Definisi
Hiperemesis gravidarum adalah mual dan
muntah yang muncul secara berlebihan
selama hamil. Mual dan muntah (morning
sickness) pada kehamilan trimester awal
sebenarnya normal. Namun
pada hiperemesis gravidarum, mual dan
muntah dapat terjadi sepanjang hari dan
berisiko menimbulkan dehidrasi.
HIPEREMESIS GRAVIDARUM

Muntah yang berlebihan biasanya pada kehamilan

trimester I sehingga mengganggu pekerjaan sehari-
hari dan keadaan umum menjadi buruk.
Primi gravida = 60-80 %
Multi gravida = 40-60 %
Mual (nausea) disebabkan :
H.Estrogen
H.HCG (Human Chorionic Gonadotropin)

ETIOLOGI
Belum diketahui secara pasti
Faktor Predisposisi :
○ Primigravida
○ Mola Hidatidosa
○ Kehamilan ganda
Faktor Organik :
○ Masuknya vili khorialis dalam sirkulasi maternal
& perubahan metabolik

 PATOLOGI
Hati :
→ Degenerasi lemak tanpa nekrosis, terletak Sentrilobuler.
Jantung :
→ Lebih kecil daripada biasa & berat ↓
→ Kadang-kadang ditemukan perdarahan Sub
Endokardial
Otak :
→ Kadang terdapat bercak perdarahan otak & seperti
Ensefalopati Wernicke :
Dilatasi kapiler
Perdarahan kecil-kecil daerah korpora mamilaria
ventrikel III & IV
Ginjal :
→ Tampak pucat & degerasi lemak pada Tubuli kontorti
 PATOFISIOLOGI

→Mual akibat Estrogen ↑

→Faktor psikologik & hormonal →
Hiperemesis Gravidarum →
Dehidrasi & tidak imbangnya
Elektrtolit dengan Alkalosis
Hiperemesis Gravidarum dibagi 3 tingkat :
Tingkat I :
Muntah terus sehingga mempengaruhi keadaan umum penderita :
Lemah, napsu makan (-),BB ↓, nyeri epigastrium, nadi ↑ 100 x /m
,Td.sistolik ↓, turgor kulit ↓, lidah kering, mata cekung.

Tingkat II :
Lebih lemah & apatis, turgor kulit ↓↓, lidah kering & kotor, nadi
kecil & cepat, suhu kadang ↑, mata ikterus, BB ↓, mata
cekung, tensi ↓, hemokonsentrasi, oliguria, konstipasi,
pernapasan bau aseton.

Tingkat III:
Kesadaran ↓ somnolen sp.koma, muntah berhenti, nadi kecil
& cepat, suhu ↑, tensi ↓, ensefalopati wernicke (nistagmus,
diplopia, perubahan mental), ikterus.

Hamil muda & muntah terus menerus
sehingga mempengaruhi keadaan
umum.
DD/
 Hamil muda dengan pielonefritis,
hepatitis, ulcus ventrikuli, tumor
