• Penyakit Addison adalah gangguan yang terjadi ketika kelenjar adrenalin
tidak bekerja secara maksimal, sehingga tubuh tidak menghasilkan cukup
hormon-hormon penting.

• Dua kelenjar adrenal kecil ini terletak di atas ginjal. Kelenjar adrenal
menghasilkan hormon kortisol dan aldosteron. Dalam penyakit Addison,
kelenjar adrenal menghasilkan sangat sedikit sekali hormon kortisol dan
aldosterone. Tanpa hormon ini, garam dan air dalam tubuh tak bisa
dikeluarkan melalui urin, yang akan mengakibatkan jatuhnya tekanan darah
hingga sangat rendah. Di saat yang bersamaan, hal tersebut akan
meningkatkan jumlah potasium hingga tingkat yang membahayakan.
ETIOLOGI
Ketidakmampuan memproduksi hormon kortisol adekuat disebut juga
insufiensi adrenal terjadi karena berbagai hal. Keadaan tersebut disebabkan
oleh gangguan di kelenjar itu sendiri (insufisiensi adrenal primer) atau
gangguan sekresi hormon ACTH oleh kelenjar hipofisis (insufisiensi adrenal
sekunder).Insufisiensi adrenal dibagi menjadi 3 tipe, tergantung dari dimana
terjadinya masalah pada kelenjar hipothalamik pituitary-adrenal dan seberapa
cepat turunnya hormon hormon tersebut. Ketiga tipe tersebut yaitu :
● Chronic primary adrenal insufisiency ( Addison disease )
Sebagian besar penyakit Addison disebabkan oleh destruksi korteks adrenal yang disebabkan oleh sistem imun
diri kita sendiri. Penyebab insufiensi adrenal primer yaitu infeksi kronis, metastasis keganasan, dan pengangkatan
kelenjar adrenal. Insufisiensi adrenal kronis terjadi ketika kelenjar adrenal gagal untuk mengeluarkan hormon
dalam jumlah yang adekuat, untuk memenuhi kebutuhan fisiologis, walaupun ACTH keluar dari kelenjar pituitari.

● Chronic Secondary Adrenal Insuficiency

Bentuk penyakit Addison ini merupakan penanda kurangnya hormon ACTH, yang dapat disebabkan kurangnya
produksi hormon kortison kelenjar adrenal tapi produksi hormon aldosteron normal. Penyebab lain insufisiensi
adrenal sekunder adalah pengangkatan kelenjar adrenal atau tumor benigna kelenjar adrenal, adanya hormon
ACTH yang diproduksi oleh sel tumor kelenjar hipofisis (sindrome Cushing).

● Acute Adrenal Insuficiency ( Krisis Addison)

Insufisiensi adrenal akut terjadi bila mana respon adrenal menurun karena sesuatu sebab atau gangguan
pelepasa ACTH akibat kerusakan hipofisis atau terapi kortikosteroid. Gejala klinis ditentukan oleh keadaan
penyakit yang mendasarinya. Keadaan umum yang buruk, disertai mual, muntah diare dan hipotensi dapat
berlanjut sampai timbul syok dan kematian. Insufisiensi adrenal akut juga dapat terjadi akibat stres, infeksi ringan.
 PATOFISIOLOGI ADDISON DISEASE
● Kekurangan produksi glukokortikoid atau mineralokortikoid pada kelenjar adrenal
mengakibatkan terjadinya insufisiensi adrenalkortikal, yang disebabkan oleh
disfungsi dari korteks adrenal. Jika fungsi pada kedua korteks adrenal 90% maka
akan menghasilkan manifestasi klinis insufisiensi adrenalkortik antara lain destruksi
dari kelenjar seperti terdapat pada kondisi idiopatik dan kondisi invasif dari suatu
penyakit. Hal ini menyebabkan terjadinya kronisitas dari suatu insufisiensi adrenal.
Fase awal dari destruksi pada kelenjar di adrenakortikal adalah terjadi pengurangan
dari cadangan adrenal,

● Dengan berkurangnya sekresi dari kortisol, level plasma dari ACTH meningkat akibat
dari penurunan umpan balik negatif yang menginhibisi sekresi ACTH. Sebagai
akibatnya peningktatan level plasma dari ACTH pada awal merupakan kondisi
sangat optimal dalam mengsekresi cadangan dari adrenokortikal.
GEJALA DAN TANDA
1. Cepat lelah
2. Lemah badan
3. Anoreksia
4. Mual-mual dan muntah
5. Diare
6. Hipotensi dan hiponatremia
7. Hiperkalemia
8. Badan panas
9. Nyeri abdomen
10. Nyeri pinggang
DIAGNOSIS
Seiring penurunan bertahap kadar kortisol
akan timbul gejala di antaranya kelemahan
secara umum dan mudah lelah, bagian kulit
tertentu menjadi gelap di daerah kulit yang tidak
terpapar sinar matahari seperti ketiak, lipatan
tangan, bagian dalam mulut, dan daerah tekanan
seperti siku. Hal ini disebabkan oleh perubahan
hormon yang mempengaruhi sel pigmen kulit.
Gejala lain adalah tekanan darah rendah pada
saat berdiri mengakibatkan pusing, tidak nafsu
makan, penurunan berat badan, mual dan
muntah sepanjang waktu, nyeri abdomen hilang
timbul, diare atau konstipasi yang hilang timbul,
kram dan nyeri otot.
PEMERIKSAAN PENUNJANG
Tes Skrining Sensitif yaitu,
pengukuran kadar kortisol
serum dari sampel darah
yang diperoleh di pagi hari

Tes toleransi insulin adala

sensitif untuk insufisiensi
adrenal. Tes ini melibatkan
stres hipoglikemik untuk
menginduksi produksi
kortisol.

gangguan jalur produksi
kortisol
