load : Fast Track Diagnostic HEV.

HEV serologi : ELISA Dia.Pro HEV IgG dan
Dia.Pro HEV IgM.

untuk menilai hubungan antara adanya
autoantibodi dan usia, jenis kelamin, diabetes,
sirosis, imunosupresi dan komplikasi ekstra-hepar.

Hasil Serologi Autoimun Hepar

•ANA positif pada 16 pasien, titer 1:80 – 1:640.

•SMA positif pada 10 pasien, titer 1:40 – 1:320.
•ANCA positif pada 7 pasien3 cANCA,3 pANNA,
1 pANCA.
•AMA positif pada 1 pasien dicek ulang negatif.
•Anti SLA, Anti LKM1, Anti LC1  negatif

Secara statistik tidak ada hubungan signifikan
antara autoantibodi dengan usia, DM, sirosis,
obat imunosupresi dan komplikasi ekstra-hepar.

Ada hubungan signifikan antara SMA dan ALT

<500 UI/L (p=0,02), SMA dan jenis kelamin
perempuan (p=0,064).

Ada hubungan signifikan antara ANA dan

penyakit penyerta autoimun. (OR 4,4, CI 95%)

Hasil Follow up

• Tidak ada penanda serologis baru.

• Kadar ALT kembali normal.
• Tidak ada pasien yang kembali ke RS
dengan persistent liver disease.

 Diskusi
• 26 pasien positif autoantibodi:
5 memiliki pola homogen ANA yang biasanya
tampak pada AIH.
2 memiliki pola SMA (VG dan VGT), karakteristik
AIH tipe 1.
Semua pasien tidak memiliki antibody diagnostik
untuk AIH tipe 2 dan tidak ada pasien yang positif
untuk anti SLA spesifik AIH.

SMA pada hepatitis virus menunjukkan pola V
yang tidak spesifik, seperti terdeteksi pada
sebagian besar pasien di penelitian ini.

7 pasien hasil ANCA (pANCA, cANCA, pANNA)

positif, tidak ada yang bereaksi dengan target
molekul ANCA, seperti hasil ELISA pada AIH.

1 pasien AMA positif  negatif pada follow up

10 bulan.

Tidak ada pasien yang menjadi AIH selama
periode follow up 20 bulan, termasuk dua pasien
dengan pola SMA yang dianggap spesifik AIH.

ANA dan / atau SMA yang positif selama infeksi

HEV akut  diagnosis AIH yang salah 
pengobatan imunosupresif yang tidak tepat.

 Kesimpulan

• Autoantibodi non-spesifik terkait dengan AIH

sering terdeteksi selama infeksi HEV akut.
• Pada subjek yang memiliki kecenderungan
penyakit autoimun, infeksi HEV kemungkinan
dapat memicu AIH.
