KULIT
• SSTI merupakan infeksi bakteri yang umum terjadi, tingkat keparahan dan
keterlibatan bergantung dari jenis bakteri yang menginfeksi dan lapisan kulit
yang terkena.
• Klasifikasi berdasarkan Infectious Disease Society of America (IDSA)
• Keperluan uji mikrobiologi bergantung pada tingkat keparahan, kondisi klinis dan
kondisi yang mendasarinya
• Tidak semua kondisi perlu dilakukan uji mikrobiologi, umumnya kondisi SSTI yang
terdapat eksudat dan abses sebaiknya diambil specimen dan dikirim ke mikrobiologi
untuk informasi lebih lanjut mengenai etiologi
• PCR memiliki tingkat sensitivitas yang lebih rendah namun dapat bermanfaat untuk
diagnosis penyebab S.aureus yang berguna untuk pemilihan rejimen antibiotik
• Radiologi dapat membantu diagnosis SSTI adanya gas pada pemeriksaan radiologi
jaringan dapat mengindikasikan infeksi nekrotik
DIAGNOSIS DAN ETIOLOGI
• CT Scan dapat membantu untuk melihat proses perluasan infeksi, dapat juga
memandu aspirasi cairan dan menemukan keberadaan benda asing bahkan
melihat partikel udara kecil pada jaringan.
• MRI berguna untuk membedakan selulitis yang terbentuk akibat pus dan abses.
Memiliki akurasi tinggi dibanding CT scan dalam deteksi nekrosis, inflamasi,
edema dan keterlibatan otot pada SSTI.
NON NECROTIZING INFECTION
IMPETIGO
• Abses kondisi apabila terdapat pus pada dermis dan jaringan kiulit yang lebih dalam
• Disebabkan oleh infeksi bakteri flora normal pada kulit yang terlibat
• Di USA penyebab tersering Methicillin Resistant S.aureus (MRSA)
• Furunkel, infeksi pada folikel rambut yang dapat membentuk abses pada jaringan
subkutis
• Karbunkel, kumpulan furunkel yang dapat meluas hingga jaringan subkutan lemak
• Furunkel & karbunkel predileksi di kulit berambut dimana saja
(leher,wajah,bokong) sering pada pasien DM
ABSES, FURUNKEL, DAN KARBUNKEL
• Infeksi primer yang disebabkan oleh MRSA saat ini menjadi fokus utama
klinisi
• Faktor predisposisi tinggal pada area padat, sering kontak antar individu,
aktivitas yang mengganggu permukaan kulit, bertukar benda yang
terkontaminasi luka, hygiene buruk
TATALAKSANA FURUNKEL &
KARBUNKEL
• Furunkel berukuran kecil dapat diberi kompres hangat untuk memberikan drainase
• Furunkel, karbunkel dan abses berukuran besar membutuhkan tindakan insisi
drainase
• Pemberian antimicrobial anti-staphylococcus pada tata laksana furunkel dan
karbunkel masih belum jelas.
• Furunkel rekuren dapat dicegah dengan penggunaan sabun yang mengandung
chlorhexidine gluconate dengan isopropyl alcohol
• Clindamycin 150mg/hari dapat digunakan untuk infeksi rekuren staphylococcus
TATALAKSANA ABSES
• Secara umum terapi antibiotik oral tidak diindikasikan pada abses local dengan
diameter <5cm
• Pertimbangan pemberian antibiotik terhadap S. aureus merupakan sebagai
tambahan apabila terdapat temperature >38 atau <36, takipnea >24x/m,
takikardia >90x/m atau Leukosit >12.000 atau <400
• Beberapa antibiotik oral yang dapat digunakan
• TMP-SMX, klindamisin, tetrasiklin (doksisiklin, minosiklin), fluoroquinolone.
CELLULITIS
• Infeksi jaringan lunak yang mengancam jiwa yang disebabkan oleh spesies
clostridium (C.perfringen, C.novyi, C.bifermentans, C.histolyticum)
• Pasien mengeluhkan nyeri pada area trauma atau luka pembedahan dengan
progresivitas yang cepat, bertambah berat dan meluas
• Kulit menjadi edema dan kencang, berwarna pucat-magenta, dapat disertai
bulla kendur, ulkus berbau
FOURNIER’S GANGRENE
• Pigmentes lesions
• Nevus/ melanocytic nevus
• Present after birth and consist concentration of nevus cel. Location at junctional (epidermal-dermal
junction, intradermal or both in dermis and junction)
• Predispotition, palms and soles tend to uniform, macular, and regular border.
• Ingradermal nevi, found on face, homogeneous, elevated, dome shaped, skin colored lesion
• Compound, can be natural pigmented
• Congenital melanocytic nevus
• Present at birth, singular and small. Size small, intermediate, giant (>20 cm). Giant have potential
to be melanoma. Characterized dar, and have thick hair.
• Blue nevus
• Appear bluish, nevus cell in dermis. Solitary, nodular lesion with smooth surface tend to blue or
blue-grey.
• Halo nevus
• When melanocytic nevus sourroude by hypopigmented halo. Occur on torso. Usually multiple.
• Spitz nevus
• Acquired, in children and young adults. Firm, domed shaped, reddis or dark brown
nodule. Frequently on head and neck. Treatment is surgical excision (complete with
clear margin), histopatology same feature with melanoma malignant
• Nevus of Ota
• Hamartomatous melanocytic lesion occur on face, ophthalmic and maxilly, divison of the
trigeminal nerve. Common in women. Treatment, laser therapy using q0switched
• Atypical mole-dysplastic nevi
• Melanocytic nevi tht have clinical feature of melanoma, asymmetry, border irregularity,
color variability, and diameter greater than 6mm. Recommend excisional biopsy.
• Solar lentigo
• Occur on sun-exposed area, face, arms and dorsum of hands. Size from 0.2 to 2 cm.
Recommend to biopsy.
• Ephelides
• Small, less than 3 mm, red or light brown macules on sun exposed area. No increase in
number melanocytes, but increase melanin in the skin.

• Eccrine poroma
• Solitary, firm, skin colored erythematous papule on sole or palm
in adults
• Cylindroma
• Solitary or multiple. Found on scalp as numerous small papule or
large nodule. Excision, electrosurgery and carbon dioxide laser
• Clear cell hidradenoma
• Eccrine sweat gland tumor. Solitary nodule. Firm nodule 0.2 to 5
vm. Surgical exicion
APOCRINE TUMORS
• Apocrine cystadenoma
• Solitary nodular lesion on face tend to skin colore to bluish, surgical

• Intradermal nodule ussualy found on head and neck, composed by sweat

• Present at birth, scalp and neck, present as multiple translucent or
pigmented plaque or papule
SEBACEOUS TUMORS
• Sebaceous nevus
• Tumor of childhood. Solitary, oval to linear, yellowish, present on scalp. Surgical
excision recommender before adloesence because potential development to BCC
• Sebaceous epithelioma
• Look like BCC, tend to more yellowish. Scalp and face. Excision, radiation, carbon
dioxide laser.
• Sebaceous hyperplasia
• Commonly on forehead, cheeks, lower eyelid or nose. Dome shaped, sometimes
umbilicated.
• Rhinophyma
• Enlargement of nose, in men. Highly potential to be BCC. Excision, and reconstruction
with forehead flap
