• Ulkus pada tungkai adalah penyakit arteri, vena, kapiler dan pembuluh darah
limfe yang dapat menyebabkan kelainan pada kulit.
• Pembagian ulkus kruris dibagi ke dalam empat golongan yaitu, ulkus tropikum,
ulkus varikosus, ulkus arterial dan ulkus neurotrofik.
 ETIOLOGI
• Trauma, higiene yang buruk, gizi buruk, gangguan pada
pembuluh darah dan kerusakan saraf perifer dianggap
sebagai penyebab yang paling sering.
• Kerusakan saraf perifer biasanya terjadi pada penderita
diabetes mellitus dan penderita kusta.
• Hipertensi juga dikaitkan sebagai salah satu penyebab
rusaknya pembuluh darah.
• Insiden penyakit ini meningkat seiring dengan
bertambahnya usia.
• Di negara tropis, insidens ulkus kruris lebih kurang 2% dari
populasi dan didominasi oleh ulkus neurotropik dan ulkus
varikosum.
• Wanita lebih banyak terserang ulkus varikosum daripada
pria, dengan perbandingan 2:1, dengan usia rata-rata di
atas 37 tahun untuk prevalensi varises.

• Ulkus yang berkembang cepat dan nyeri.
• Tungkai bawah
• Anak-anak kurang gizi
• Banyak di temukan di daerah pedesaan
• Etiologi ada 3 (truma, gizi &hygene buruk dan infeksi)
• Gejala klinis soliter pada tungkai bawah, biasa di
dahului oleh trauma
• Ulkus berbentuk seperti cawan dan sekitarnya tampak
meradang.
• KU baik.

× Ulkus pada tungkai bawah yang di sebabkan oleh gangguan aliran
darah vena.
× Etiologi : dari luar( hamil,tumor)dan dalam(trombosis dan
tromflebitis)
× Patogenesis bendungantekanan vena meningkatoedem
dan perdarahan(purpura)
Kelamaan akan menjadi fibrotiknekrotik
Biasanya di sertai dengan dermatitis stasis.

bulat,lonjong,dangkal dan tertutup oleh jaringan nekrotik.
× Dx/ gambaran klinis
× DD dengan ulkus karena gangguan arteri (lebih dalam dan nyeri)
× Th tungkai bawah di letaj kan lbh tinggi, kompres dan antibiotik

× Etiologi : Ektra mural, mural dan intra
mural
× Patogenesis gangguan aliran darah
arteri  penyempitan lumen
hipoksia perubahan pada kulit ( tipis,
kering, bersisik, sianotik) gangren
dan timbul ulkus.

× Gejala klinis : hipertensi(lateral dari
pergelangan kaki), arterosklerosis
(tonjolan tulang)
× Lesi eritem bula
hemoragiknekrosis
× Ulkus yang timbul lebih dalam
berbentuk punch out, tepi jelas
× Nyeri lebih hebat pada malam hari,
timbul mendadak/ perlahan dan nyeri
++ bila dingin atau tungkai di angkat

× Dx anamnesis dan gambaran klinis
dan cari fc predisposisi
× Khas : nyeri pada malam hari
× Th/ ulkus di terapi (kompres, AB)
× Obati fc predisposisi
× Nasehati hindari dingin dan merokok

 Ulkus Neurotrofik
• Ulkus yang terjadi karena tekanan atau trauma pada kulit
yang anestetik
• Pada telapak kaki, ujung jari, sela jari kaki.
• Etiologi  saraf rusak  hilang rasa nyeri trauma
berulang rusak jaringan
• Gejala klinis pada kaki bisa tunggal maupun multipel
• Ulkus bulat, tidak nyeri, kering, kalus
