Bronkhospasmolitik kronik yang diperantarai oleh sel-sel, terutama
sel mast, eosinophil, makrofag, neutrophil, dan

 GEJALA ASMA
Gejala Asma Bersifat Episodik
Seringkali reversibel dengan/atau tanpa
pengobatan. Gejala awal berupa :
 Batuk terutama pada malam / dini
hari
 Sesak napas
 Napas berbunyi (mengi) yang
terdengar
 Jika pasien menghembuskan
napasnya
 Rasa berat di dada
 Dahak sulit keluar.
Gejala yang Berat
Keadaan gawat darurat yang mengancam
jiwa. Yang termasuk gejala yang berat
adalah:
 Serangan batuk yang hebat
 Sesak napas yang berat dan tersengal-
sengal
 Sianosis (kulit kebiruan, yang dimulai
dari sekitar mulut)
 Sulit tidur dan posisi tidur yang nyaman
adalah dalam keadaan duduk
 Kesadaran menurun

OBAT ASMA (E)-ANTIDEPRESSAN
SIKLIK
Antidepressan siklik memblok reuptake neurotransmitter, seperti norepinefrin

Interaksi obat asma – antidepressan

→ menurunkan metabolisme obat asma
→ kadar obat asma meningkat
→ meningkatkan efek obat asma
→ aritmia jantung / kenaikan tekanan darah yang berbahaya

Antidepressan siklik:
Doksepin
Desipramin
Nortriptilin
Imipramin
Amitriptilin

Solusi : Hindari penggunaan bersamaan

 OBAT ASMA (E)-OBAT JANTUNG
DIGITALIS
Obat jantung digitalis bekerja dengan mengatur ion Calsium sehingga kontraksi
jantung lebih baik.

Epinefrin mempunyai efek kronotropik positif (meningkatkan kecepatan denyut

jantung) dan inotropik positif (memperkuat kontraksi myokardium) sehingga
cardiac out put (curah jantung) meningkat.

Interaksi obat asma – Obat jantung digitalis

→ meningkatkan efek obat jantung digitalis
→ terjadi aritmia jantung

Solusi :
 OBAT ASMA (E)-POTASSIUM
DEPLETING DRUG
• Obat jantung digitalis bekerja dengan mengatur ion Calsium sehingga kontraksi
jantung lebih baik.

• Epinefrin mempunyai efek kronotropik positif (meningkatkan kecepatan denyut

jantung) dan inotropik positif (memperkuat kontraksi myokardium) sehingga
cardiac out put (curah jantung) meningkat.

• Interaksi obat asma – Obat jantung digitalis

→ meningkatkan efek obat jantung digitalis
→ terjadi aritmia jantung

Solusi :
OBAT ASMA (E)-ANTIHIPERTENSI
• Antihipertensi bekerja menurunkan tekanan darah tinggi dengan cara menurunkan tekanan
darah perifer dan vasodilatasi pembuluh darah;

• Obat antihipertensi al menghambat enzym ACE, sbg vasodilator (α2- bloker)

• Epinefrin mempunyai efek meningkatkan tekanan darah melalui aktivasi adrenoseptor - β1

jantung yang terjadi setelah pelepasan atau pemberian Epinefrin.

• Epinefrin juga berefek pada timbulnya vasokontriksi karena stimulasi adrenoseptor-α pada
otot polos dinding pembuluh darah perifer.

• Interaksi obat asma – Obat antihipertensi

→ menurunkan efek obat antihipertensi
→ tekanan darah tetap tinggi

Solusi :
TRANSMISI NOREPINEFRIN
 OBAT ASMA (E) – ANTIDIABETES
Epinefrin meningkatkan :
- glikogenolisis di hepar dan otot rangka,
- menghambat sekresi insulin melalui aktivasi adrenoseptor – α (lebih
dominan dibanding peningkatan sekresi insulin melalui aktivasi
adrenoseptor - β2). Pada glikogenolisis, glikogen dideggradasi berturut-turut dengan 3
enzim, glikogen fosforilase, glukosidase, fosfoglukomutase, menjadi glukosa. Akibatnya
kadar gula darah tetap tinggi.
Interaksi obat asma – antidiabetes
→ menurunkan efek obat antidiabetes
→ kadar gula darah tetap tinggi

Antidiabetes: Klorpropamid, Tolazalamid, Tolbutamid,Insulin

 OBAT ASMA (E) – Beta BLOKER
Beta bloker mempunyai mekanisme antagonis di reseptor beta
Beta bloker selektif hanya bekerja di reseptor β1 di jantung, sedangkan
beta bloker non selektif bekerja antagonis di reseptor β1 di jantung dan
β2 di bronkus
Interaksi obat asma – Beta bloker
→ saluran bronkus tidak akan terdilatasi
→ menurunkan efek obat asma
Catatan : Efek penurunan obat asma oleh beta bloker seletif akan lebih
kecil daripada beta bloker non seletif
Beta bloker selektif: Atenolol, Esmolol, Metoprolol
Beta bloker non selektif: Propranolol, Labetalol

Solusi : Hindari penggunaan bersamaan

OBAT ASMA (T) – ALUPURINOL
Alupurinol bekerja menghambat kerja enzym xantin oksidase
(menghambat pembentukan xanthin menjadi asam urat)
Interaksi obat asma – Alupurinol

→ Alupurinol akan meningkatkan kadar xanthin (penurunan

metabolisme) (teofilin adalah turunan xanthin)
→ efek obat asma akan meningkat
→ efek samping meningkat

ES T : sakit kepala, tremor, insomnia, takikardia, aritmia jantung, dapat

terjadi kejang

Solusi : monitoring atau penyesuaian dosis teofilin

 OBAT ASMA (T) – BARBITURAT /
FENITOIN / ROKOK
Barbiturat / Fenitoin / Rokok akan meningkatkan metabolisme zat lain di hati

Interaksi obat asma – barbiturat / Fenitoin / Rokok

→ barbiturat / fenitoin / rokok akan meningkatkan metabolisme obat asma karena

merupakan inducer enzim di CYP1A2, konsentrasi obat asma di plasma akan kecil
→ efek obat asma akan menurun

Solusi : Monitoring
 OBAT ASMA (T) – SIMETIDIN
Simetidin akan menurunkan metabolisme zat lain di hati

Interaksi obat asma – Simetidin

→ Simetidin akan menurunkan metabolisme obat asma karena
merupakan inhibitor enzim di CYP1A2 dan CYP3A4, konsentrasi
obat asma di plasma akan tinggi
→ efek samping obat asma akan meningkat

ES T : sakit kepala, tremor, insomnia, takikardia, aritmia jantung, dapat

terjadi kejang

Solusi : Hindari penggunaan bersamaan

OBAT ASMA (T) – ERITROMISIN

Eritromisin akan menghambat P-glikoprotein sehingga menghambat

efflux zat lain

Interaksi obat asma – eritromisin

→ eritromisin akan meningkatkan konsentrasi obat asma di
plasma
→ efek obat asma akan meningkat
→ efek samping meningkat

Solusi : Hindari penggunaan bersamaan

OBAT ASMA (E/T) – STIMULAN LAIN
Stimulan lain bekerja merangsang sistem saraf pusat.

Interaksi obat asma – stimulan lain

→ bekerja sinergis meningkatan rangsang di SSP
→ terjadi takikardia, tremor, agitasi, gelisah, palpitasi
jantung, demam, hilangnya koordinasi otot, pernafasan
cepat dan dangkal, insomnia

Stimulan lain : Amfetamin Antidepresan MAOI
