Jika memungkinkan, ahli radiologi harus menilai indikator positif dari perbedaan ini
sebagai bagian dari pemeriksaan awal. Pada anak-anak tanpa diagnosis histologis sebelumnya,
ahli radiologi harus berhati-hati untuk menegaskan diagnosis spesifik terhadap temuan
radiologis karena dapat terjadi tumpang tindih yang cukup besar antara penyakit radang usus
dan kondisi ini.
Pencitraan
Ada data terbatas tentang strategi pencitraan yang optimal pada anak-anak ini. Pendekatan
pencitraan standar yang digunakan pada anak-anak yang lebih tua, remaja dan orang dewasa
mungkin tidak sesuai . Enterografi / enteroclysis resonansi magnetik (MR) sulit karena anak-anak
ini sering membutuhkan anestesi umum yang menghalangi distensi luminal konvensional

abdomen harus dilakukan sebagai bagian dari pemeriksaan awal untuk menilai diagnosis banding
dan komplikasi penyakit primer. Pemeriksaan dapat dianggap lengkap jika ileum terminal dan
sebagian besar kolon telah divisualisasikan dan loop usus telah dinilai di keempat kuadran
abdomen. Jika faktor pasien seperti distress atau gas usus menghalangi penilaian ini, maka
pemeriksaan tidak lengkap dan tes lain seperti enterografi MR harus dipertimbangkan. Ingatlah
bahwa pemeriksaan ultrasonografi pada usus besar bagian distal dan rektum sulit dilakukan.

Tomografi terkomputasi
Jika pencitraan cross-sectional dianggap perlu, pertimbangan juga harus diberikan pada enterografi
computed tomography (CT) sebagai pemeriksaan usus satu kali jika hal ini dapat dicapai tanpa memerlukan
anestesi umum atau sedasi. Dosis radiasi yang timbul dari CT abdomen terbukti serupa dengan yang terjadi
pada follow-through usus halus dalam makalah tahun 2008 oleh Gaca et al.. Dengan CT modern, dosis
radiasi terus menurun, dan kemungkinan besar dalam dekade berikutnya perbandingan antara fluoroskopi dan
CT menjadi lebih menguntungkan terhadap CT. Kita tidak boleh mengabaikan penggunaan CT dalam

Fluoroskopi

menyaring keberadaan penyakit. Hal ini penting, karena dengan didapatkannya penyakit usus halus
dapat membantu membedakan penyakit Crohn dari kolitis ulseratif. Ini merupakan pendapat dari
ESPR bahwa USG usus halus harus menjadi pilihan diawal pencitraan usus halus. Ini dikarenakan
adanya tantangan dalam melakukan pencitraan cross-sectional yang membutuhkan distensi luminal
pada usia-usia di mana anestesi umum diperlukan. Pencitraan cross-sectional harus dipertimbangkan
dalam skenario berikut:
-Dimana USG tidak meyakinkan atau non-diagnostik, dan
-Dimana USG normal dan ada kecurigaan klinis yang tinggi terhadap penyakit usus halus.
Pilihan pencitraan bergantung pada institusi terkait. Kasus-kasus ini kemungkinan besar
akan dikelola diperguruan tinggi besar dengan keahlian troenterologi gas pediatrik khusus serta
spesialisasi terkait seperti ahli anestesi pediatrik. Kolaborasi yang erat dengan rekan anestesi sangat
diperlukan, mengingat risiko yang ada pada saat pemberian cairan intraluminal sambil dibius. MR
Enterografi direkomendasikan jika terdapat ahli anestesi. Ini memberikan studi bebas radiasi dengan

Abstract
Very early onset inflammatory bowel disease (VEO-IBD) is defined as disease presenting before the age of 6. These children
require a tailored imaging approach because conventional imaging studies can be difficult to perform at such a young age. Unlike
inflammatory bowel disease in older children and adults, colonic disease predominates in VEO-IBD, and small-bowel disease is
rare. Distinguishing Crohn disease from ulcerative colitis is challenging both clinically and on histology. Radiology offers the
greatest utility for detecting small-bowel disease because it helps to distinguish the two main disease entities and guide clinical
management. Small-bowel ultrasound is recommended as the first-line investigation because it requires relatively little prepara-
tion, is readily available and is generally well tolerated in young children. We present these recommendations, based on the
