◉ Umumnya senyawa bioaktif dalam obat herbal merupakan metabolit sekunder

yang berperan sebagai berbagai sifat farmakologis
◉ Obat herbal ini bisa digunakan sebagai zat obat modern. Contohnya seperti morfin
dari Papaver somni ferum untuk pengobatan nyeri, kolkisin dari Colchicum musim
gugur untuk pengobatan penyakit perikardial
◉ Beberapa penelitian telah melaporkan aktivitas antibakteri dari berbagai herbal.
namun, bukti yang cukup untuk mendukung senyawa aktif dan efektivitasnya
untuk antibakteri dan antibiofilm, terutama pada patogen oral masih terbatas.

◉ Ketidakseimbangan bakteri dalam biofilm gigi dapat menyebabkan karies gigi dan periodontitis
◉ Dosis tinggi agen antimikroba diperlukan untuk menghilangkan biofilm dalam pendekatan klinis
◉ penggunaan obat kumur chlorhexine (CHX) jangka panjang dapat menyebabkan efek samping; gangguan sensasi rasa,
perubahan warna coklat pada dorsum lidah dan lesi deskuamasi mukosa mulut
◉ Obat herbal dapat menjadi alternatif sebagai agen antibakteri untuk mencegah penyakit infeksi rongga mulut. Studi yang
diperluas ini adalah untuk mengevaluasi aktivitas antibakteri dan antibiofilm dari 12 ekstrak etanol ramuan tradisional

Alpinia galanga, Curcuma longa, Curcuma zedoaria, Piper betle, Piper chaba, Piper nigrum, Piper sarmentosum, Mentha
cordifolia, Ocimum africanum, Ocimum basilicum, Ocimum sanctum dan Zingiber officinale, dan untuk mengetahui senyawa
aktif dari ekstrak yang paling efektif.

◉ Persiapan Ekstrak Ramuan Obat

Dua belas herbal dibeli dari pasar lokal di Thailand

Tanaman kering dimaserasi dengan etanol selama 3 hari

Disaring melalui kertas saring Whatman No. 4. Setiap filtrat dikeringkan menggunakan rotary
evaporator pada suhu 40oC dan disimpan pada -20oC
Untuk pengujian, 0,1 mg setiap ekstrak kering awalnya dilarutkan dengan 100 ml dimetil sulfoksida
(DMSO) kemudian disesuaikan menjadi 1 mL dengan menambahkan akuades steril

◉ Strain Bakteri Dan Kondisi Pertumbuhan

Sebanyak 7 mikroorganisme mulut digunakan dalam penelitian ini

5 bakteri kariogenik Gram positif : Enterobacter faecalis ATCC 19433 (Ef), Lactobacillus
fermentum ATCC 14931 (Lf), Lactobacillus salivarius ATCC 11741 (Ls), Streptococcus sobrinus
ATCC 33478 (Ss) and Streptococcus mutans ATCC 25175 (Sm)
2 bakteri periodontopatogen gram negative : Aggregatibacter actinomycetemcomitans ATCC 33384
(Aa) and Fusobacterium nucleatum ATCC 25586 (Fn)
Mikroorganisme dipelihara pada brain heart infusion agar (BHA) dengan 5% (v/v) darah untuk
bakteri fakultatif, dan ditambah dengan 0,5% (b/v) ekstrak ragi, hemin dan vitamin K untuk bakteri
anaerob. Strain ditanam di bawah aerobik atau anaerobik (10% H2, 10% CO2 dan 80% N2)

Uji Anti Bakteri
◉ Metode Difusi Sumur Agar

100 µL, setara dengan 107 CFU/mL, dicampur dengan 20 mL BHA cair hangat

Campuran tersebut kemudian dituang ke dalam pelat dengan cawan logam berdiameter 6 mm

Setelah BHA memadat, cawan logam diangkat dan sumur ditambahkan dengan 100 ml masing-
masing ekstrak tumbuhan

Plat diinkubasi pada suhu 37oC selama 24 jam.

DMSO 10% diambil sebagai kontrol negatif. Aktivitas antimikroba masing-masing ekstrak
tumbuhan ditentukan dengan mengukur diameter zona hambat dalam milimeter.

Uji Anti Bakteri
◉ Metode Mikrodilusi Kaldu

Larutan stok 10% dari masing-masing ekstrak diencerkan dalam kaldu brain heart infusion
broth (BHI). Pengenceran dilakukan sebanyak 2 kali untuk mendpatkan konsentrasi 0,02
hingga 25 mg/mL pada volume total 100 ml per sumur di pelat microtiter 96.

Setiap strain yang diuji (100 µL) yang ditambahkan kedala plat sumur, diinkubasi pada suhu 37 oC.

0,1% (b/v) CHX dan 10% DMSO masing-masing digunakan sebagai kontrol yang tidak diberi
perlakuan

Uji Antibiofilm
◉ Penghambatan Pembentukan Biofilm

Efek dari ekstrak P. Betle pada pembentukan biofilm dari masing-masing strain repesentatif S.
mutans ATCC 25175 dan A. Actinomycetemcomitans ATCC 33384, diperiksa dengan
menggunakan metode mikrodilusi.
Dilakukan pengenceran seri sebanyak dua kali dari ekstrak P. Betle, dengan konsentrasi akhir
berkisar antara 0,02 sampai 25 mg/mL.
Disiapkan suspensi sel dari strain yang diuji. 100 ml (1x106CFU/mL) diinokulasi pada masing-
masing 96-well plate.
1% CHX, PBS, dan ekstrak free medium yang masing-masing digunakan sebagai control positif.
Pembentukan biofilm diukur dengan menggunakan uji perubahan warna MTT (3-[4,5-dimetil-2-
tiazoli]-2,5-difenil-tetra zolium bromide)
Nilai OD yang lebih tinggi menunjukan peningkatan jumlah mikroorganisme yang bertahan
dalam biofilm. Persentase penghambatan dihitung dengan menggunakan persamaan :
