I. PENDAHULUAN
Pertusis atau yang lebih dikenal orang awam sebagai “batuk rejan” atau “batuk 100 hari” merupakan
salah satu penyakit menular saluran pernapasan yang sudah diketahui adanya sejak tahun 1500-an. Penyebab
tersering dari pertusis adalah kuman gram (-) Bordetella pertussis.
Di seluruh dunia insidensi pertussis banyak didapatkan pada bayi dan anak kurang dari 5 tahun..
meskipun anak yang lebih besar dan orang dewasa masih mungkin terinfeksi oleh B.pertussis. Insidensi
terutama didapatkan pada bayi atau anak yang belum diimunisasi.
Dahulu pertusis adalah penyakit yang sangat epidemic karena menyerang bukan hanya negara-negara
berkembang namun juga beberapa bagian dari negara maju, seperti Amerika Serikat, Italia, Jerman. Namun
setelah mulai digalakkannya vaksinasi untuk pertusis, angka kematian bisa ditekan hingga 10/10.000 populasi.
Seiring dengan semakin berkembangnya ilmu pengetahuan dan teknologi, pertusis diharapkan tidak
diketemukan lagi, meskipun ada kasusnya namun tidak signifikan atau kurang.
Dengan mendiagnosa secara dini kasus pertusis, dari gejala klinis,foto roentgen, dan pemeriksaan
penunjang lainnya, diharapkan para klinisi mampu memberikan penanganan yang tepat dan cepat sehingga
derajat penyakit pertusis tidak menimbulkan komplikasi yang lebih lanjut, seperti ensefalopati, Respiratory
distress syndrome, dan penyakit paru-sistemik lainnya.
II. TINJAUAN PUSTAKA
Batasan :
Pertussis artinya batuk yang intensif, merupakan penyakit infeksi saluran pernafasan akut yang dapat
menyerang setiap orang yang rentan seperti anak-anak yang tidak diimunisasi atau pada orang dewasa dengan
kekebalan menurun. Istilah pertussis (batuk kuat) pertama kali diperkenalkan oleh Sydenham pada tahun 1670.
dimana istilah ini lebih disukai dari “batuk rejan (whooping cough)”. Selain itu sebutan untuk pertussis di Cina
adalah “batuk 100 hari”. 1,2,3
Pertussis adalah penyakit yang serius pada anak-anak kecil diseluruh dunia. Pada orang dewasa juga
sering terjadi karier yang asimptomatik atau infeksi yang ringan.
Prevalensi pertussis di seluruh dunia sekarang berkurang karena adanya imunisasi aktif.
Etiologi
Penyebabnya adalah Bordetella pertusis. B. pertussis ini merupakan satu-satunya penyebab pertusis
endemis dan penyebab biasa pertusis sporadis, terutama karena manusia merupaka satu-satunya host untuk
spesies ini. Penyakit serupa- disebut juga a mild pertussis-like illness- juga dapat disebabkan oleh B.
parapertussis(terutama di Denmark, Republik Ceko, Republik Rusia, dan Slovakia) dan B.
bronchiseptica (jarang pada manusia karena merupakan patogen yang lazim pada binatang-kucing dan binatang
pengerat-, kecuali pada manusia dengan gangguan imunitas dan terpapar secara tidak biasa pada binatang).
Kadang-kadang sindroma klinik berupa batuk yang lama dan tidak sembuh-sembuh sehingga susah dibedakan,
juga terdapat pada infeksi adenovirus (tipe 1,2,3, dan 5), Respiratory Syncitial Virus, parainfluenza virus atau
influenza virus, enterovirus dan mycoplasma. 1,3

Epidemiologi
Pertussis adalah satu dari penyakit-penyakit yang paling menular, dapat menimbulkan “attack rate” 80-
100% pada penduduk yang rentan. Di seluruh dunia ada 60 juta kasus pertusis setahun dengan lebih dari
500.000 meninggal. Selama masa pra-vaksin tahun 192-1948, pertusis adalah penyebab utama kematian dari
penyakit menular pada anak di bawah usia 14 tahun di Amerika Serikat. Dilaporkan juga bahwa 50 persen
adalah bayi kurang dari setahun, 75 persen adalah anak kurang dari 5 tahun.1,2,3
Pertusis terutama mewabah di negara-negara berkembang dan maju, seperti Italian, daerah-daerah tertentu
di Jerman dimana cakupan vaksin rendah atau Nova Scatia dimana digunakan vaksin yang kurang poten,
dengan angka insidensi rata-rata mencapai 200-500/100.000 populasi dengan angka kematian 350.000 pada
anak dibawah 5 tahun.2 Di Amerika Serikat sendiri dilaporkan insidensi tertinggi 4500 kasus sejak tahun 1967.
namun setelah hal tersebut, pertusis jarang sekali kasusnya karena sudah lebih di galakkan vaksinasi . 3
Pertusis adalah endemik, dengan ditumpangin siklus endemik setiap 3-4 tahun sesudah akumulasi kelompok
rentan yang cukup besar. Dilaporkan sebagian kasus terjadi dari bulan Juli sampai dengan Oktober. 1,3. Pertusis
sangat menular dengan angka serangan 100% pada individu rentan yang terpajan pada aerosol dengan rentang
yang rapat. Penyebaran terjadi melalui kontak langsung atau melalui droplet yang ditularkan selama batuk.
Dahulu dikatakan bahwa Perempuan terkena lebih sering daripada laki-laki dengan perbandingan 0.9:1 .
Namun dengan laporan terbaru (Farizo, 1992) perbandingan insidensi antara perempuan dan laki-laki menjadi
sama sampai umur dibawah 14 tahun. Sedangkan proporsi anak belasan tahun dan orang dewasa yang terinfeksi
pertusis naik secara bersama samapai 27% pada tahun 1992-1993.
Tanpa reinfeksi alamiah dengan B.pertussis atau vaksinasi booster berulang, anak
yang lebih tua dan orang dewasa lebih rentan terhadap penyakit ini jika terpajan. Sedangkan antibodi dari ibu
secara transplasental pada anak tidaklah konsisten mencegah bayi yang baru lahir terhadap pertussis. Pertussis
pada neonatus yang berat dapat ditemukan dengan gejala-gejala pertussis normal. 3
Patogen :
B. pertussis : kecil, tidak bergerak, cocobacillus gram (-). Terbaik dibiak pada “glycerin-potato-blood
agar media (border-gengou)”. Organisme yang didapat umumnya tipe virulen (disebut fase I). Pasase dalam
kultur dapat merangsang pembentukan varian yang avirulen (fase II, III, dan IV). Strain fase I berperan untuk
penularan penyakit dan menghasilkan vaksin yang efektif. 4

Hanya B. pertussis yang mengeluarkan toksin pertusis (TP), protein virulen utama. B.pertussis juga
menghasilkan beberapa bahan aktif, yang banyak darinya dimaksudkan untuk memainkan peran dalam penyakit
dan imunitas. Aerosol, hemaglutinin filamentosa (HAF), beberapa aglutinogen (FIM2-FIM3), dan protein
permukaannonfimbria 69-kD yang disebut pertaktin (PRN) penting untuk perlekatan terhadap sel epitel bersilia
saluran pernapasan. Sitotoksin trakea, adenilat siklase, dan TP menghambat pembersihan organisme. Sitotoksin
trakea, factor dermonekrotik dan adenilat siklase diterima secara dominant menyebabkan cedera epitel local
yang menghasilkan gejala-gejala pernapasan dan mempermudah penyerapan TP. 2,3,4
TP mempunyai 2 sub unit, yaitu A dan B. TP (B) akan berikatan dengan reseptor pada sel taret dan
mengaktivasi TP(A) pada membran sel yang merangsang pengeluaran enzim. TP akan merangsang pengeluaran
Adenosin Diphosphate (ADP) sehingga akan mempengaruhi fungsi dari leukosit, limfosit, myocardial sehingga
bermanifestasi peradangan saluran napas dengan hyperplasia kelenjar lymph peribronchial dan meningkatkan
produksi mucus yang akan menutupi permukaan silia. Yang pada akhirnya bias mengarah ke komplikasi
bronchopneumonia, infeksi sekunder bakteri lain (ex: Pneumococcus, Haemophilus influenzae, S.aureus,
S.pyogenes), sianosis karena apnea dan ventilation perfusion mismatch. 2,3
Patologi :
- organisme bermultiplikasi pada epitel yang bersilia dan menghasilkan faktor-faktor virulen (termasuk
toksin)
- Ada bendungan dan infiltrasi mukosa oleh sel-sel limfosit dan leukosit PMN, dan hasil hasil peradangan
dalam lumen bronki. Pada awalnya terjadi hiperplasia limfoid peribronkial. Terjadi bronkopneumonia
dengan nekrosis dan deskuamasi epitel permukaan bronki.
- Obstruksi bronkial dan atelektasis terjadi karena penumpukan sekresi mukus. Dapat pula timbul
bronkiektasi.
- Perubahan patologis juga ditemukan pada otak dan hati. Dapat ditemukan perdarahan serebral dan atrofi
kortikal yang kemungkinannya karena adanya anoksia. Pada hati dapat ditemukan infiltrasi lemak.
Background
Pertussis, commonly known as whooping cough, is a respiratory tract infection characterized by a paroxysmal cough. It
was first identified in the 16th century. In 1906, Bordet isolated the most common causative organism, Bordetella
pertussis. Bordetella parapertussis has also been associated with whooping cough in humans. Before the advent of
vaccinations, pertussis was a major cause of morbidity and mortality among infants and children. Reported cases of
pertussis decreased by more than 99% after the introduction of pertussis vaccine combined
with diphtheria andtetanus toxoids in the 1940s. However, despite considerable advances in the control of infectious
diseases in children through global immunization programs, pertussis remains a disease of public health concern.
Pathophysiology
Humans are the sole reservoir for B pertussis and B parapertussis. B pertussis is a gram-negative pleomorphic bacillus
that spreads via aerosolized droplets from coughing of infected individuals. B pertussis attaches to and damages ciliated
respiratory epithelium.
Frequency
United States
Since the early 1980s, pertussis incidence has cyclically increased, with peaks occurring every 2-5 years.1 Most cases
occur between June and September. Neither acquisition of the disease nor vaccination provides complete or lifelong
immunity. Protection against typical disease wanes 3-5 years after vaccination and is not measurable after 12 years.
From 1980-2005, the reported incidence of pertussis in the United States dramatically increased across all age
groups. Although the largest increase in pertussis cases has been among adolescents and adults, the annual reported
incidence remained highest among infants younger than one year, at 55.2 cases per 100,000 population. 2,3
International
The worldwide incidence is estimated to include 48.5 million cases and nearly 295,000 deaths per year. 4 The case-fatality
rate among infants in low-income countries may be as high as 4%.
Mortality/Morbidity
Infants born prematurely and patients with underlying cardiac, pulmonary, neuromuscular, or neurologic disease are at
high risk for complications of pertussis (eg, pneumonia, seizures, encephalopathy, death). Older children, adolescents,
and adults often have mild or atypical illness. Approximately one half of adolescents with pertussis cough for 10 weeks or
longer. Complications among adolescents and adults include syncope, sleep disturbance, incontinence, rib fractures, and
pneumonia.
Compared with older children and adults, infants younger than 6 months with pertussis are more likely to have severe
disease, to develop complications, and to require hospitalization. From 2001-2003, 69% of infants younger than 6 months
with pertussis required hospitalization.2
Pneumonia, either from B pertussis infection or from secondary infection with other pathogens, is a relatively common
complication, occurring in approximately 13% of infants with pertussis. 5
CNS complications such as seizures and encephalopathy are less common and are thought to result from severe
paroxysm-induced cerebral hypoxia and apnea, metabolic disturbances such as hypoglycemia, and small intracranial
hemorrhages.
Reported deaths due to pertussis in young infants have substantially increased over the past 20 years. 6,7,8 From 1990-
1999, the case fatality rate was approximately 1% in infants younger than 2 months and less than 0.5% in infants aged 2-
11 months. Pertussis has been reported as a cause of sudden infant deaths.
Race
Data on race were available for 75% of patients younger than 20 years from 1997-2000. 9 Of these patients, 88% were
white, 8% were black, 2% were Asian/Pacific Islander, and 2% were American Indian/Alaska Native. In comparison, the
national population estimates for persons younger than 20 years in 1998 were 79% white, 16% black, 4% Asian/Pacific
Islander, and 1% American Indian/Alaska Native.
Sex
From 1997-2000, among pertussis cases in patients younger than 20 years, males and females were equally affected. 9
Age
From 2001-2003, of patients with pertussis, 23% were younger than 1 year, 12% were aged 1-4 years, 9% were aged 5-9
years, 33% were aged 10-19 years, and 23% were older than 20 years. 5
Clinical
History
Typically, the incubation period of pertussis ranges from 3-12 days. Pertussis is a 6-week disease divided into catarrhal,
paroxysmal, and convalescent stages, each lasting from 1-2 weeks. The 3 stages of disease progression are as follows:
Stage 1: The initial (catarrhal) phase is indistinguishable from common upper respiratory infections with nasal
congestion, rhinorrhea, and sneezing, variably accompanied by low-grade fever, tearing, and conjunctival
suffusion. Pertussis is most infectious when patients are in the catarrhal phase, but pertussis may remain
communicable for 3 or more weeks after the onset of cough.
Stage 2: Patients in the second (paroxysmal) phase present with paroxysms of intense coughing lasting up to
several minutes. In older infants and toddlers, the paroxysms of coughing occasionally are followed by a loud
whoop as inspired air goes through a still partially closed airway. Infants younger than 6 months do not have the
characteristic whoop but may have apneic episodes and are at risk for exhaustion. Posttussive vomiting and
turning red with coughing are common in affected children.
Stage 3: Patients in the third (convalescent) stage have a chronic cough, which may last for weeks.
Older children, adolescents, and adults may not exhibit distinct stages. Symptoms in these patients include uninterrupted
coughing, feelings of suffocation or strangulation, and headaches.
Physical
In patients with uncomplicated pertussis, physical examination findings contribute little to the diagnosis. In all patients with
pertussis, fever is typically absent. Most patients do not have signs of lower respiratory tract disease. Conjunctival
hemorrhages and facial petechiae are common and result from intense coughing.
Causes
B pertussis and B parapertussis are the causative organisms for pertussis infection in humans. Bacteria spread via
aerosolized droplets from coughing of infected individuals. Humans are the sole reservoir for the organisms.
Transmission can occur through direct face-to-face contact, sharing a confined space, or through contact with oral, nasal,
or respiratory secretions from an infected source. Pertussis is highly contagious, with as many as 80% of susceptible
household contacts becoming infected after exposure. Family members or relatives were the suspected source of
infection in 75% of cases.10
Young infants, especially those born prematurely, and patients with underlying cardiac, pulmonary, neuromuscular, or
neurologic disease are at high risk for contracting the disease and for complications.
Adenoviral respiratory infection: Children present with fever, sore throat, and conjunctivitis.)
Mycoplasmal pneumonia: Patients with mycoplasmal infections have more pronounced systemic symptoms, fever
and headache may occur, and rales may be appreciated on chest auscultation.
Chlamydial pneumonia: Young infants with chlamydial infections present with staccato cough, purulent
conjunctival discharge, tachypnea, rales, and wheezing.
Respiratory syncytial virus infection: Patients present with predominantly lower respiratory tract signs (eg,
wheezing, rales).
Workup
Laboratory Studies
Laboratory confirmation of pertussis is difficult and delayed. Therefore, clinicians need to make the diagnosis of pertussis
presumptively in patients with a history of intense paroxysmal coughing with or without whooping, color changes,
posttussive vomiting, incomplete or absent pertussis vaccination, and finding of lymphocytosis on laboratory examination.
Imaging Studies
Imaging studies typically add little to the diagnosis of pertussis but should be obtained when clinically indicated, based on
examination or if the patient requires supplemental oxygen.
Chest radiography may reveal perihilar infiltrates or edema with variable degrees of atelectasis. Consolidation is indicative
of secondary bacterial infection or, rarely, pertussis pneumonia. Occasionally, pneumothorax,pneumomediastinum, or air
in the soft tissues may be seen.
