In recent years there have been many major incidents reported world-wide,
involving fatalities, serious injuries and environmental damage, as a result of
breaking into pressurised systems which had not been fully depressurised,
isolated or purged before the commencement of work.
Dalam beberapa tahun terakhir telah terjadi insiden besar yang banyak dilaporkan di seluruh
dunia. melibatkan kematian, luka-luka serius dan kerusakan lingkungan, sebagai akibat dari

During the normal course of our operations we are regularly required to work on
areas of plant which, under running conditions, are normally charged with steam,
liquid or gas under pressure and it is essential that we observe the following
guidelines to help prevent an accidental release.

biasanya diisi dengan uap, cair atau gas di bawah tekanan dan sangat penting untuk kita amati

1. Before starting work ensure that: -

Sebelum memulai pekerjaan pastikan bahwa:
 a safe system of work (a Permit) is in place;

 you have read and understood the risk assessment for the task;
Anda telah membaca dan memahami beban risiko suatu pekerjaan
 you have read and understood the procedures relating to the task;

 you have been given clear instructions to start work, by your
supervisor;

 the system has been isolated, depressurised and, if necessary,
purged; and

2. When breaking joints, ensure that: -

fasteners in place, until you are absolutely sure that there is no

3 When remaking joints, ensure that: -

 the correct jointing materials and fasteners are used:

Remember: - Don't take anything for granted
