Pada Kala III Persalinan

Fisiologi kala III
Mekanisme pelepasan plasenta :
a. fase pelepasan plasenta
b. fase pengeluaran plasenta
Fase pelepasan plasenta :
 Kontraksi rahim akan mengurangi area perlekatan placenta,
karena rahim bertambah kecil dan dindingnya bertambah tebal
beberapa sentimeter.
 Kontraksi tadi menyebabkan bagian yang longgar dan lemah
dari ujung plasenta pada dinding rahim, bagian ini akan
terlepas, mula-mula sebagian dan kemudian seluruhnya dan
tinggal bebas dalam kavum uteri.
 Kadang-kadang ada sebagian kecil placenta yang masih
melekat pada dinding rahim.
 Teknik untuk mengetahui lepasnya plasenta :

1. KUSTNER :
Dengan meletakkan satu tangan disertai tekanan pada / diatas simfisis pubis,
dengan tangan kiri/kanan tali pusat ditegangkan, bila tali pusat masuk, berarti
belum lepas, diam atau maju, berarti sudah lepas.

2. STRASSMAN :
Tegangkan tali pusat & ketok pada fundus, bila tali pusat bergetar, berarti
belum lepas, jika tidak bergetar berarti sudah lepas

3. KLEIN :
Sewaktu ada his, rahim kita dorong sedikit, bila tali pusat kembali masuk,
berarti belum lepas, jika diam atau turun, berarti sudah lepas
 Macam-macam pelepasan
plasenta :

1. SCHULTZE : Lepasnya sprt kita menutup payung, cara ini

paling sering terjadi (80%). Yg lepas dahulu adalah bagian
tengah, lalu terjadi retroplasental hematoma yg menolak
plasenta mula2 bagian tengah, kemudian seluruhnya.
Perdarahan biasanya tidak ada sebelum plasenta lahir &
banyak setelah uri lahir

2. DUNCAN : Lepasnya plasenta mulai dari pinggir, jadi

pinggir uri lahir dahulu (20%). Darah akan mengalir keluar
antara selaput ketuban. Serempak dari tengah & pinggir
plasenta
Tanda-tanda pelepasan plasenta :
1. Adanya darah yg keluar atau menyembur secara tiba
–tiba Tali pusat tampak memanjang di introitus
vagina
2. Bentuk uterus globuler seiring dengan kontraksi
uterus itu sendiri Uterus naik ke abdoment akibat
plasenta pada SBR atau bagian atas vagina
menggeser uterus keatas

Fase pengeluaran plasenta :

1. Setelah plasenta lepas, dengan adanya retraksi &
kontraksi maka plasenta terdorong ke dlm SBR
atau ke dlm bagian atas dari vagina.
2. Selanjutnya plasenta didorong ke luar oleh
tenaga mengejan, shg plasenta akan keluar atau
dilahirkan.

Kala IV Persalinan
Kala IV adalah kala
pengawasan, dari 1-6
jam setelah bayi dan
plasenta lahir untuk
memantau kondisi
ibu.
Hal penting dalam masa nifas :
Proses Involusi Organ genitalia, Proses
Laktasi, Proses Haemokonsentrasi
 Proses Involusi Uterus :
Perobahan Bentuk : Seperti buah Alpukad
gepeng ( 15 X 12 X 10 cm ) Perobahan
besar : 2 jari dibawah pusat ( post
partum ), setengah simpisis pusat ( hari ke
5 ) tidak teraba ( 12 hari ) sempurna 6
minggu. Perobahan berat : post partum
1000 gr, 1 minggu 500 gr, 2 minggu 300 gr
dan 6 minggu 30 gr ( normal )
Bagaimana involusi uteri terjadi :
Kontraksi uterus post partum Adanya
proteolisis pada miometrium Pada
Endometrium : trombosis, degenerasi dan
nekrosis pada implantasi plasenta. Servik
Uteri : Post partum : membuka seperti
corong, berwarna kemerahan dan
konsistensi lunak. 2 jam : hanya terbuka 2
– 3 jari 1 minggu : terbuka 1 jari
Perubahan Masa Nifas

Vagina : Pengembalian kekencangan otot

dasar panggul. Pada masa nifas sekret
yang keluar dari vagina disebut Lokhia.
Hari 1 – 2 ; Lokhia Rubra Hari : Lokhia
Sanguionolente 1 – 2 mg : Lokhia Serosa >
2 mg ; Lokhia Alba
 Ligamen-ligamen & diafragma pelvis :
Setelah janin lahir, berangsur-angsur ciut,
seperti sedia kala.
Kalau Lig. Rotundum kendor, uterus jatuh
ke belakang ( kandungan turun )
diperlukan latihan-latihan tertentu.
Fisioterapi dapat diberikan pada 2 hari post
partum.
Proses Laktasi Persiapan ASI : Perobahan
pada payudara mulai kehamilan muda
dengan proliferasi ( kalenjer, alveolus ,dan
proliferasi lemak ) dan hipervaskularisasi.
Produksi ASI ; post partum dengan aktifnya
hormon prolaktin Pengeluaran ASI :
kontraksi mioepitelium.
 Hari I post partum : Colustrum
Produksi ASI pada hari 2 – 3 post partum
Dipengaruhi oleh Refleks Milk Ejection.

Mempererat rasa kasih sayang antara ibu
dan bayi ASI dapat melindungi bayi
terhadap infeksi seperti : Gastroenteris
Radang jalan nafas & paru-paru Otitis
media. ASI mengandung lactoferin,
lysozyme & IgA
Perubahan Lain Pada Nifas
Rasa Mules setelah partus Peningkatan
Suhu tubuh maksimal 38 C dan kembali
normal dalam 12 jam. Segera setelah
partus terjadi bradikardi. Pada masa nifas
umumnya denyut nadi lebih labil
dibandingkan dengan suhu badan. Pada
beberapa kasus ditemukan hipertensi post
partum.
Definisi : Masa nifas (puerpurium )adalah
masa setelah partus selesai sampai 6
mingu ( 40 hari ) pasca persalinan Namun
pengembalian fungsi alat genital baru
sempurna dalam waktu 3 bulan.
Kebutuhan Dasar Ibu Masa
Nifas
Ibu yang berada
dalam masa nifas
mempunyai
kebutuhan dasar
khusus agar dapat
melewati masa nifas
dengan aman, sehat
dan sejahtera
sekaligus menunjang
keberhasilan
menyusui
Nutrisi dan Cairan Ibu yang berada dalam
masa nifas dan menyusui membutuhkan
kalori yang sama dengan wanita dewasa,
ditambah 700 kalori pada 6 bulan pertama
untuk memberikan ASI eksklusif dan 500
kalori pada bulan ke tujuh dan selanjutnya.
Ibu dianjurkan untuk minum setiap kali
menyusui dan menjaga kebutuhan hidrasi
sedikitnya 3 liter setiap hari
Tablet besi masih tetap diminum untuk
mencegah anemia, minimal sampai 40 hari
post partum. Vitamin A ( IU) dianjurkan
untuk mempercepat proses penyembuhan
pasca salin dan mentransfernya ke bayi
melalui ASI Ibu nifas yang membatasi
asupan kalori secara berlebihan sehingga
menyebabkan terjadinya penurunan berat
badan lebih dari setengah Kg/minggu,
akan mempengaruhi produksi ASI
Ambulasi Pada persalinan normal, Ibu
diperbolehkan untuk ke kamar mandi
dengan dibantu, satu atau dua jam setelah
melahirkan. Namun sebelumnya Ibu
diminta untuk melakukan latihan menarik
nafas yang dalam serta latihan tungkai
sederhana dengan cara mengayunkan
tungkainya di tepi tempat tidur Mobilisasi
sebaiknya dilakukan secara bertahap.
Diawali dengan gerakan miring ke kanan
dan ke kiri di atas tempat tidur Mobilisasi
ini tidak mutlak, bervariasi tergantung pada
ada tidaknya komplikasi persalinan, nifas
dan status kesehatan ibu
Pemulihan pasca salin akan berlangsung
lebih cepat bila Ibu melakukan mobilisasi
dengan benar dan tepat, terutama untuk
system peredaran darah, pernafasan dan
otot-rangka. Penelitian membuktikan
bahwa early ambulation atau ambulasi dini
bisa mencegah terjadinya sumbatan pada
aliran darah.Tersumbatnya aliran darah
bisa menyebabkan terjadinya trombosis
vena dalam atau DVT (Deep Vein
Thrombosis) dan bisa menyebabkan
infeksi pada pembuluh darah
Eliminasi : BAK/BAB Pada kala IV
persalinan pemantauan urin dilakukan
selama 2 jam, setiap 15 menit sekali pada
1 jam pertama dan 30 menit sekali pada
jam berikutnya. Pemantauan urin dilakukan
untuk memastikan kandung kemih tetap
kosong sehingga uterus dapat berkontraksi
dengan baik Ibu diharapkan untuk
berkemih dalam 6-8 jam pertama.
Pengeluaran urin masih tetap dipantau dan
diharapkan setiap kali berkemih urin yang
keluar minimal sekitar 150 ml
 Kebutuhan untuk defekasi biasanya timbul
pada hari pertama sampai hari ke tiga post
partum. Kebutuhan ini dapat terpenuhi bila
ibu mengkonsumsi makanan yang
mengandung tinggi serat, cukup cairan dan
melakukan mobilisasi dengan baik dan
benar. Bila lebih dari waktu tersebut ibu
belum mengalami defekasi mungkin perlu
diberikan obat pencahar
Kebersihan diri/perineum
Kebersihan adalah keadaan bebas dari
kotoran, termasuk di antaranya, debu,
sampah, bau, virus, bakteri patogen dan
bahan kimia berbahaya. Kebersihan
badan atau personal hygiene meliputi
kebersihan diri sendiri, seperti mandi,
menyikat gigi, mencuci tangan, dan
memakai pakaian yang bersih. Tingkat
kebersihan antara setiap orang berbeda-
beda satu sama lain
Pada masa nifas kebersihan vagina perlu
mendapat perhatian lebih, karena :
1. Adanya darah dan cairan yang keluar
dari vagina selama masa nifas / lochea. 2.
Secara anatomis, letak vagina berdekatan
dengan meatus eksternus uretrae dan
anus. Kedua saluran tersebut merupakan
saluran pembuangan (muara eksreta) dan
banyak mengandung mikroorganisme
patogen. 3. Adanya luka / trauma di daerah
perineum yang terjadi akibat proses
persalinan dan bila terkena kotoran dapat
terinfeksi. 4. Vagina merupakan organ
terbuka yang mudah dimasuki
mikroorganisme yang dapat menjalar ke
rahim.
Istirahat Kebutuhan istirahat sangat
diperlukan ibu beberapa jam setelah
melahirkan. Proses persalinan yang lama
dan melelahkan dapat membuat ibu
frustasi bahkan depresi apabila kebutuhan
istirahatnya tidak terpenuhi Bila ibu
mengalami kesulitan untuk tidur pada
malam hari, satu atau dua pertama setelah
melahirkan, dapat diberikan bantuan obat
tidur dengan mengkonsultasikannya
terlebih dulu dengan dokter. Insommia
pada ibu nifas merupakan salah satu tanda
peringatan untuk psikosis nifas.
Pola tidur akan kembali mendekati normal
dalam 2 sampai 3 minggu setelah
persalinan.
Kebutuhan tidur rata-rata pada orang
dewasa sekitar 7 – 8 jam per 24 jam.
Semakin bertambahnya usia, maka
kebutuhan tidur juga akan semakin
berkurang. Pada ibu nifas, kurang istirahat
akan mengakibatkan : . Berkurangnya
produksi Asi, Memperlambat proses
involusi uterus dan meningkatkan
perdarahan, Menyebabkan depresi dan
ketidakmampuan untuk merawat bayi dan
dirinya sendiri.
 Seksual Masa nifas yang berlangsung
selama 6 minggu atau 40 hari merupakan
masa pembersihan rahim. Sama hal nya
seperti pada saat menstruasi, darah nifas
mengandung trombosit, sel - sel
degeneratif, sel- sel mati dan sisa sel-sel
endometrium.
 Ibu yang baru melahirkan boleh
melakukan hubungan seksual kembali
setelah 6 minggu persalinan. Batasan
waktu 6 minggu didasarkan atas pemikiran
pada masa itu semua luka akibat
persalinan, termasuk luka episiotomi dan
luka bekas SC biasanya telah sembuh
dengan baik. Bila suatu persalinan
dipastikan tidak ada luka atau
laserasi/robek pada jaringan, hubungan
seks bahkan telah boleh dilakukan minggu
setelah proses melahirkan
Beberapa penyebab dyspareunia :
Setelah melahirkan ibu - ibu sering
mengkonsumsi jamu - jamu tertentu. Jamu
- jamu ini mungkin mengandung zat zat
yang memiliki sifat astringents yang
berakibat menghambat produksi cairan
pelumas pada vagina. Jaringan baru yang
terbentuk karena proses penyembuhan
luka guntingan jalan lahir masih sensitif.
Faktor psikologis yaitu kecemasan yang
berlebihan.
 Latihan/senam nifas Senam nifas
sebaiknya dilakukan dalam 24 jam setelah
persalinan, secara teratur setiap hari. Ibu
tidak perlu khawatir terhadap luka yang
timbul akibat proses persalinan karena 6
jam setelah persalinan normal dan 8 jam
setelah persalinan caesar, ibu sudah
dianjurkan untuk melakukan mobilisasi dini.
Tujuan utama mobilisasi dini adalah agar
peredaran darah ibu dapat berjalan dengan
baik sehingga ibu dapat melakukan senam
nifas
Manfaat Senam nifas antara lain :
Memperbaiki sirkulasi darah sehingga
mencegah terjadinya trombosis pada
pembuluh darah terutama pembuluh
tungkai. Memperbaiki sikap tubuh setelah
kehamilan dan persalinan dengan
memulihkan dan menguatkan otot-otot
punggung. Memperbaiki tonus otot pelvis
Memperbaiki regangan otot tungkai bawah
Memperbaiki regangan otot abdomen
setelah hamil dan melahirkan
Meningkatkan kesadaran untuk melakukan
relaksasi otot-otot dasar panggul
Mempercepat terjadinya proses involusi
organ-organ reproduksi
Tanda-tanda infeksi yang bisa dialami ibu
pada masa nifas apabila tidak melakukan
perawatan vagina dengan baik : Suhu
tubuh pada aksila melebihi 37,5° C. Ibu
menggigil, pusing, dan mual Keputihan
yang berbau Keluar cairan seperti nanah
dari vagina yang disertai bau dan rasa
nyeri Terasa nyeri di perut Terjadinya
perdarah pervagina yang lebih banyak dari
biasanya

Atonia Uteri :
III & IV
Keadaan lemahnya tonus/ kontraksi rahim yg menyebabkan uterus tidak mampu menutup perdarahan terbuka dari
tempat implatasi plasenta setelah bayi dan plasenta lahir

Retensio Placenta :
 Lepasnya plasenta tdk bersamaan sehingga sebagian masih melekat pada tempat implantasi
 Terlambatnya kelahiran plasenta selama setengah jam setelah kelahran bayi

Emboli air ketuban :

Masuknya cairan ketuban beserta komponennya kedam sirkulasi darah ibu Yang dimaksud komponen disini ialah unsur-
unsur yang terdapat di air ketuban, seperti lapisan kulit janin yang terlepas, rambut janin, lapisan lemak janin, dan
musin/cairan kenta

Robekan jalan Lahir :

Robekan Perineum, Robekan Vagina dan Robekan Serviks

Inversio Uteri :
Keadaan dimana fundus uteri terbalik sebagian atau seluruhnya masuk kedalam kavum uteri

Perdarahan ( Primer dan Sekunder) :

Perdarahan post partum dalam kala IV lebih dari 500 cc dalam 24 jam setelah anak dan plasenta lahir

• Lepasnya plasenta tdk bersamaan sehingga sebagian masih melekat pada
tempat implantasi
• Terlambatnya kelahiran plasenta selama setengah jam setelah kelahran bayi

menyebabkan kegagalan mekanisme separasi fisiologis..
