Praktik Kefarmasian adalah sautu kegiatan yang
dilakukan oleh tenaga farmasi dalam menjalankan
pelayanan farmasi yang meliputi pembuatan termasuk
pengendalian mutu sediaan farmasi, pengamanan,
pengadaan, penyimpanan dan pendistribusian obat,
pelayanan obat atas resep dokter, pelayanan informasi
obat serta pengembangan obat, bahan obat dan obat
tradisional
Praktek pelayanan kefarmasian merupakan kegiatan yang
terpadu dengan tujuan untuk mengidentifikasi, mencegah
dan menyelesaikan masalah obat dan masalah yang
berhubungan dengan kesehatan.
Tujuan Pelayanan Kefarmasian :

 Melangsungkan pelayanan farmasi yang optimal baik dalam keadaan biasa maupun
dalam keadaan gawat darurat, sesuai dengan keadaan pasien maupun fasilitas yang
tersedia.
 Menyelenggarakan kegiatan pelayanan profesional berdasarkan prosedur
kefarmasian dan etik profesi.
 Melaksanakan KIE (Komunikasi Informasi dan Edukasi) mengenai obat.
 Menjalankan pengawasan obat berdasarkan aturan-aturan yang berlaku.
 Melakukan dan memberi pelayanan bermutu melalui analisa, telaah dan evaluasi pelayanan.
 Mengawasi dan memberi pelayanan bermutu melalui analisa, telaah dan evaluasi pelayanan.
 Mengadakan penelitian di bidangfarmasi dan peningkatan metoda.
STANDAR PELAYANAN FARMASI
DI RUMAH SAKIT
 Pelayanan farmasi rumah sakit masih bersifat
konvensional yang hanya berorientasi pada produk yaitu
sebatas penyediaan dan pendistribusian .

Peraturan Menteri Kesehatan (PMK) nomor 58 Tahun 2014 tentang

standar pelayanan kefarmasian di rumah sakit menjelaskan bahwa
pelayanan kefarmasian adalah suatu pelayanan langsung dan
bertanggung jawab kepada pasien yang berkaitan dengan sediaan
farmasi dengan maksud mencapai hasil yang pasti untuk
meningkatkan kualitas hidup pasien.
 Tujuan Standar Pelayanan Kefarmasian di
rumah sakit :

 Meningkatkan mutu pelayanan kefarmasian

 Menjamin kepastian hukum bagi tenaga kefarmasian
 Melindungi pasien dan masyarakat dari penggunaan obat
yang tidak rasional dalam rangka keselamatan pasien (patient
safety).
 Pelayanan Kefarmasian di Rumah Sakit meliputi
2 (dua) kegiatan, yaitu:

bersifatmanajerial berupa pengelolaan Sediaan
Farmasi, Alat Kesehatan, dan Bahan Medis Habis

Instalasi Farmasi Rumah Sakit (IFRS)

Instalasi Farmasi Rumah Sakit (IFRS) adalah suatu unit di rumah sakit tempat
penyelenggaraan semua kegiatan pekerjaan kefarmasian yang ditujukan untuk keperluan
rumah sakitdan pasien.

IFRS adalah fasilitas pelayanan penunjang medis, di bawah pimpinanseorang Apoteker yang
memenuhipersyaratan peraturan perundang-undangan yang berlaku dan kompeten
secaraprofesional, yang bertanggung jawab atas seluruh pekerjaan serta
pelayanankefarmasian, yang terdiri atas pelayanan paripurna, mencakup
perencanaan;pengadaan; produksi; penyimpanan perbekalan kesehatan/sediaan
farmasi;dispensing obat berdasarkan resep bagi penderita rawat inapdan rawat
jalan;pengendalian mutu dan pengendalian distribusi dan penggunaan seluruhperbekalan
kesehatan di rumah sakit; serta pelayanan farmasi klinis (Siregar danAmalia, 2004).
Tugas IFRS : Melaksanakan pengelolaan sediaan farmasi dan pengelolaan
perbekalan kesehatan.

Tanggung jawab IFRS : Mengembangkan pelayanan farmasi yang luas dan

terkoordinasi dengan baik dan tepat untuk memenuhi kebutuhan unit
pelayanan yang bersifat diagnosis dan terapi untuk kepentingan pasien yang
lebih baik.

Fungsi IFRS : sebagai unit pelayanan dan unit produksi. Unit pelayanan
yang dimaksud adalah pelayanan yang bersifat manajemen (nonklinik) adalah
pelayanan yang tidak bersentuhan langsung dengan pasien dan tenaga kesehatan
lain.
Ruang lingkup pelayanan kefarmasian di rumah sakit adalah:
• Administrasi dan Pengelolaan.
• Pimpinandan staf.
• Fasilitas dan Peralatan.
• Kebijakan dan Prosedur.
• Pengembangan Staff dan Program Pendidikan.
• Evaluasi dan Pengendalian Mutu.
 Seleksi atau pemilihan obat merupakan proses kegiatan sejak dari
meninjau masalah kesehatan yang terjadi di rumah sakit, identifikasi
pemilihan terapi, bentuk dan dosis, menentukan kriteria pemilihan
dengan memprioritaskan obat esensial, standarisasi sampai menjaga
dan memperbaharui standar obat. Penentuan seleksi obat merupakan
peran aktifPanitia Farmasi dan Terapi (PFT).

mempertimbangkan:
 Anggaran yang tersedia.
 Penetapan prioritas.
 Sisa persediaan.
 Data pemakaian periode yang lalu.
 Waktu tunggu pemesanan.
 Rencana pengembangan.
