The Glomerular Syndromes
Acute nephritic syndrome•
Hematuria,azotemia, variable proteinuria, oliguria,
edema, and hypertension
Rapidly progressive glomerulonephritis•
Acute nephritis, proteinuria, and acute renal failure
Nephrotic syndrome• >3.5 gm proteinuria,
hypoalbuminemia, hyperlipidemia, lipiduria
Chronic renal failure• Azotemia ➙ uremia
progressing for years
Asymptomatic hematuria or proteinuria•
Glomerular hematuria; subnephrotic proteinuria
Primary glomerular diseases leading to chronic glomerulonephritis (GN). The thickness of the arrows reflects the
approximate proportion of patients in each group who progress to chronic glomerulonephritis: poststreptococcal (1%
to 2%); rapidly progressive (crescentic) (90%), membranous (30% to 50%), focal glomerulosclerosis (50% to 80%),
membranoproliferative glomerulonephritis (50%), IgA nephropathy (30% to 50%).

Patologia:
Proliferacion difusa de células mesangiales e
infiltración del glomérulo por macrofagos, aumento de
la matriz mesangial con engrosamiento y repliegue de
la membrana basal glomerular
Causas mas comunes.
GN por inmunocomplejos
Asociada a microangiopatia trombotica
Asociada a enfermedades por deposito
Postraspalnte de medula osea o renal
Idiopatica
GNMP tipo I deposito inmunitario subepiteliales y
mesangiales
GNMP tipo II depósitos densos
intramembranosos con poca Ig, asociado a factor
nefritico C3
GNMP tipo III con rasgos de tipo I y GN
Membranosa
Asociada a otras causas:
Enfermedades por inmunocomplejos
Infecciones crónicas
Enfermedades malignas
Hepatopatias
Otros: uso de heroína , sarcoidosis
Evolución
cursa hacia falla renal, puede tener
progresión lenta y tener agudizaciones en el
tiempo, se espera que la enfermedad
termine con el paciente en reemplazo renal
(dialisis, trasplante)
Terapia
Se usa terapia sintomática
Dieta hiposódica < 3 gr/d
Dieta hipoproteica 0.6 gr/kg/d
Diureticos de asa
Diureticos tiazidicos (HCTZ)
IECA …
Inmunosupresores es incierto . Se intenta con
Prednisona 0.5 a 1 mg /Kg pero tiene una
evolución incierta para retrasar la progresión de
la enfermedad.
Esteroides y agentes citotóxicos no demostraron
ser valiosos.
Hipercolesterolemia
Su influencia sobre la mortalidad y morbilidad es
incierta.
Lovastatina y sinvastatina producen 25-40% de
descenso del colesterol
No se ha demostrado que reduzca el riesgo de
aterosclerosis ni de enfermedad isquémica
cardiaca o cerebral
Complicaciones tromboembólicas
Pueden producirse oclusiones espontáneas de las
venas periféricas, de las arterias periféricas, de las
arterias pulmonares o de las venas renales.
