 RECORD KEEPING!
Record-keeping is an integral part
of nursing, midwifery and allied
health professionals’ practice and
is essential to the provision of safe
and effective care.

This guidance applies to both

paper and electronic records and
includes handwritten clinical
notes, e-mails and letters to and
from other health professionals as
well as care plans, birth plans and
observation charts.
RECORD KEEPING
Dokumentasi adalah suatu proses
pencatatan, penyimpanan informasi data
atau fakta yang bermakna dalam
pelaksanaan kegiatan baik yang berbentuk
tulisan maupun rekaman lainnya seperti
dengan pita suara/cassete, vidio, film,
gambar, dan foto.

Secara umum dokumentasi dapat diartikan

sebagai suatu catatan otentik atau semua
surat asli yang dapat dibuktikan atau
dijadikan bukti dalam persoalan hukum.
 Record Keeping
Berdasarkan Peraturan 43 bidan diwajibkan :
memberikan fasilitas yang layak untuk
pemeriksaan dan pemeriksaan yaitu :
a. catatan
b. metode praktik
c. peralatannya.
Seorang bidan yang berpraktik harus menyimpan catatan rinci yang wajar yaitu :
1. pengamatan
2. perawatan yang diberikan
3. obat-obatan atau bentuk pereda nyeri lainnya yang diberikan olehnya kepada
semua ibu dan bayi.
 Seorang bidan yang berpraktik harus menyimpan catatan rinci

Catatan sebagaimana dimaksud pada ayat (1) aturan ini :

 memperoleh ketepatan diperlukan ketelitian penggunaan seperti
penilaian gambaran klinis pasien, hasil laboratorium, pemeriksaan
tambahan, pencatatan terhadap setiap rencana tindakan,
pelayanan kesehatan, observasi yang dilakukan pada lembar atau
Ketepatan bagan yang ditentukan, dan kesesuaian hasil pemeriksaan
dengan hasil atau intruksi dokter dan tenaga kesehatan lainnya,
dimana kesalahan dikoreksi dengan baik dan
pada tanda bukti pencantuman di paraf.

 Aspek Legal dalam dokumentasi

perselisihan antara individu dan
masyarakat
5. secara keseluruhan.
 Hal-Hal yang harus diperhatikan agar dokumentasi
dapat diterapkan sebagai aspek legal secara hukum :

a. Catatan kebidanan pasien/ klien diakui secara legal/

1. Dokumentasi informasi yang
berkaitan dengan aspek legal.
hukum.
b. Catatan/grafik secara universal dapat dianggap
sebagai bukti dari suatu pekerjaan.
c. Informasi yang didokumentasikan harus memberikan
catatan ringkas tentang
riwayat perawatan pasien.
d. Dokumentasi perlu akurat sehingga sesuai dengan
standar kebidanan yang telah ditetapkan.
2. Petunjuk untuk mencatat data yang relevan secara legal.

Mengetahui malpraktek yang melibatkan bidan

-Klien menjadi tanggung jawab perawat yangbersangkutan.
-Bidan tidak melaksanakan tugas yang diemban.
-Bidan menyebabkan perlukaan atau kecacatan pada klien.

Memperhatikan informasi yang memadai  kondisi klien dan

perilaku, mendokumentasikan tindakan kebidanan dan medis,
follow up, pelaksanaan pengkajian fisik per shift, dan
mendokumentasikan komunikasi antara bidan dokter.
