1. Foto Konvensional
• Kelebihan:
– Banyak tersedia di tempat tempat pelayanan
kesehatan
– Dapat dilakukan dengan mudah dan cepat
pada pasien yang dirawat
– Biaya lebih murah
– Dapat menyingkirkan penyakit utama seperti
obstruksi usus atau perforasi viskus
walaupun tidak sesensitif CT scan
Foto Konvensional
• Keterbatasan:
– Keterbatasan diagnostik karena sifat
pemeriksaan itu sendiri dan faktor kesulitan
teknis termasuk posisi pasien
– Menggunakan sinar x (radiasi pengion)
– Sensitifitas dan spesifisitas kurang
sehingga perlu pencitraan lebih lanjut untuk
menegakkan diagnosis yang lebih tepat

• Posisi tambahan:
– Lateral dekubitus RLD/LLD):
Untuk menilai udara bebas di peritoneum dan air-
fluid level pasien tidak mampu berdiri / tegak
– Foto thorax PA:
posisi ini lebih unggul dalam mendeteksi udara
bebas peritoneum dan dapat menggantikan posisi
AP tegak jika diduga adanya perforasi viskus
Keuntungan lain dari foto thorax PA ini adalah
untuk mengevaluasi keadaan paru-paru dan
jantung, jika pasien benar-benar membutuhkan
intervensi bedah

• Kelebihan:
– Cepat, handal, banyak tersedia di UGD
– Langsung visualisasi organ-organ dalam
abdomen
– Tidak menggunakan radiasi pengion
– Dynamic real-time scanning ada atau
tidak adanya peristaltik dan menggambarkan
aliran darah
– Biaya lebih rendah dibandingkan CT scan
atau MRI
Ultrasonografi
• CT scan : alat
pencitraan tunggal
yang paling utama
untuk mengevaluasi
pasien dengan nyeri
abdomen akut
• Di USA : Penggunaan CT scan meningkat
141% (1996 s/d 2005)
• Akurasi CT scan yang tinggi

– CT scan dengan pemberian kontras media: kontras
intravena, oral atau rektal
– Multiplanar reformasi (sagittal, koronal, oblik /
melengkung)
– Teknik dosis rendah

– MIP (Maximum intensity projection)

• Keunggulan:
• Keunggulan:
– Tidak adanya paparan radiasi pengion
– Aman digunakan pada pasien hamil
– Memiliki resolusi kontras intrinsik yang tinggi
sehingga berpotensi baik untuk penilaian dan
diagnosis penyakit panggul pada pasien wanita
– Sangat sensitif dan spesifik dalam kasus tertentu
(terutama jaringan lunak)

• Sistematis :
Dari lateral ke sentral
Dari sentral ke lateral
• Harus diingat :
– nyeri akut abdomen adalah gejala klinis
– meliputi berbagai penyakit yang kompleks
– dapat berasal dari saluran pencernaan, saluran
genitourinari atau organ reproduksi
– sering kali sulit untuk membedakan satu dari
yang lain jika kita hanya mengandalkan temuan
klinis dan laboratorium
Appendisitis akut
• Prevalensi: 14% dari pasien yang masuk
dengan nyeri perut ke UGD
• 250.000 pasien apendisitis / tahun di AS
• Gejala klasik:
– Samar-samar, nyeri viseral di wilayah
periumbilikalis (non-deskriptif) diikuti oleh mual
dan muntah
– Bila penyakit tetap berlangsung, maka nyeri
biasanya akan pindah ke RLQ karena
peradangan peritoneum lebih terlokalisasi.
Appendisitis akut
• Radiografi abdomen:
– Appendicoliths, biasanya di RLQ
– Pelebaran loop usus di RLQ (sentinel loop)
– Kesuraman di abdomen kanan bawah
• USG:
– Pembesaran, non-compressible dan edema
appendiks.Terisi cairan (diameter > 7 mm)
– Inflamasi lemak sekitar dan adanya focal fluid
collection
– Terdeteksinya appendikolith (juga dapat
ditemukan pada orang tanpa gejala)

– Appendix membesar, edema dan terisi
cairan (diameter > 6 mm).
– Fat strandings disekitar appendix / haziness

orang tanpa gejala)
– Dilatasi loop usus disekitarnya
– Appendiceal abscess / mass (apabila terjadi
perforasi)

• Abdominal Radiograph:
– Dilated large bowel segments with sudden
decrease in caliber at the point of obstruction
– Few or no gas present distally to the point of
obstruction
– “Coffee bean” appearance (plain AXR) or “bird‟s
beak” appearance (barium enema) volvulus
– “Coil-spring” appearance (barium enema)

LBO: Imaging Findings
• CT scan is the imaging modality of choice in
the diagnosis of LBO
• The accuracy of CT in the diagnosis of LBO:
– 96% sensitivity
– 93% specificity
• CT findings:
– Dilated colon (colon diameter 5.5 cm, cecal
diameter 10 cm) and filled with feces, gas, and
fluid proximal to an abrupt transition point, after
which the colon is collapsed distally.
– Cause of obstruction maybe identified (tumor,
adhesion, hernia)

Urinary Colic
• The most common cause :
– Urolithiasis, particularly ureterolithiasis
• Urolithiasis :
– 20% for males
– 5% to 10% for females
• Incidences:
– 20.1% in Saudi Arabia (the highest incidence)
– 12–13% in Canada and North America
– 5–9% in Europe
– 1–5% in Asia
Urinary Stone
• Stone composition :
– calcium oxalate : 33%
– calcium phosphate : 6%
– both co and cp : 34%
– uric acid : 8%
– cystine : 3%
– mixed struvite and apatite : 15%
• Pure uric acid stones are not visible with plain film
radiography.
• However, almost all urinary calculi, including uric
acid stones, are visible with unenhanced CT
Urinary Stone
• Conventional radiography:
– 60% sensitivity for the detection of renal and
ureteric calculi.
• CT scan:
– Sensitivities : 98% to 100%
– Specificities : 92 to 100%.
Imaging in Urinary Colic
USG
• is a safe method of examining for urolithiasis,
• particularly in pediatric and pregnant patients,
• avoiding the use of radiation
• is inferior to plain radiography and MDCT for the
quantification of stone
• has been reported to have sensitivities as low as
24% for the detection of calculi
• Conventional radiography in combination with
USG increases sensitivity for stone detection to
70%

USG
– Echogenic lines or spots with comet-tail
reverberation artifacts representing free
intraperitoneal air are seen adjacent to the
abdominal wall in a supine patient
– A sensitivity of 92% and a specificity of 53% have
been reported for the detection of perforation with
US and constitute an overall accuracy of 88%.
– It is important to note that establishing the cause
and location of the perforation is difficult with US.

– A concentration of extraluminal air

indicator of the site of the perforation :

– Drugs (e.g., azathioprine, furosemide,
