Technique
Preparation :
Take blood sample for glucose 15 before LP
Explain the procedure to patient
Obtain informed consent
Exclude possibility of increased ICP or
CNS mass lesion (eye exam/ head CT).
Position :
Lateral decubitus in full flexion posture
At the bed side
Small cushion on head or knee (if needed)

Pendahuluan
Gangguan serebral pada penderita HIV /
AIDS sering terjadi.
Mengenai SSP dan SST
Terjadi akibat infeksi HIV, oportunistik,
atau akibat obat yang digunakan
Kelainan bervariasi dari ringan s/d berat
Mulai berkurang setelah ditemukan obat
obatan yang lebih baik
Gejala klinis yang muncul biasanya hampir sama,
walaupun etiologi berbeda
Etiologi yang sama bisa memunculkan gejala
klinis yang berbeda
Menyebabkan berkurangnya kualitas hidup
penderita
Pada keadaan yang lanjut, bisa berakibat fatal
Patogenesis
HIV masuk ke SSP dan LCS segera setelah
infeksi terjadi
Biasanya menyerang sel monosit dan makrofak,
sel astroglia dan oligodendrosit, sel neuron jarang
terganggu
proses patologis lebih sering disebabkan oleh
reaksi imflamasi
Kerusakan patologi pada awalnya terjadi pada
substansi grisea (Deep Grey Matter) di ikuti oleh
substansi alba dan korteks sel nueron berkurang,
struktur dendritik menjadi lebih sederhana
HIV masuk ke tubuh dan menyerang sel limfosit
multiplikasi sel mati, meyerang sel lainnya
Masuk ke SSP reaksi inflamasi
kerusakan jaringan
Penurunan kekebalan tubuh menyebabkan
reaktivasi agent penyakit yang sebelumnya sudah
ada
Obat obat yang digunakan bisa menyebabkan
terjadinya reaksi alergi yang merusak saraf dan
tidak dapat ditolenrasi oleh penderita
Toksoplasmosis
Serebral
Biasanya terjadi akibat reaktivasi infeksi sebelumnya baik
di SSP maupun ditempat lainnya
Terlihat pada fase lanjut infeksi HIV (CD4 < 200
sel/mm3)
Di AS diperkirakan 3% 10% penderita HIV / AIDS
terinfeksi oleh toksoplasma
Gejala klinis yang muncul (demam, sakit kepala, delirium,
demensia, depresi, gangguan penglihatan, hemiparesis,
ataksia, kejang)
Diagnosis ditegakkan berdasarkan anamesis,
gejala klinis, gambaran CT / MRI, test PCR
Diagnosis definitif ditegakkan dengan
ditemukannya toksoplasma dari cairan LCS atau
Biopsi otak
Pada pemeriksaan CT / MRI bisa ditemukan
abses multipel yang tersebar di kedua hemisfer
Pada 1/3 penderita ditemukan lesi tunggal yang
sering di salah tafsirkan dengan limfoma
Pengobatan : diberikan segera setelah
dicurigai adanya infeksi toksoplasma
Obat yang digunakan kombinasi
sulfadiazin dan pyrimethamine
Bila penderita alergi dengan sulfadiazin
bisa diganti dengan clindamycin, lecoforin
5-10 mg dapat ditambahkan untuk
mencegah depresi sum sum tulang
Kortiko steroid digunakan apabila ada
edema yang luas
90% penderita membaik dalam waktu 14
hari, dilanjutkan dengan pengobatan
maintenance dengan TMP / SMX
Prognosis tergantung pada fase penyakit
(kadar CD4) dan pengobatan pararel
dengan HAART
Pencegahan : menghindari makan daging
mentah atau kurang matang (babi,
kambing, sapi)
Mencuci sampai bersih buah buahan dan
sayuran
Jangan lupa cuci tangan setelah kontak
dengan daging mentah, tanah, dan kucing
Ensefallitis
Cytomegalovirus
Sering menginfeksi orang sehat tanpa gejala klinis
Pada penderita HIV / AIDS dimana kadar sel
CD4 < 50 dapat menyebabkan infeksi yang
serius
20% penderita dengan sel CD4 < 100 mengalami
infeksi CMV diberbagai organ tubuh
Pada Otopsi ditemukan CMV pada 5% 40%
penderita HIV / AIDS dimana sebelumnya tidak
terdiagnosis adanya infeksi CMV
Diagnosis : ditegakkan berdasarkan gejala klinis,
pungsi lumbal, tes PCR, dan CT / MRI
Pada pemeriksaan CT / MRI tidak ditemukan
kelainan spesifik, lesi masa (abses) jarang terjadi
Pengobatan : dapat diberikan ganciclovir dan
foscavir secara tersendiri atau kombinasi
Kadang diperlukan terapi maintenance seumur
hidup
Prognosis ditentukan oleh stadium penyakit HIV
/ AIDS
Cryptococcosis
serebral
Merupakan jenis jamur yang paling sering
menginfeksi SSP penderita HIV / AIDS
Ditemukan jika kadar sel CD4 <
100/mm3
Sama dengan toksoplasma bisa muncul
dalam bentuk meningitis, ensefalitis, SOL
(Cryptococcoma)
Gambaran klinis tidak spesifik, sering
terlambat diobati
Diagnosis : berdasarkan klinis, pungsi lumbal, CT
/ MRI
Pada pemeriksaan CT / MRI bisa ditemukan
adanya inflamasi, abses, hidrosefalus yang
memerlukan tindakan VP shunt
Pengobatan : dapat diberikan amphotericin B
dan bisa dikombinasi dengan flucytosine, bisa
diberikan fluconazole dengan dosis 200 400
mg/hari (oral)

T2-weighted MRI shows left occipital hyperintense white
matter changes with the lesion margin reaching the cortex.
This sliced fixed brain shows multiple isolated or
confluent gray demyelinative foci. Atrophy may be

Microscopically, multiple demyelinative foci are detected. The
microscopic hallmark of the disease is intranuclear basophilic or
eosinophilic inclusions within the swollen nuclei of oligodendrocytes,
often at the periphery of lesions. Large, occasionally multinucleated
astrocytes with prominent processes are another characteristic feature.
