2.00 p.m. Take over from morning shift to afternoon shift
nurses and does patient’s round. General
condition of patient is stable, patient is sleeping
soundly
Serah terima dari shift pagi ke perawat shift
sore dan melakukan kunjungan pasien. Kondisi
umum pasien stabil, pasien tidur nyenyak.
3.00 p.m. Take vital signs of patient ,BP : 120/80, P : 88
bpm, Rr : 24x/m, T : 37,5°C
Mengukur tanda vital pasien , Tekanan Darah :
120/80, Nadi : 88 x/m, Respirasi : 24x/m, Suhu
tubuh : 37,5°C
4.00 p.m. Evening complete bed bath.
Dressing renewed .Wound is still wet.
Patient still has pain on the leg.
Bed making and make patient comfortable
position.
Mandi sore lengkap di tempat tidur.
Ganti perban. Luka masih basah.
Pasien masih merasakan nyeri pada kaki.
Merapikan tempat tidur dan membuat posisi
pasien nyaman
4.15 p.m. Patient tea and extra Ponstan 500 mgs given.
Beri pasien teh dan tambahan Ponstan 500 mg
4.30 p.m. Patient try to ambulate.
Pasien mencoba untuk bergerak
5.15 p.m. Apply a urine catheter, urine (+), blood (-).
Pain on lower abdomen (-)

 Pasang kateter urine, urine (+), darah (-).
Sakit perut bagian bawah (-)
6.30 p.m. Patient ‘s family visits the patient .
Keluarga pasien menjenguk pasien
7.00 p.m. Prepare patient for dinner ,patient eats a lot
Siapkan pasien untuk makan malam , pasien
makan banyak
7.15 p.m. Collect plate and cup and return to ward
kitchen for washing.
Kumpulkan piring dan cangkir dan kembalikan
ke dapur untuk dicuci
8.00 p.m. Treatment to be given as prescribed
Pengobatan diberikan sesuai resep
8.15 p.m. Control all condition of patient
Kontrol semua kondisi pasien
8.30 p.m. Complete intake and output charts
Lengkapi grafik pemasukan dan pengeluaran
8.45 p.m. Patients settled for night
Pasien diatur untuk malam hari
9.00 p.m. Make afternoon report.
Take over the ward from afternoon to night
nurses and does patient‘s round.
Membuat laporan sore.
Serah terima ruangan dari dinas sore ke dinas
malam dan melakukan kunjungan pasien
