Pendahuluan: Prevalensi periodontitis di Indonesia menempati peringkat kedua
penyakit gigi dan mulut yaitu sebesar 60%. Inflamasi pada periodontitis menyebabkan destruktif tulang alveolar, terbentuknya poket dan resesi gingiva. Resorpsi tulang alveolar dipengaruhi oleh Osteoclast Activating Factor (OAF) serta produk komplemen aktivator yang terdiri dari sitokin, IL-1, TNF-α, IL-6 dan IL-11. Terapi periodontitis pada saat ini dilakukan dengan root scalling yang hanya mengeliminasi bakteri periopatogen dan mengontrol inflamasi saja. Oleh karena itu, diperlukan terapi periodontitis dengan upaya regenerasi jaringan periodontal, terutama tulang alveolar. Tujuan Penelitian: Mengkaji sinergisme nanofluoride dan polifenol Theobroma cacao dalam inhibisi OAF melalui Receptor Activator Nuclear Kappa β Ligand (RANKL) sebagai terapi periodontitis. Tinjaun Pustaka: Kakao (Theobroma cacao L.) merupakan salah satu jenis tanaman unggulan di Kabupaten Jember dengan presentase 29,4%. Banyak masyarakat berpikir kakao merupakan makanan perusak gigi, padahal kakao mengandung polifenol dengan persentase terbanyak yaitu 12-18%. Biji kakao dengan senyawa aktif tannin, katekin, saponin, dan flavonoid. Pembahasan: Senyawa polifenol yang terkandung pada biji kakao memiliki kemampuan sebagai antioksidan, antibakteri, dan antiinflamasi melalui efek penghambat pada jalur metabolisme asam arakidonat dan pembentukan prostaglandin. Nanofluoride berperan dalam mempengaruhi aktivitas osteoblas dan osteoklas tulang. Pada terapi periodontitis, kombinasi polifenol kakao dan nanofluoride dapat berinteraksi dengan mineral matriks tulang. Nanofluoride dapat mempengaruhi remodeling tulang melalui RANKL dan osteoprogenitor (OPG). Jadi, kombinasi nanofluoride dan polifenol kakao dapat menghambat resorpsi tulang alveolar. Kesimpulan: Sinergisme nanofluoride dan polifenol kakao dalam inhibisi OAF melalui mekanisme RANKL berpotensi sebagai salah satu alternatif terapi periodontitis. SYNERGISM OF NANOFLUORIDE AND THEOBROMA CACAO POLYPHENOLS IN OAF INHIBITION THROUGH RANKL AS PERIODONTITIS THERAPY Malihatul Rosidah1, Elindah Ayunin2, Azizarohaina Dirza3 Background: The prevalence of periodontitis in Indonesia ranks second in oral and dental disease at 60%. Inflammation in periodontitis causes destructive alveolar bone, pocket formation, and gingival recession. Resorption of alveolar bone is affected by Osteoclast Activating Factor (OAF) and complement activator products such as cytokines, IL-1, TNF-α, IL-6 and IL-11. Current periodontitis therapy is root scaling which is only eliminates periopathogen bacteria and controlling inflammation. Therefore, periodontitis therapy is needed to regenerate periodontal tissue, especially alveolar bone. Purpose: Review the synergism of nanofluoride and Theobroma cacao polyphenols in OAF inhibition through Receptor Activator of Nuclear Factor Kappa β Ligand (RANKL) as a periodontitis therapy. Literature Review: Cacao (Theobroma cacao L.) is one of superior types of plants in Jember Regency with a percentage of 29.4%. Many people think cacao is a tooth-destroying food, even though cacao contains polyphenols with the highest contribution 12-18%. Cacao beans with active compounds of tannins, catechins, saponins, and flavonoids. Discussion: The polyphenol have antioxidant, antibacterial and anti-inflammatory abilities through inhibitory effects on the arachidonic acid metabolic pathway and prostaglandin formation. Nanofluoride plays a role in influencing osteoblast and osteoclast bone activity. In this periodontitis therapy, combination of cacao polyphenols and nanofluoride can interact with bone matrix minerals. Nanofluoride can affect bone remodeling through RANKL and osteoprogenitor (OPG). Thus, the combination of nanofluoride and cacao polyphenols can inhibit alveolar bone resorption. Conclusion: Synergism of nanofluoride and cacao polyphenols in the inhibition of OAF through the RANKL mechanism has potential as an alternative therapy for periodontitis.
