Kadar TSH pada subgrup Metformin lebih rendah dibandingkan dengan pasien non
DM ( = 0.032)
& Seiring dengan dinaikkannya dosis Metformin, didapati Kadar TSH semakin
Bila menggunakan Kadar TSH berdasarkan
umur terhadap Pasien Lansia dengan DM
menurun
didapati 6.4% (45/751) terdiagnosis HIPOTIROID
DISKUSI
Pada Studi Kami, didapati pasien dengan
HIPOTIROID pada
pasien dengan DM dibandingkan dengan Non
DM
Hasil dari studi sebelumnya (Kidayala, 2010)
didapati pasien DM dengan kondisi hipotiroid lebih
tinggi dibandingkan dengan non DM namun dengan
prevalensi yang lebih rendah dibandingkan pada
hasil penelitian
Hasil dari Pubmed didapati pada kasus hipotiroid
pada pasien Diabetes Mellitus didapati kadar TSH
yang bervariasi
(5.7% - 27.7%)
DISKUSI
Hasil TPOAb (+) pada pasien DM cenderung
tinggi (13,8%) terutama pada pasien yang
Hipotiroid (+) (64,4%)
DISKUSI
Pada pasien DM yang diterapi dengan
metformin memiliki kadar TSH cenderung
rendah dibandingkan dengan Px DM yang
dapat OAD jenis lain.
Hasil dari studi sebelumnya (Vigersky, 2010) dan
(Cappelli, 2012) Didapati pasien DM yang
mengkonsumsi metformin, hasil TSH cenderung lebih
rendah
DISKUSI
DISKUSI
Pada pasien DM yang diterapi dengan Insulin
memiliki kadar TSH tertinggi dibandingkan
dengan Grup Lain
DISKUSI
Hasil studi peneliti menunjukkan
bahwa Reference Interval
terhadap kadar TSH berdasarkan
umur pada pasien DM
memberikan efek positif dan
mengurangi kemungkinan salah
mendiagnosis Hipotiroid pada
pasien Geriatri

CRITICAL APPRAISAL
A. ARE THE STUDY RESULTS VALID?
1. Was the assignment of patients to treatment groups truly
randomized?
YES it was a double blind trial
2. Were all patients who entered the study accounted for
appropriately at the end?
YES, Because authors used the patients data from the
outpatient clinic
3. Were patients, physicians, and those doing the
assessments
"blind" to treatment?
YES, The participants, the researcher and the healthcare
were All BLINDED
4. Was similarity between groups documented?
YES, there is a table of baseline characteristics
5. Aside from the intervention, were the groups treated in
the same way?
YES, both group have the same characteristics
CRITICAL APPRAISAL
B. WHAT WERE THE RESULTS?
1. HOW LARGE WAS THE TREATMENT EFFECT?
Among the 751 patients whom had diabetes, using age-specific RI,
we observed that the diagnosis of hypothyroidism was slightly but
significantly higher in patients with diabetes than in persons
without diabetes. In conclusion, the Use of TSH-RI is appropriate
for the elderly and can avoid the misdiagnosis of hypothyroidism.
CRITICAL APPRAISAL
C. CAN THE RESULTS BE APPLIED TO
YOUR PATIENT(S)?
1. CAN THE RESULTS BE APPLIED TO MY PATIENT (CARE)?
YES, it can be applied to my patients, both incare and outcare
patients.
2. WERE ALL CLINICALLY IMPORTANT OUTCOMES REPORTED?
YES, the primary outcome were reported.
