Bu evi dan Pak Klara

Sehubungan dengan adanya kunjungan DLHK Propinsi Riau dan DLH Kab. Siak yang
dimana pada siang ini mereka akan balik ke Pekanbaru dan Senin pagi tanggal 9
September 2019 tim akan melanjutkan lagi kunjungan ke LIBM dan pada hari
selasanya akan dilakukan Closing Meeting atau pembuatan Berita Acara. Maka dari
itu kami mohon bantuan dari Ibu untuk dapat menyediakan transportasi buat tim
dari pekanbaru untuk 4 Orang pada hari senin tanggal 09 september 2019 pukul
06.30 WIB.

Selama kunjungan saat ini ada juga yang ingin di komfirmasi oleh Tim DLHK Riau
berkaitan legalitas yaitu :
1. Berkaitan dengan HGU perusahaan dan luasan masing masing kebun
2. Izin SIPA 3 PKS
3. Bukti retribusi pembayaran SIPA

Sesuai arahan pak RC, kami mohon kehadiran ibu berkaitan dengan hal ini pada hari
senin dan selasa atau tanggal 09 dan 10 September 2019.

Selain itu kami juga mohon bantuan dari pak Klara berkaitan dengan peta peta yang
diminta oleh tim DLHK Riau, adapun peta peta yang diminta yaitu :
1. Peta Kebun
2. Peta wilayah kerja per unit
3. Peta rawan kebakaran
4. Peta areal sempadan sungai dan HCV

Peta peta tersebut juga dibutuhkan pada hari senin atau sebelum closing meeting
dilakukan.

Bapak CEO,

Setelah dilakukan koordinasi kepada tim DLHK Riau berkaitan jadwal kunjungan,
mereka tidak bisa tunda lagi sampai dengan tanggal 9 september 2019 dikarenakan
diatas tanggal 10 September 2019 mereka ada kegiatan lainnya.

Untuk kunjugan dimulai dari hari jumat tanggal 6 september 2019 dan pada hari
sabtu tanggal 7 september 2019 mereka balik ke pekanbaru dan hari senin tanggal
09 september mereka melanjutkan lagi sampai dengan hari selasa tanggal 10
September 2019.

Mohon arahan dari Bapak Hendra, Ibu Amanah dan Bapak Ismu, Adapun untuk minggu
ini dan minggu depan di region siak ada beberapa kegiatan :

1. Pemantauan Lingkungan Semester 2 2019
2. Mempersiapakan sanggahan PROPER untuk 3 PKS (untuk LIBM batasan sanggahan
sampai tanggal 12 September 2019)
3. Kunjungan DLHK Propinsi Riau

Berkaitan dengan 3 kegiatan ini agar bisa berjalan secara maksimal maka dari itu

Supaya Team Siak dapat memperhatikan dan mengecek kembali semuanya pesan2 Pak Ismu
supaya bisa dilaksanakan segera.

Demikian juga region dan pabrik lain supaya selalu diperhatikan meskipun tidak ada kunjungan
DLHK. Khususnya air hujan dan air proses,lindi dari pabrik.

Team Gakkum dari Propinsi dan KLHK juga setiap waktu bisa melakuan pemeriksaan, sehingga
pabrik dan kebun khususnya hal2 mengenai lingkungan agar dapat dilaksanakan dg disiplin .

Housekeeping dan kebersihan harus selalu dan senantiasa di jaga di semua lingkungan pabrik
dan kebun. Biasakan Selalu Kebersihan dalam areal kerja.

Ukuran Gudang Limbah B3 yang kurang besar supaya segera di koordinasikan utk segera
diperbesar sesuai standard. Tks

1. Mohon dapat disiapkan kondisi lapangan dan dokumen terkait aspek Lapangan :

a. Penaatan dalam Pengendalian Limbah Cair di Pabrik baik LCPKS

maupun Limbah cair ikutan lainnya. Tidak ada pembuangan LCPKS dan
Limbah cair lainnya dari Pabrik (sedimen, lindi fibre, Air Lindi EBA dll)
yang langsung ke Badan Penerima sungai/Parit, namun ke Kolam Limbah.

b. Penataan dalam pengelolaan aplikasi LCPKS ke lahan (LA).

Dipastikan tidak ada Bypas dalam Aplikasi LA, dan aplikasi yang
meluber ke parit atau sungai, serta aplikasi melebihi luasan Ijin LA.

c. Pengelolaan Limbah B3 baik di Pabrik dan Kebun sesuai Peraturan

dan SOP. Tidak ada tetesan atau tumpahan LB 3 . Perbaikan sarana TPS
LB 3 jika kurang memadai

d. Perbaikan sarana pengendalian Emisi Udara dari Boiler dan Genset

e. Tidak ada aplikasi JJK di sempadan sungai ataupun ceceran JJK

yang jatuh di sekitar atau dekat sungai.

f. Kondisi Houskeeping Pabrik di dalam areal maupun di workshop,

gudang serta lokasi EBA, TPS LB3 dll

g. Kondisi Housekeeping di Sekitar Kantor Besar estate beserta areal

Workshop, Gudang Central, Gudang Pupuk, Gudang Pestisida, TPS LB 3

h. Papan papan amaran baik di lokasi lokasi penting dan beresiko B3

dan LB 3 mohon diperhatikan dan dilengkapi

2. Persiapan kondisi dokumen dan pelaporan, termasuk :

 a. Kelengkapan Log book LB 3

b. Data debit dan pH LCPKS harian dan pemantauan LCPKS bulanan

di Pabrik

c. Data pelaporan RKL RPL beserta hasil Uji LAB (SHU dll)

Jika ada permasalahan yang perlu dukungan Tim EPM - PCDV kami siap
