 Kewaspadaan isolasi adalah tindakan pencegahan atau pengendalian infeksi

yang disusun oleh Center for Desease Control(CDC) dan harus diterapkan di

rumah sakit dan pelayanan kesehatan lainnya. Kewaspadaan isolasi diterapkan

untuk menurunkan resiko trasmisi penyakit dari pasien ke pasien lain atau ke

pekerja medis.Kewaspadaan isolasi memiliki 2 pilar atau tingkatan, yaitu

Kewaspadaan Standar (Standard/Universal Precautions) dan Kewaspadaan

berdasarkan cara penularan (Transmission based Precautions) (Muchtar, 2014;

Akib, dkk, 2008; Rosa, 2015).

 Kewaspadaan Standar (Standard/Universal Precautions)
 
Kewaspadaan standar adalah kewaspadaan dalam pencegahan dan
pengendalian infeksi rutin dan harus diterapkan terhadap semua
pasien di semua fasilitas kesehatan.Kewaspadaan Universal yaitu
tindakan pengendalian infeksi yang dilakukan oleh seluruh tenaga
kesehatan untuk mengurangi resiko penyebaran infeksi dan
didasarkan pada prinsip bahwa darah dan cairan tubuh dapat
berpotensi menularkan penyakit, baik berasal dari pasien maupun
petugas kesehatan (Nursalam, 2007).
Kewaspadaan berdasarkan Transmisi
( Transmissionbased precautions)
Kewaspadaan berdasarkan transmisi merupakan tambahan untuk

kewaspadaan standar, yaitu tindakan pencegahan atau pengendalian

infeksi yang dilakukan setelah jenis infeksinya sudah terdiagnosa atau

diketahui (Akib, dkk, 2008).Tujuannya untuk memutus mata rantai

penularan mikroba penyebab infeksi, jadi kewaspadaan ini diterapkan

pada pasien yang memang sudah terinfeksi kuman tertentu yang bisa

ditransmisikan lewat udara, droplet, kontak kulit atau lain-lain

(Muchtar, 2014).
Berdasarkan IPC tahun 2018 , jenis Kewaspadaan berdasarkan transmissi yaitu:
1) Kewaspadaan transmisi kontak
Transmisi kontak merupakan cara transmisi yang terpenting
dan tersering menimbulkanHealthcare Associated
Infections(HAIs). Kewaspadaan transmisi kontak ini ditujukan untuk
menurunkan resiko transmisi mikroba yang secara epidemiologi ditransmisikan
melalui kontak langsung atau tidak langsung.

2) Kewaspadaan transmisi droplet

Transmisi droplet langsung, dimana droplet mencapai mucus
membrane atau terinhalasi. Transmisi droplet ke kontak, yaitu
droplet mengkontaminasi permukaan tangan dan ditransmisikan ke
sisi lain misal: mukosa membrane. Transmisi jenis ini lebih sering
terjadi daripada transmisi droplet langsung, misal: commoncold, respiratory
syncitial virus (RSV)
3).Kewaspadaan transmisi melalui udara ( Airborne Precautions )
Kewaspadaan transmisi melalui udara diterapkan sebagai tambahan
kewaspadaan standar terhadap pasien yang diduga atau telah diketahui
terinfeksi mikroba yang secara epidemiologi penting dan ditransmisikan
melalui jalur udara

Alat Pelindung Diri (APD)

a. Definisi
Alat Pelindung Diri (APD) adalah suatu alat yang membantu seseorang
untuk melindungi atau mengisolasi tubunnya dari segala macam bahaya
yang dapat mengancam jiwa di tempat kerja .
(Permenaker, 2010).

b. Macam-macam Alat Pelindung Diri (APD)

1. Sarung Tangan
2. Alat Pelindung Wajah
3. Penutup Kepala
4. Gaun Pelindung (Cover Gown)
5. Sepatu pelindung (Pelindung Kaki)
C. Tujuan dan Manfaat Alat Pelindung Diri
Penggunaan APD bertujuan untuk melindungi tenaga kerja dan
merupakan salah satu upaya mencegah terjadinya KAK dan PAK oleh
bahaya potensial pada suatu perusahaan yang tidak dapat dihilangkan
atau dikendalikan .
Berdasarkan penjelasan tentang manfaat dan tujuan alat pelindung
diri dapat diambil kesimpulan bahwa APD memiliki manfaat dan tujuan
sebagai pelindung tubuh pekerja dari bahaya-bahaya yang berada di
tempat kerja.

D. Prinsip dalam Penggunaan APD

Prinsip penggunaan APD berdasarkan Panduan Pemakaian Alat
Pelindung Diri di Rumah Sakit PKU Muhammadiyah Yogyakarta Unit II
tahun 2015, yaitu:
1. Setiap pegawai RS PKU Muhammadiyah Yogyakarta Unit II harus
dapat menggunakan APD dengan baik dan benar.
2. Setiap tindakan atau kegiatan yang dapat menimbulkan potensi
bahaya di rumah sakit harus dilakukan dengan menggunakan APD.
3. Penggunaan APD disesuaikan dengan jenis tindakan dan kegiatan
disetiap instalasi RS PKU Muhammadiyah Yogyakarta Unit II.
4. Kejadian tidak diharapkan yang disebabkan oleh kelalaian dalam
menggunakan APD di rumah sakit, bukan merupakan tanggung
jawab rumah sakit.

pendengaran, penglihatan, perasa, penghidu dan peraba tetapi sebagian besar
pengetahuan manusia diperoleh dari proses penglihatan dan pendengaran.
2. Pengawasan
Pengawasan dilakukan bertujuan untuk meningkatkan kedisiplinan pekerja untuk
melakukan pekerjaan sesuai dengan SPO.Begitu pula pada penerapan penggunaan alat
pelindung diri harus diatas pengawasan yang tepat agar terlaksana sesuai dengan SPO yang
ada di rumah sakit .
3. Standar Prosedur Operasional ( SPO)
SPO adalah instruksi atau langkah-langkah yang dilakukan untuk menyelesaikan suatu
proses kerja rutin tertentu, yang berupa kebijakan yang telah ditetapkan. SPO bertujuan
untuk memberikan langkah-langkah yang benar agar mengurangi terjadinya kesalahan dan
pelayanan di bawah standar dalam melaksanakan berbagai kegiatan dari fungsi pelayanan
(Siburian, 2012).
4. Fasilitas APD di Rumah Sakit
Fasilitas APD yang tersedia di rumah sakit ini sangat berpengaruh, karena walaupun tingkat
pengetahuan tenaga keperawatan sudah baik, adanya pelatihan dan terdapat SPO apabila
fasilitas pendukung APD rumah sakit tidak terpenuhi/tidak sesuai standar maka
penggunaan APD oleh perawat tidak maksimal.

 Hand Hygiene / Mencuci Tangan

Larson mendefinisikan mencuci tangan adalah menggosok dengan

sabun secara bersamaan seluruh kulit permukaan tangan dengan
kuat dan ringkas yang kemudian dibilas dengan air yang
mengalir.Tujuan melakukan mencuci tanganadalah untuk membuang
kotoran dan organisme yang menempel dari tangan dan untuk
mengurangi jumlah mikroba yang ada saat itu serta mencegah
perpindahan organisme multi resisten dari lingkungan rumah sakit ke
pasien dan dari pasien ke petugas kesehatan begitu juga sebaliknya .

adalah perawat pelasana. Menurut Gaffar dalam Praptianingsih
(2006)
peran sebagai pelaksana bertidak sebagai Comforter and
protector
(melindungi pasien dan mengupayakan terlaksananya hak dan
kewajiban pasien dalam pelayanan kesehatan), commmunicator
(tampak ketika perawat bertindak sebagai penghubung antara
pasien dengan anggota tim kesehatan) serta rehabilitator
(perawat membantu pasien untuk beradaptasi dengan
perubahan tubuhnya).
b. Perawat yang berstatus pegawai tetap dan kontrak di
Rumah Sakit
PKU Muhammadiyah Gamping Sleman.
Menurut Hasibuan (2002) karyawan tetap merupakan
karyawan
yang telah memiliki kontrak ataupun perjanjian kerja dengan
perusahaan/instansi dalam jangka waktu yang tidak
ditetapkan (permanent). Karyawan tetap biasanya cenderung
memiliki hak yang jauh lebih besar dibandingkan dengan
karyawan tidak tetap. Selain itu, karyawan tetap juga
cenderung jauh lebih aman (dalam hal kepastian lapangan
pekerjaan) dibandingkan dengan karyawan tidak tetap.
