BANTUAN HIDUP DASAR
A. PENDAHULUAN
Resusitasi Jantung Paru (RJP) atau Cardiac Pulmonary Resusitation (CPR)
adalah teknik penyelamatan hidup yang bermanfaat pada banyak
kedaruratan, seperti serangan jantung, tenggelam, dimana pernafasan dan
denyut jantung seseorang berhenti. Ketika jantung berhenti, tiadanya
darah teroksigenasi dalam beberapa menit dapat menyebabkan kerusakan
otak yang tidak dapat diperbaiki. Kematian terjadi dalam 8 - 10 menit.
Perhitungan terhadap waktu adalah penting saat menolong pasien tidak
sadar yang tidak bernafas.
Pada tahun 1950, Peter Safar memperkenalkan nafas mulut ke mulut, bidan
mersesusitasi neonates. Pada tahun 1960, Kouwenhoven dkk mengenalkan
kompresi dada. selanjutnya Peter Safar memperkenalkan kombinasi keduanya,
sebagai dasar RJP yang saat ini digunakan. Yaitu yang dibutuhkan hanya dua
tangan.
Pemberian bantuan hidup dasar sangat diperlukan pada klien yang sedang
mengalami kegawadaruratan yaitu henti nafas dan atau henti jantung yang
bertujuan untuk mencegah berhentinya sirkulasi atau berhentinya respirasi dan
untuk memberikan bantuan eksternal terhadap sirkulasi dan ventilasi dari
korban yang mengalami henti jantung atau henti nafas.
B. TUJUAN PEMBELAJARAN
Mahasiswa dapat melakukan penatalaksanaan RJP. Tujuan khusus :
1. Mahasiswa dapat mengetahui indikasi RJP.
2. Mahasiswa mengetahui cara memeriksa dan mengatasi kegawatan C-A-B
3. Mahasiswa mengetahui jenis terapi yang diberikan sesuai kasus (secara
konsep).

D. PROSEDUR
Idealnya, RJP terdiri dari dua komponen : kompresi dada
dikombinasi dengan pernafasan bantuan mulut kemulut.Keterangan :
Perbandingan 30:2 antara lain juga untuk melatih anda melakukan tiupan
nafas buatan. Kompresi dada tapa diikuti tiupan dianjurkan AHA bagi orang
tidak atau kurang terlatih, tetap bisa life saving. Sejak 2015 AHA
menerapkan urutan CAB ( Compression-Airway-Breathing ) dengan alasan
bahwa masih ada sisa oksigen dari nafas terakhir diparu-paru dan aliran
darah, waktu yang terbuang dikala membuka jalan nafas, dan pemberian
nafas mouth to mouth yang sulit dilakukan oleh orang yang belum terlatih,
kecuali bagi bayu baru lahir karena biasanya penyebab arrest adalah asfiksia.

Seorang wanita, usia 60 Tahun dirawat di ruang CVCU dengan diagnosa medis
CHF, tampak terpasang monitor. Dari haemodinamik diketahui gambaran EKG
monitor sinus rhytm TD. 100/60 mmHg, HR 68 x/mnt, RR 24 x/mnt, sat O2 97 %.
Pada saat overran ganti sip dinas didapatkan pasien henti nafas dengan gambaran
EKG Vibrilasi Ventrikel.
A. PENDAHULUAN
Kejadian mati mendadak masih merupakan penyebab kematian utama baik
di Negara maju maupun negara berkembang seperti di Indonesia. Henti
jantung (cardiac arrest) bertanggungjawab terhadap 60 % dari angka
kematian penderita dewasa yang mengalami penyakit jantung koroner
(PJK). (PERKI, 2010)
B. TUJUAN PEMBELAJARAN
1. Mahasiswa mampu melakukan bantuan hidup lanjut pada kondisi henti
jantung
2. Mahasiswa mampu melakukan bantuan hidup lanjut pada kondisi
bradikardi
3. Mahasiswa mampu melakukan bantuan hidup lanjut pada kondisi
takikardi

Hal-hal yang perlu diperhatikan dalam memberikan Bantuan Hidup
Jantung Lanjut:
Kompresi kuat dan cepat (100-120x/mnt, kedalaman 5-6 cm
Penghentian kompresi seminimal mungkin
1 siklus RKP = 30 kompresi & 2 pernapasan, 5 siklus = 2 mnt
Hindari hiperventilasi
Bebaskan jalan napas
Usahakan pasang ET
Setelah ET terpasang, lakukan kompresi terus, berikan pernapasan 8-
10x/menit tanpa menghentikan kompresi
Cek irama setiap 2 menit Usahakan setiap 2 menit ganti petugas
kompresi
Menghentikan kompresi hanya pada saat cek irama & saat memberikan
shock

Managing Bradycardia
The ACLS Bradycardia Algorithm outlines the steps for assessing and managing a
patient who presents with symptomatic bradycardia. It begins with the decision
that the patient's heart rate is < 60 bpm and that is the reason for the patient’s
symptoms.
Steps
1. Decision: Heart rate is < 60 bpm and is symptomatic.
2. Assess and manage the patient using the primary and secondary surveys:
o Maintain patent airway.
o Assist breathing as needed.
o Administer oxygen if oxygen saturation is less than 94% or the
patient is short of breath.
o Monitor blood pressure and heart rate.
o Obtain a 12-lead ECG.
o Review patient's rhythm.
o Establish IV access.
o Complete a problem-focused history and physical exam.
o Search and treat possible contributing factors.
3. Answer two questions to help you decide if the patient's signs and
symptoms of poor perfusion are caused by the bradycardia (see Figure 2).
o Are the signs or symptoms serious, such as hypotension,
pulmonary congestion, dizziness, shock, ongoing chest pain,
shortness of breath, congestive heart failure, weakness or fatigue,
or acute altered mental status?
o Are the signs and symptoms related to the slow heart rate?
4. There may be another reason for the patient’s symptoms other than the
slow heart rate.
5. Decide whether the patient has adequate perfusion. The treatment
sequence is determined by the severity of the patient's clinical
presentation.
o If perfusion is adequate, monitor and observe the patient.
o If perfusion is poor, move quickly through the following actions:
Prepare for transcutaneous pacing. Do not delay pacing. If
no IV is present pacing can be first.
Consider administering atropine 0.5 mg IV if IV access is
available. This may be repeated every 3 to 5 minutes up to
3mg or 6 doses.
If the atropine is ineffective, begin pacing.
Consider epinephrine or dopamine while waiting for the
pacer or if pacing is ineffective.
Epinephrine 2 to 10 µg/min
Dopamine 2 to 20 µg/kg per minute
Progress quickly through these actions as the patient could be in pre-cardiac arrest
and need multiple interventions done in rapid succession: pacing, IV atropine, and
infusion of dopamine or epinephrine.

The New 2010 guidelines for ACLS (Advanced cardiac life support)were change
last October 2010 by American Heart Association. Every 5 years they evaluates
the existing guidelines base upon the data collected within previous 5 years to
determine if changes need to be made to improve the effectiveness of lifesaving
procedure. Therefore the New 2010 ACLS guidelines are also the guideline for
the next 4 years .

A-B-C to C-A-D
The ABCs alphabet that a lot of nurses know has changed to CAB- as in
Circulation first then Airway then Breathing ratio is still 30:2
compressions:breaths. Although ventilations are an important part of resuscitation,
evidence shows that compressions are the critical element in adult resuscitation.
Compressions are often delayed while providers open the airway and deliver
breaths. If a pulse is not detected within 10 seconds, do start compressions
without further delay.
