Perawat menggunakan riwayat kesehatan dan
pemeriksaan fisik untuk memperoleh data tentang pola
pergerakan yang biasa dilakukan seorang. Data tersebut
dikoordinasikan dengan riwayat perkembangan dan
informasi tentang latar belakang sosial dan psikososial
pasien. Riwayat kesehatan meliputi informasi tentang
aktivitas hidup sehari-hari, pola ambulasi, alat bantu
yang digunakan (misal; kursi roda, tongkat, walker),
dan nyeri (jika ada nyei tetapkan lokasi, lama, dan
faktor pencetus) kram atau kelemahan.
