Fraktur femur atau patah tulang paha
adalah rusaknya kontinuitas tulang pangkal
paha yang disebabkan oleh trauma
langsung, kelelahan otot, dan kondisi
tertentu, seperti degenerasi tulang atau

DEFINISI tulang femur yang disebabkan oleh
benturan atau trauma langsung
maupun tidak langsung. Fraktur femur
juga didefinisikan sebagai hilangnya
kontinuitas tulang paha, kondisi
fraktur femur secara klinis bisa berupa
fraktur femur terbuka yang disertai
adanya kerusakan jaringan lunak (otot,
kulit, jaringan saraf dan pembuluh
darah) dan fraktur femur tertutup
yang dapat disebabkan oleh trauma

 ETIOLOGI
1. Fraktur akibat peristiwa trauma Sebagian fraktur disebabkan oleh kekuatan yang tiba-
tiba berlebihan yang dapat berupa pemukulan, penghancuran, perubahan tempat.
2. Akibat kelelahan atau tekanan retak dapat terjadi pada tulang seperti halnya pada
logam dan benda lain akibat tekanan berulang.
3. Fraktur patologik karena kelemahan pada tulang fraktur dapat terjadi oleh tekanan
yang normal bila tulang tersebut lunak (misalnya oleh tumor) atau tulang-tulang sangat
rapuh.

Menurut Mutaqqin penyebab fraktur femur antara lain:

1. Fraktur Femur terbuka disebabkan oleh trauma langsung pada paha.
2. Fraktur Femur tertutup disebabkan oleh trauma langsung atau kondisi tertentu,
seperti degenerasi tulang (osteoporosis) dan tumor atau keganasan tulang paha
yang menyebabkan fraktur patologis

PATOFISIOLOGI
Fraktur dibagi menjadi fraktur terbuka dan fraktur tertutup.
Tertutup bila tidak terdapat hubungan antara fragmen tulang
dengan dunia luar. Sedangkan fraktur terbuka bila terdapat
hubungan antara fragmen tulang dengan dunia luar oleh karena
perlukaan di kulit. Sewaktu tulang patah perdarahan biasanya
terjadi di sekitar tempat patah ke dalam jaringan lunak sekitar
tulang tersebut, jaringan lunak juga biasanya mengalami
kerusakan. Reaksi perdarahan biasanya timbul hebat setelah
fraktur. Sel- sel darah putih dan sel anast berakumulasi
menyebabkan peningkatan aliran darah ketempat tersebut
aktivitas osteoblast terangsang dan terbentuk tulang baru
umatur yang disebut callus. Bekuan fibrin direabsorbsidan sel- sel
tulang baru mengalami remodeling untuk membentuk tulang
sejati.

PENATALAKSANAAN MEDIS

Tindakan penanganan fraktur dibedakan berdasarkan bentuk dan lokasi serta usia. Berikut
adalah tindakan pertolongan awal pada penderita fraktur :
1. Kenali ciri awal patah tulang memperhatikan riwayat trauma yang terjadi karena
benturan, terjatuh atau tertimpa benda keras yang menjadi ala
2. Jika ditemukan luka yang terbuka, bersihkan dengan antiseptik dan bersihkan
perdarahan dengan cara dibebat atau diperban.
3. Lakukan reposisi (pengembalian tulang ke posisi semula) tetapi hal ini tidak boleh
dilakukan secara paksa dan sebaiknya dilakukan oleh para ahli dengan cara operasi oleh
ahli bedah untuk mengembalikan tulang pada posisi semula.
4. Pertahankan daerah patah tulang dengan menggunakan bidai atau papan dari kedua
posisi tulang yang patah untuk menyangga agar posisi tetap stabil.
5. Berikan analgetik untuk mengaurangi rasa nyeri pada sekitar perlukaan.
6. Beri perawatan pada perlukaan fraktur baik pre operasi maupun post operasi.
