Case 1

Bayi B, anak ke dua, perempuan, umur 6 hari anak kedua, dibawa ke poliklinik RS D ok
tampak kuning.(1) Bayi terlihat kuning oleh ibunya ketika usia 3 hari.(1) Bayi B
lahir spontan ditolong bidan, segera menangis BB 2800gr, PB 49 cm.Kuning terlihat dari
kepala sampai pergelangan kaki Bayi dirawat di ruang perinatologi RS D. Keluhan kuning
tidak disertai dengan panas, kejang dan penurunan kesadaran. Buang air besar tidak
tampak seperti dempul dan buang air kecil tidak tampak berwarna teh pekat.
ANC di bidan 4x , BB naik 15 kg. Tidak aa keluhan selama hamil. Riwayat kelainan darah
dalam keluarga tidak ada. Golongan darah ibu adalah A sedangkan golongan 1 darah ayah
adalah O rhesus tidak diketahui. Bayi mendapatkan imunisasi Hepatitis B pada umur 1 hari.
Kakak ada riwayat kuning dan disinar

Pemeriksaan Fisik:

Keadaan umum: Alert, menangis kuat, gerak aktif, Lingkar kepala 33 cm; Lingkar dada:
31cm. Laju denyut jantung:110x/menit, Laju napas:48 x/menit, regular, tipe abdominal.
Suhu:36,9oC Kepala:cephal hematome +) Sklera ikterik +/+, Abdomen:Datar, ikterik (+), tali
pusat terawat, palpasi:lembut, hepar dan lien tidak teraba. Pemeriksaan Kremer 4

Kadar Bilirubin total 15,2 mg/dl, Bil Direk 0,4 mg/dl. (1) Gol darah bayi A

Case 2

A baby girl was born healthy, weigh 3500 gram and length 502cm. Head circumference 36cm. She is
breastfed by her mother and dischargted home at the age of 16 hours with instruction to return to
the paediatrician’c clinic 2 days after discharge. On the fifth day, she is brought to the clinic because
the mother noticed that she was increasingly jaundiced over the previous 2 days and had nursed
poorly. She had refused breastfeeding for the past 6 hours, was weak and loosen weight

PE: lethargic, extremely jaundiced. RR 60bpm, temperature 36 0C. HR 160bpm. Sclera icteric . Heart
and Lungs: normal. Abdominal wall: No. specific abnormalities. Extremities: jaundice

Laboratory findings:

Hb:14,0g% ( ref 13-18g%) WBC:18500/mm3 (10,000-30.000/mm3) 42%. PCV::42%; Platelet

256.000/mm3. Total bil: 29,5 mg/dl. Direct Bil:0,40 mg/dl, Indirect Bil: 29,,1 mg/dl. Mother’s blood
group Type A, Baby:O. Coombs test negatif
