Bayi Ny. VS umur 0 bulan masuk perawatan NICU dengan keluhan pada saat bayi lahir bayi
mengalami sulit bernafas, tidak menangis saat lahir dan bayi mengalami sianosis (kebiruan) saat
dilakukan pemeriksaan fisik di dapatkan hasil BBL: 3600 g, PBL: 50 cm, HR: 144x/m, RR:
40x/m, SB : 36,50C Hasil Lab GDS: 62mg.dl

Meconium aspiration Displasia bronkopulmoner adalah
(RDS) adalah kesulitan atau

1. Penyebab bayi sulit bernafas yaitu :
• Kelainan pada jalan napas/trakea, Kelainan bawaan/kongenital ini pun paling banyak ditemui pada bayi. Gejalanya, napas sesak dan napas
berbunyi "grok-grok".
• Tersedak air ketuban, Hal ini akan mengakibatkan kala lahir ia langsung tersedak. Bayi tersedak air ketuban akan ketahuan dari foto
rontgen.
• Pembesaran kelenjar thymus, Kelenjar ini semasa dalam kandungan berfungsi untuk sistem kekebalan. Letaknya di rongga mediastinum
(diantara dua paru-paru).
• Kelainan pembuluh darah, Ada lagi kelainan yang gejalanya seperti mendengkur atau napasnya bunyi (stridor), yang dinamakan dengan
vascular ring.
• Infeksi, Selain itu sesak napas pada bayi bisa terjadi karena penyakit infeksi. Bila anak mengalami ISPA (Infeksi saluran Pernapasan Akut)
bagian atas, semisal flu harus ditangani dengan baik.

2. Ada beberapa penyebab bayi lahir tidak menangis yaitu :

• Asfiksia, Salah satu penyebab bayi tidak menangis saat dilahirkan adalah asfiksia. Kondisi ini terjadi ketika bayi tidak mendapatkan cukup
oksigen selama proses kelahiran.
• Lahir premature, Bayi dianggap prematur jika lahir sebelum minggu ke-37 usia kehamilan. Bayi prematur lebih berisiko mengalami
beragam komplikasi kesehatan
• Keracunan air ketuban, Air ketuban yang normal berwarna bening atau agak kekuningan.

3. Sianosis atau warna biru pada kulit dan membran mukosa disebabkan adanya peningkatan konsentrasi hemoglobin tereduksi. Sianosis
dapat terdeteksi secara klinis jika kadar hemoglobin tereduksi di dalam pembuluh vena kulit mencapai kadar kritis 5 g/dl.

1. Defisiensi surfaktan pada bayi prematur dengan respiratory distress syndrome (RDS) atau hyalin
membrane disease (HMD) dapat menyebabkan alveoli kolaps dan gagal napas. Sejak tahun 1980, pemberian
surfaktan eksogen telah terbukti dapat mengurangi kematian dan risiko akibat ventilasi mekanik. Continuous
positive airway pressure (CPAP) memberikan hasil serupa dengan pemberian surfaktan profilaksis.
2. Terapi surfaktan diberikan pada bayi prematur dengan Hyaline Membrane Disease (HMD) yang
mengalami Respiratory Distress Syndrome (RDS). Pemberian terapi surfaktan dengan metode Intubation –
Surfactant – Extubation (INSURE) merupakan salah satu metode yang lebih minimal invasif dibandingkan
ventilasi mekanik konvensional.
