Batu saluran kemih merupakan salah satu masalah dibidang urologi yang angka kejadiannya di Indonesia
masih cukup tinggi, salah satunya adalah batu ureter yang menimbulkan gejala kolik yang menyiksa
penderita yang bila mengenai kedua ureter dapat mengakibatkan sumbatan total saluran kemih sehingga
terjadi hidronefrosis dengan segala akibatnya. (1,4,9,10,11)
Ada 3 faktor yang mempengaruhi pembentukan batu yaitu :

Teori inti (nucleus) kristal dan benda asing merupakan tempat pengendapan kristal pada urine
yang sudah mengalami supersaturasi
Teori matrix : matrix organic yang berasal dari serum atau protein urine memberikan kemungkinan
pengendapan kristal.
Teori inhibitor kristalisasi : Beberapa substansi dalam urine menghambat terjadinya kristalisasi,
konsentrasi yang rendah atau absennya substansi ini menunjukkan terjadinya kristalisasi. (6)
Adanya batu pada ureter akan menyebabkan obstruksi yang bila mengenai kedua ureter akan berpengaruh
pada ginjal menyebabkan terjadinya hidronefrosis sampai gagal ginjal. (4,10)
Batu ureter berasal dari ginjal oleh gaya gravitasi peristaltic ureter, batu bisa masuk dan turun ke ureter.
Batu ini dapat menyebabkan sumbatan komplit pada ureter. Oleh karena adanya penyempitan pada 3
tempat pada ureter yaitu pada uretero pelvic junction, persilangan antara arteri dan vena iliaca dan uretero
vesical junction, maka biasanya batu ureter tersangkut pada daerah tersebut. (1,9,10)
Obstruksi ureter bilateral dapat menyebabkan peninggian tekanan intra ureter 50 70 mmHg sehingga
mengakibatkan aliran balik ureter ke forniks ginjal, pielo kanalikuli, pielo limfatik dan pielo venous. (1,3)
Prinsip dasar dalam urologi untuk menangani obstruksi akut total bilateral adalah membebaskan obstruksi
untuk mencegah kerusakan ginjal lebih lanjut.
1. 1. Anatomi
Kaliks, pelvis renis dan ureter merupakan struktur yang serupa. Pelvis dan ureter terdiri dari 3 lapis :
Fibrosa superficialis, muskularis media, dan mukosa dalam. Otot-otot dari pelvis renis dan kaliks identik
dengan ureter meskipun kurang berkembang bila dibandingkan ureter. Terdapat lapisan-lapisan otot
longitudinal superficial dan profunda yang tegas dengan lapisan otot sirkuler ditengahnya. Dijelaskan
bahwa pada hubungan antara kaliks mayor dan minor, lapisan otot sirkuler meningkat dan memberikan
aliran sphincter parsial.(5)
Ureter memanjang dari pelvis renis hingga kandung kemih dan bervariasi panjangnya dari 24 34 cm.
panjang ureter kanan 1 cm lebih pendek dari ureter kiri. Ureter menyerupai kurva bentuk S yang secara
relative lurus pada bagian tengahnya. Tatkala ureter meninggalkan pelvis renis dan menuju ke medial dari
m. Psoas, lateral dari proc. Transversus spinosus dan tepat pertengahan jalan kandung kemih, ia melintas di
belakan funiculus spermaticus (atau ovarium). Bagian distal dari area ini ureter tidak melekat erat pada
peritoneum, yang mana merupakan hal penting dalam pembedahan. (5) tiga titik penyempitan fisiologis pada
ureter adalah uretero pelvic junction, persilangan pada arteri iliaca dan uretero vesical junction, (gambar 1).
Ateri iliaca membagi ureter ke dalam 2 serabut fungsional : serabut proksimal ( 10 mm ) dan yang di
distal serabutnya lebih pendek dan kecil ( 4 6 mm ). (5)
Suplai Darah Ureter
Suplai darah ureter berasal dari banyak cabang, antara lain cabang A. Renalis untuk 1/3 atas ureter dan
pelvis renis, cabang dari aorta, A. Illiaca, A. Mesenterika inferior, A. Hipogastrika profunda dan arteri-arteri
ovarium/spermatica untuk 1/3 tengah ureter dan cabang vesical pada 1/3 bawah ureter terdapat anastomosis
bebas dari pembuluh-pembuluh ini pada tunica adventitia ureter dan pelvis renis dan sampai menembus
arteriol, pembuluh-pembuluh darah ini ber-anastomose secara bebas dengan otot-otot longitudinal dan
mukosa prekapiler. Anastomose ini mensuplai ureter melalui beberapa arteri saja, itu sebabnya bila ini
terputus tidak menyebabkan iskemik ureter secara bermakna. Vena-vena pada sub mukosa dan yang ke
adventitia mengalirkan darah ke vesical, vaginal, uterus, spermatica, iliaca, lumbal dan pembuluh darah
renal. (5)
Aliran Lymphe
Aliran limphe bersama arteri dan beberapa anastomosis mengalirkan limphe ke 3 daerah limphonodus :
hipogastrica, iliaca, lumbal atau pre aorta. (5)
Sistem Persarafan
Inervasi ureter adalah autonom dengan sel-sel ganglion terbatas pada adventitia. Dengan kata lain fungsi
ureter baik, inervasi tidak terganggu selama adventitia tetap intak, kecuali jika adventitia mengalami atrofi
ureter dan dilatasi innervasi pre ganglionik berasal dari renal, vesica inferior, hipogastric, aorta, spermatica,
mesenterica inferior, pleksus vaginalis dari coeliaca dan ganglion sacralis superior. Nyeri alih pada kolik
ureter kadang-kadang disebabkan N. Illiohypogastrica ( L.1 atau T12 dan L1 ), N. Illioinguinal ( T12 dan
L1 ) dan cabang spermatica externa dari N. Genito femoralis. (1,2,5)
1. 2. Patologi
Uropati obstruktif dengan akibat hidronefrosis merupakan hasil akhir dari penyakit urologi, yang mana
diketahui berakhirnya obstruksi ureteral komplit pada akhirnya merusak fungsi ginjal. Mekanismenya
diduga dari peningkatan tekanan ureteral dan penurunan aliran darah ginjal yang menyebabkan atrofi
seluler dan nekrosis.(13)
Obstruksi ureter total bilateral menyebabkan pelvis renis berdilatasi secara progresif dalam beberapa
minggu pertama berat ginjal meningkat seiring dengan oedemnya walaupun jaringan parenkim ginjal
mengalami atrofi. Jadi merupakan oedem peri renal dan peri ureteral, setelah 4-8 minggu ada penurunan
berat karena atrofi jaringan lebih banyak terjadi dibanding dengan oedem intra renal. (10)
Obstruksi ureter total bilateral menyebabkan dilatasi bagian proksimal dengan perubahan morfologi dan
fungsi pada ureter proksimal dan pelvis renis, menyebabkan aliran balik urine ke proksimal ke pielo
kanalikuli, pielo limfatik, pielo venous dan forniks ginjal. (3,12,13)
Selama beberapa hari pertama obstruksi terjadi pendataran papilla dengan dilatasi nefron distal, tubulus
proksimal tampak berdilatasi sementara selama beberapa hari pertama dan kemudian secara perlahan-lahan
mengalami atrofi. Pada hari ke-4 terjadi dilatasi. Pada hari ke-7 obstruksi, tubulus kollektivus mengalami
atrofi dan nekrosis. Pada hari ke-14 obstruksi, terjadi dilatasi progresif pada tubulus kollektivus, tubulus
distal dan atrofi tubulus proksimal, sel-sel epitel terlihat. Pada hari ke-28 obstruksi terjadi penurunan 50
% dari ketebalan medulla dengan atrofi dan dilatasi lanjut pada tubular distal dan kollektivus, korteks
menjadi lebih tipis dengan atrofi tubulus proksimal. Setelah 8 minggu obstruksi, ketebalan parenkim 1 cm
yang mengandung jaringan ikat dan sisa-sisa glomerulus berbentuk oval kecil. (3,12,13)
Obstruksi traktus urinarius menyebabkan dilatasi proksimal dengan perubahan fungsional dan morfologis
pada tubulus proksimal dan pelvis renis. (3)
Perubahan patologis pada ginjal yang mengalami obstruksi total berhubungan dengan perubahan yang
terlihat, perubahan-perubahan histologis meliputi atrofi, mulai pada 7 hari pertama pada nefron distal, pada
hari ke 14, atrofi terlihat pada daerah kortikal, adanya cetakan protein TammHossfall pada ruang bowman
glomerulus merupakan patognomonik khas untuk obstruksi. (3)
Pada obstruksi ureter akut total 1 minggu, terjadi reabsorbsi pielo limfatik ke dalam limfatik hilus, ke
dalam interstitial sel dan terjadi udem parenkim ginjal dan peri ureter. Pada obstruksi lanjut, terjadi
rebsorbsi urine dari pelvis renis pada hidronefrosis masuk kedalam system vena. Banyak peneliti
membuktikan berbagai senyawa yang disuntikkan ke dalam pelvis renis dengan obstruksi total keluar
melalui pembuluh limfe dan vena. (3)
Pada obstruksi ureter akut total, menyebabkan peninggian tekanan di proksimal obstruksi sehingga terjadi
ekstravasasi urin melalui forniks ginjal ke ruang peri renal membentuk urinoma yang mana pada urinoma
retroperitoneal mengalami proses ekstravasasi, urin masuk kedalam rongga peritoneum membentuk ascites.

Pada hidronefrosis cairan keluar dari pelvis renis dengan cara : ekstravasasi ke ruang peritoneal, aliran
balik pielovenous dan aliran balik pielo limfatik. (3)
Urinoma memberi respon yang baik terhadap drainase urin pada obstruksi ureter akut total dan dapat hilang
spontan pada drainase urine adekuat.
1. 3. Patofisiologi
Pada obstruksi ureter akut total, gangguan fungsi ginjal ditentukan oleh lamanya obstruksi. Pada tikus
percobaan dengan obstruksi ureter total selama 4 jam GFR 52 %, selama 12 jam GFR 23 %, selama 24 jam
GFR % dan selama 48 jam GFR %. Aliran darah ginjal dan tekanan ureter menunggi karena vasodilatasi
preglomerulus pada 1,5 jam obstruksi. Pada obstruksi 1,5 5 jam aliran darah ginjal menurun dan tekanan
ureter terus meninggi akibat dari peninggian resistensi post glomerulus. Pada fase 5 18 jam, aliran darah
ginjal dan tekanan ureter menurun akibat dari vasokonstriksi preglomerulus dan fungsi tubulus terganggu.

Pada ginjal normal volume aliran limfe sama dengan out put urin. Surgam dkk melaporkan, volume cairan
limfe ginjal normal 0,5 1 ml/menit. Murphy dkk (1958) menemukan peningkatan volume cairan limfe
pada obstruksi ureter akut total. Drainase limfatik ginjal melalui pembuluh limfe hilus dan kapsula ginjal.
Obstruksi ureter dan pembuluh limfe menyebabkan kerusakan ginjal yang berat dengan nekrosis dan
destruksi dalam beberapa hari. (3)
Menurut Resznyak dkk (1960), yang merumuskan bahwa pemeliharaan fungsi ginjal pada hidronefrosis
menyebabkan aliran balik urin pielolimfatik. Reabsorbsi urin dari pelvis ginjal kedalam aliran limfatik
menyebabkan terjadinya penggantian filtrasi glomerulus. Dan merumuskan bahwa penetrasi urin ke dalam
ruang interstitial, menyebabkan pelepasan histamine, yang mengakibatkan peningkatan permeabilitas
kapiler dan dengan eksudasi cairan kaya protein ke dalam ruang interstitial dan limfatik, terjadi udem
parenkim ginjal, hilus dan kapsul. (3)
Naber dan Madsen (1973) melaporkan bahwa, pada hidronefrosis akut terjadi reabsorbsi urin kedalam
limfe hilus. Jumlah urin yang keluar melalui pelvis ginjal pada hidronefrosis akut adalah 0,06 ml/menit,
pada hidronefrosis kronik selama 6 34 hari jumlah urin keluar dari pelvis ginjal adalah 0,04 ml/menit.
Pada obstruksi ureter yang berlangsung selama 7 hari, akan terjadi reabsorbsi pielolimfatik ke dalam limfe
hilus dan tingkat filtrasi glomerulus pada obstruksi ureter total adalah 1,74 ml/menit dan setelah 34 hari
obstruksi ureter komplit filtrasi glomerulus 0,4 ml/menit. (3)
Pada binatang percobaan setelah pembebasan obstruksi ureter total selama 2 minggu fungsi ginjal kembali
normal, setelah obstruksi total selama 3 minggu fungsi ginjal kira-kira 50 %. Setelah obstruksi total selama
4 minggu fungsi ginjal kira-kira 30 % dan setelah pembebasan obstruksi ureter selama 6 8 minggu
binatang percobaan tidak dapat hidup. (3)
1. Jika kedua ginjal mengalami hidronefrosis, stimulus yang kuat tetap dilanjutkan untuk

menggunakan kedua ginjal untuk mempertahankan fungsi maksimal ini juga terjadi pada
hidronefrosis yang soliter, konsekuensinya pengembalian fungsi dari ginjal ini setelah perbaikan
obstruksi dapat membaik. (9)
Studi eksperimental telah menunjukkan waktu penyembuhan pada penderita obstruksi komplit
sekitar 4 minggu. Fungsi dapat membaik kembali setelah obstruksi 56 atau 69 hari namun
demikian kehilangan fungsi yang irreversible biasanya terjadi pada 7 hari pertama ini terjadi
karena dilatasi dan nekrosis tubular proksimal yang berkembang secara progresif dari waktu ke
waktu. (9)
Dilaporkan seorang penderita setelah mengalami obstruksi ureter akut total selama 69 hari, ginjal dapat
berfungsi kembali. (3)
Setelah obstruksi ureter dihilangkan terjadi diuresis hebat dengan produksi urin meningkat 3 5 kali dari
normal, natriuresis akibat gangguan reabsorbsi natrium di tubulus proksimal. (3)
Diuresis post obstruksi jarang terjadi, biasanya setelah obstruksi ureter akut bilateral atau obstruksi
unilateral pada ginjal soliter. Diuresis post obstruksi bersifat sementara fisiologis dan sembuh sendiri
dengan ekskresi natrium dan air yang berlebihan. Diuresis berlangsung beberapa jam sampai 4 hari, tetapi
dapat berlangsung lebih lama jika terapi koreksi cairan tidak adekuat, tetapi cairan biasanya 50 60 % dari
jumlah produksi urin dengan memakai cairan NaCl fisiologis atau Ringer Laktat. (3)

4. Diagnosis
I. Gambaran Klinik
Keluhan utama adalah berupa nyeri yang menjalar dan hilang timbul, juga dapat berupa nyeri yang
menetap di daerah costo vertebra. Nyeri ini dapat menjalar dari daerah pinggang sampai ke testis atau
labium majus ipsi lateral. Sesuai penjalaran dari nyeri ini dapat memperkirakan letak batu, jika batu berada
di ureter bagian atas penjalaran dari nyeri biasanya ke testis dan jika di ureter bagian tengah nyeri biasanya
terdapat di bagian bawah, bila batu berada di ureter bagian bawah, penjalaran nyeri biasanya ke skrotum
atau ke vulva. (1)
Jika nyeri menjalar ke penis biasanya menunjukkan batu sedang melalui uretero vesical junction ke bulibuli. (1)
Perut kembung, mual, dan muntah karena system persarafan sama, ginjal lambung dan kolon yang letaknya
berdekatan serta ditutupi oleh peritoneum sehingga peradangan pada ginjal dan usus dapat menimbulkan
tanda-tanda peritonitis. (1)
Anemia, gross hematuri dan penurunan berat badan dapat dialami penderita.
Pemeriksaan Fisis
Bila sudah terjadi hidronefrosis, ginjal yang membesar dapat teraba sebagai massa di pinggir dengan
konsistensi lunak sampai kenyal. (1,7,9,10)
Pada pemeriksaan fisis tidak ditemukan tanda-tanda spesifik, kecuali bila telah terjadi hidronefrosis, maka
ginjal yang membesar akan dapat teraba sebagai massa di pinggang dengan konsistensi lunak sampai
kenyal. (1,7,9,10)

Pada foto polos abdomen menunjukan adanya pembesaran ginjal, klasifikasi ureter atau ginjal,
bayangan batu radio opak atau perselubungan pada kasus-kasus bukan batu.
Bila fungsi ginjal masih baik dapat dilakukan pemeriksaan IVP yang biasanya terlihat pelebaran
dari ureter di proksimal batu
Bila kedua fungsi ginjal jelek tidak boleh dilakukan IVP, tetapi dilakukan RPG, jika ginjal
terdorong ke kranial, ke kaudal atau kelateral maka pergeseran ini akan terlihat pada foto antero
posterior sedangkan pergeseran anterior akan terlihat pada foto lateral (15,17)
Ultra sonografi (USG)
Pemeriksaan dengan USG ini hanya untuk menentukan hidronefrosis atau hidroureter, tetapi tidak untuk
menentukan letak obstruksi, dapat mendeteksi causa obstruksi seperti tumor peritoneal dan abses dan dapat
juga digunakan sebagai monitor pada tindakan nefrostomi percutaneus (13,15)

Isotop Scanning
Dengan adanya obstruksi, radio isotop renogram dapat memperlihatkan penurunan fase vasculer dan
sekresi yang disebabkan oleh retensi urine dalam pelvis renis (10)

CT Scan
Untuk teknik yang lebih baik dan lebih teliti dengan memakai kontras untuk menentukan tempat obstruksi
dan mendeteksi massa retroperitoneal atau intra abdominal, sebagai penyebab obstruksi ureter dapat
digunakan pada proses tindakan drainase percutanius dan biopsi untuk prognosis (13,15)

MRI
Dengan pemeriksaan ini suatu organ dapat divisualisasikan dalam tiga dimensi tanpa kontraks dapat
menggambarkan pembuluh darah ginjal secara luas dan ada atau tidaknya kerusakan parenkim ginjal,
sensitifitasnya lebih tinggi dan lebih jelas lagi bila memakai kontraks
PENATALAKSANAAN
Bila terjadi batu ureter bilateral maka akan terjadi obstruksi total akut bilateral sehingga dapat terjadi
kerusakan kedua ginjal yang hebat, untuk mencegah terjadinya hal tersebut maka harus dilakukan
penanganan yang segera oleh karena dapat mengakibatkan kerusakan parekim ginjal dan gangguan fungsi
ginjal. Bila obstruksi disertai dengan infeksi maka dapat terjadi sepsis yang akan memperberat gangguan
fungsi ginjal sehingga akan mengancam jiwa penderita. (3,14,21)
Oleh karena itu harus segera ditangani dengan cara drainase urine untuk memperlancar aliran urine,
sehingga dapat mengurangi tingkat komplikasi dan memperbaiki kualitas hidup (22)
Tindakan drainase urine seperti kateterisasi ureter retrograd, nefrostomi terbuka dan nefrostomi
perkutaneus, juga disertai pemberian antibiotik yang sesuai dengan tes kepekaan. Dengan tindakan
draiunase urine, dapat memperbaiki fungsi ginjal dan gangguan keseimbangan elektrolit yang
memungkinkan dilakukan tindakan pembnedahan definitif. (19,21)
Tindakan drainase yang dapat dilakukan antara lain :
(1) Kateterisasi ureter retrograd
(2) Nefrostomi terbuka
(3) Nefrostomi perkutaneus
Ad. 1. Kateterisasi ureter retrograde (16,19)
Pemasangan kateter ureter retrograde mempunyai kekurangan dan menurut laporan oleh Happelen dkk
(1979), angka keberhasilan mencapai 80-85% kasus, laporan oleh Khan (1975), mendapatkan angka
keberhasilan sangat rendah disebabkan oleh distorsi dasar buli-buli oleh tumor dan orifisium ureter
menjadi tidak jelas. (19)
Drainase urine dengan kateterisasi ureter retrograde biasanya dilakukan pada penderita kausa metastase
tumor dan pasase urine kurang adekuat jika dibandingkan dengan nefrostomi terbuka dan nefrostomi
perkutaneus. (15)
Ad. 2 Nefrostomi terbuka
Cara diversi urine yang sudah lama tetapi masih dipakai sampai sekarang dan masih efektif dalam
mengatasi kasus obstruksi seperti kasus obstruksi ureter akut bilateral. Tindakan nefrostomi adalah suatu
tindakan darurat dan sementara. Jadi sebelumnya harus dipikirkan tindakan definitif selanjutnya.
Nefrostomi terbuka memberikan drainase urine lebih baik pada dilatasi sistem pelvic dibandingkan
nefrostomi perkutaneus. (14,19,20)
Teknik Operasi
Dilakukan dengan sebaik-baiknya untuk menjamin lancarnya urine keluar dengan demikian dapat
mengatasi urine yang tersumbat. Sayatan kulit pada ICS XI-XII seperti lumbotomi panjang kira-kira 10-15
cm. diperdalam sampai kapsula gerota pada ginjal.
Ada dua macam teknik operasi terbuka :

Bila korteks masih tebal, ginjal dibebaskan sampai terlihat pervis renalis. Pada pervis dibuat insisi
1-1,5 cm, klem bengkok dimasukkan melalui insisi ke arah kaliks medius, inferior sampai
menembus keluar ginjal. Kemudian kateter Foley kateter F 20 dijepit dengan klem sampai pielum,
isi balon 5 cc. tutup pelvis dengan benang jahit simpel yang bisa diserap.
Bila korteks sangat tipis, insisi pada korteks kira-kira 1-1,5 cm, pasang kateter Foley kateter F 22
dengan klem sampai pielum, isi balon 5 cc. di ginjal difiksasi dengan benang dapat diserap. Pada
dinding abdomen difiksasi dengan benang sutra nomor 1. perhatikan warna urine yang keluar dan
catat jumlahnya. Luka operasi dijahit lapis demi lapis dengan drain isap satu buah.
Batu dalam kaliks dan pielum yang mudah dikeluarkan diambil, yang sulit dibiarkan ditunda untuk
tindakan operasi elektif, sebab nefrostomi adalah tindakan cepat dan urine mengalir keluar dan lancar.
Ad. 3. Nefrostomi perkutaneus
Penanganan penderita dengan obstruksi batu ureter akut total bilateral dengan nefrostomi perkutaneus
merupakan suatu tindakan yang cukup baik, dengan tindakan komplikasi dan mortalitas rendah tetapi
harus memperhitungkan potensi untuk meningkatkan harapan dan kualitas hidup penderita (17,21)
Berat kerusakan parenkim ginjal, asal penyakit primer dan potensi penawaran lanjut menentukan harapan
hidup penderita, oleh karena itu semua cara diagnosis harus dilakukan untuk mengidentifikasi alas
penyakit. (17,21)
Nefrostomi perkutaneus telah mengalami perkembangan sejak 15 tahun terakhir, metode yang sering
digunakan adalah memakai percutaneus nephostomy tube (PNT) dengan menggunakan fluoroskopi dan
ultrasonografi yang memungkinkan penempatan drain tube tepat dalam pelvis ginjal. Nefrostomi
perkutaneus juga dapat digunakan untuk memasukkan kontras pielografi antegrad, pengambilan batu ginjal
ureter perkutaneus, nefroskopi, dan biopsy serta penempatan balon intraureter. (10,17,21)
Cara perkutaneus dilakukan pada ginjal yang teraba dari luar, korteks tipis dan dilakukan pada orang yang
tidak terlalu gemuk. Dapat dilakukan dengan anestesi umum, regional dan lokal.
Teknik Operasi
Penderita tengkurap atau miring, dilakukan punksi ke arah ginjal dengan jarum 22 tepat pada pelvis ginjal,
aspirasi nurine dan masukkan kontras sebanyak urine yang diaspirasi. (17,18,22)
Setelah sistem pelviokalisis terlihat jelas dengan fluoroskopi, dilakukan punksi ke arah ginjal dengan
mandarin pada garis aksillaris posterior di bawah arkus kosta XII, keluarkan mandrin dan masukkan kawat
penuntun (guide wire) ke dalam pembungkus (sheath) jarum punksi. Lakukan dilatasi dengan jarum dari
Diamond, masukkan kateter Foley kateter F 20 dengan tuntunan kanula tepat pada pielum, isi balon 5 cc
(17,18,,22)
selama 2 3 minggu, jika terdapat ekstravasasi selama prosedur berlangsung harus dikerjakan kontras
follow up (17,18)
PENATALAKSANAAN PASCA PENANGANAN
Pembebasan obstrnuksi ureter berakibat hilangnya cairan dan elektrolit dalam jumlah besar 3-5 kali lipat
dari normal, sehingga penting melakukan pengukuran secara cermat produksi jumlah cairan dan elektrolit
melalui nefrostomi dan drain sebagai pegangan dalam pemberian cairan dan elektrolit post obstruksi. Tetapi
cairan diberikan 50-60% dari produksi urine dengan memakai NaCl fisiologi atau Ringer Laktat
Antibiotik diberikan sesuai test kepekaan dengan memperhatikan faal ginjal (3,8,9,22)
Perawatan kateter nefrostomi diperhatikan dengan baik supaya agar tidak terjadi atau tertarik. Setelah
keadaan umum penderita, tensi dan nadi baik, serta tidak febris, segera dibuat foto pielografi antegrad
dengan memasukkan kontras melalui kateter nefrostomi untuk melihat letak obstruksi dan menentukan
tindakan definitif yang akan dilakukan (15,19)
Nefrostomi bersifat sementara pada abstruksi ureter segera setelah keadaan penderita memungkinkan
(fungsi ginjal optimla, tidak ada febris), dilakukan tindakan definitif untuk menghilangkan obstruksi (19,22)
Bila penyebabnya adalah batu dilakukan pengambilan batu pada ginjal yang fungsinya lebih baik. Bila ada
pus, dan setelah beberapa hari pus mereda, urine jernih dan fungsi ginjal baik, radiologik parenkim ginjal
masih tebal, dilakukan pengambilan batu atau pembedahan untuk menghilangkan obstruksi dengan
pengambilan batu, pyelolitotomi, uretyerokutaneustomi, dan ureteroneosistostomi. (23,24,25)
RINGKASAN
Obstruksi batu ureter birateral dapat menyebabkan obstruksi total akut bilateral sehingga dapat terjadi
hidronefrosis sehingga terjadi aliran balik prokskimal melalui rupture forniks, pielokanlikuli, pielolimfatik,
pielovenous.
Obstruksi batu ureter bilateral harus segera ditangani untuk mencegah terjadinya kerusakan ginjal lebih
lanjut.
Penanganan utama adalah dilakukan drainase urine sesegera mungkin dengan kateterisasi ureter retrograde,
nefrostomi terbuka, nefrostomi perkutaneus.
Bila peralatan tidak lengkap batu yang ada dilakukan ureterotomi sedangkan bila peralatan lengkap dapat
dilakukan ESWL, endoscopy dan ureterocopy.
Perawatan pasca operasi harus diperhatikan karena sering terjadi diuresis yang berakibat hilangnya cairan
dan elektrolit 3-5 kali lipat dari normal.

Definisi
sepsis yang disebabkan oleh dekomposisi dan absorpsi substansi yang berasal dari saluran kemih.
Bakteremia simtomatik yang menyebabkan syok dan kematian akibat bakteri berasal dari traktus urinarius
yang merupakan komplikasi dari ISK.1,2
Bakteremia :
Bakteri terdapat dalam darah yang dikonfirmasi dengan kultur, dapat bersifat sementara. 3
Septikemia :
Sama seperti bakteraemia, tetapi menunjukkan kondisi yang lebih berat. Bukti klinis infeksi ditambah bukti
respon sistemik terhadap infeksi. Respon sistemik ini dapat bermanifestasi 2 atau lebih kondisi berikut :
Temperatur > 38C atau < 36C
Denyut nadi > 90 kali / min
Frekuensi pernafasan > 20 kali /min or PaCO2 < 32 mmHg (< 4.3 kPa)
Leukosit > 12,000 sel/mm3, < 4,000 sel/mm3 atau 10% bentuk imatur (batang).3
Sepsis syndrome
Infeksi ditambah bukti gangguan perfusi organ berupa: hipoksemia; peningkatan laktat; oliguria; gangguan
kondisi mental.3
Syok septik
Sepsis dengan hipotensi walaupun telah dilakukan resusitasi cairan yang cukup dan masih tetap terdapat
gangguan perfusi berupa asidosis laktat, oliguria dan gangguan mental akut. Pasien dengan obat inotropik
dan vasopressor dapat tidak memberikan gambaran hipotensi saat terjadi gangguan perfusi.
Refractory septic shock
Syok septik yang berlangsung > 1 jam dan tidak respon terhadap pemberian cairan atau intervensi
farmakologi.
Systemic inflammatory response syndrome
Respon terhadap berbagai jenis gangguan klinis, dapat berupa infeksi atau non infeksi (seperti luka bakar
atau pankreatitis).
Diagnosis
Diagnosis pasti adalah bila dapat dibuktikan bahwa bakteri dari kultur darah sama dengan yang ditemukan
pada kultur urin.
Kecurigaan bakteri berasal dari traktus urinarius umumnya bila disertai oleh gejala sistitis atau pielonefritis.
Bila tidak ada manifestasi ISK, kelainan pada urinalisis berupa piuria dan bakteriuri dapat menjadi
petunjuk.
Secara umum dikatakan urosepsis merupakan komplikasi dari beberapa situasi antara lain (1) tindakan
instrumentasi pada traktus genitourinaria (2) abses renal (3) pielonefritis akut (4) Infeksi akibat obstruksi
saluran kemih atau pasien dengan gangguan kekebalan imunitas (5) bakteriuri akibat pemasangan kateter
pada obstruksi dan pasien dengan gangguan kekebalan imunitas.1

While this classification is useful in establishing a differential diagnosis, many pathophysiologic features
are shared among the different categories. (See Etiology.)
Oliguric and nonoliguric patients with AKI
Patients who develop AKI can be oliguric or nonoliguric, have a rapid or slow rise in creatinine levels, and
may have qualitative differences in urine solute concentrations and cellular content. (Approximately 5060% of all causes of AKI are nonoliguric.)
This lack of a uniform clinical presentation reflects the variable nature of the injury.
Classifying AKI as oliguric or nonoliguric based on daily urine excretion has prognostic value. Oliguria is
defined as a daily urine volume of less than 400 mL/d and has a worse prognosis, except in prerenal failure.
Anuria is defined as a urine output of less than 100 mL/d and, if abrupt in onset, suggests bilateral
obstruction or catastrophic injury to both kidneys. Stratification of renal failure along these lines helps in
decision-making (eg, timing of dialysis) and can be an important criterion for patient response to therapy.
The RIFLE system
In 2004, the Acute Dialysis Quality Initiative work group set forth a definition and classification system for
acute renal failure, described by the acronym RIFLE (Risk of renal dysfunction, Injury to the kidney,
Failure or Loss of kidney function, and End-stage kidney disease; see the table, below). [2] Investigators have
since applied the RIFLE system to the clinical evaluation of AKI, although it was not originally intended
for that purpose. AKI research increasingly uses RIFLE.

Note: Patients can be classified by GFR criteria and/or UO criteria. The criteria that support the most severe
classification should be used. The superimposition of acute on chronic failure is indicated with the
designation RIFLE-FC; failure is present in such cases even if the increase in SCreat is less than 3-fold,
provided that the new SCreat is greater than 4.0 mg/dL (350 mol/L) and results from an acute increase of at
least 0.5 mg/dL (44 mol/L).
When the failure classification is achieved by UO criteria, the designation of RIFLE-F O is used to denote
oliguria. The initial stage, risk, has high sensitivity; more patients will be classified in this mild category,
including some who do not actually have renal failure. Progression through the increasingly severe stages
of RIFLE is marked by decreasing sensitivity and increasing specificity.
A vast array of fluid and electrolyte abnormalities can be seen with acute kidney injury (AKI).
Cardiovascular complications
Cardiovascular complications (eg, congestive heart failure [CHF], myocardial infarction, arrhythmias,
cardiac arrest) have been observed in as many as 35% of patients with AKI. Fluid overload secondary to
oliguric AKI is a particular risk for elderly patients with little cardiac reserve.
Pericarditis is a relatively rare complication of AKI. When pericarditis complicates AKI, consider
additional diagnoses, such as systemic lupus erythematosus (SLE) and hepatorenal syndrome.
AKI also can be a complication of cardiac diseases, such as endocarditis, worsening CHF, or atrial
fibrillation with emboli. Cardiac arrest in a patient with AKI always should arouse suspicion of
hyperkalemia. Many authors recommend a trial of intravenous calcium chloride (or gluconate) in all
patients with AKI who experience cardiac arrest.
Pulmonary complications
Pulmonary complications have been reported in approximately 54% of patients with AKI and are the single
most significant risk factor for death in patients with AKI. Several diseases exist that commonly present
with simultaneous pulmonary and renal involvement, including pulmonary/renal syndromes (eg,
Goodpasture syndrome, Wegener granulomatosis, polyarteritis nodosa, cryoglobulinemia, sarcoidosis).
Hypoxia commonly occurs during hemodialysis and can be particularly significant in the patient with
pulmonary disease. This dialysis-related hypoxia is thought to occur secondary to white blood cell (WBC)
lung sequestration and alveolar hypoventilation.
GI complications
GI symptoms of nausea, vomiting, and anorexia are frequent complications of AKI and represent one of the
cardinal signs of uremia. GI bleeding occurs in approximately one third of patients with AKI. Most
episodes are mild, but GI bleeding accounts for 3-8% of deaths in patients with AKI.
Mild hyperamylasemia commonly is seen in AKI (2-3 times controls). Elevation of baseline amylase can
complicate diagnosis of pancreatitis in patients with AKI. Lipase, which commonly is not elevated in AKI,
often is necessary to make the diagnosis of pancreatitis. Pancreatitis has been reported as a concurrent
illness with AKI in patients with atheroemboli, vasculitis, and sepsis from ascending cholangitis.
Jaundice has been reported to complicate AKI in approximately 43% of cases. Etiologies of jaundice with
AKI include hepatic congestion, blood transfusions, and sepsis.
Hepatitis occurring concurrently with AKI should prompt the differential diagnosis of common bile duct
obstruction, fulminant hepatitis B, leptospirosis, acetaminophen toxicity, and Amanita phalloides toxin.
Infectious complications
Infections commonly complicate the course of AKI and have been reported to occur in as many as 33% of
patients with AKI. Most common sites are pulmonary and urinary tracts. Infections are the leading cause of
morbidity and death in patients with AKI. Various studies have reported mortality rates of 11-72% in
infections complicating AKI.
Neurologic complications
Neurologic signs of uremia are a common complication of AKI and have been reported in approximately
38% of patients with AKI. Neurologic sequelae include lethargy, somnolence, reversal of the sleep-wake
cycle, and cognitive or memory deficits. Focal neurologic deficits rarely are due solely to uremia and
should remain a diagnosis of exclusion in patients with AKI.
The pathophysiology of neurologic symptoms is still unknown, but they do not correlate well to levels of
BUN or creatinine.
A number of diseases express themselves with concurrent neurologic and renal manifestations (eg, SLE,
thrombotic thrombocytopenic purpura [TTP], hemolytic uremic syndrome [HUS], endocarditis, malignant
hypertension).
Etiology
The driving force for glomerular filtration is the pressure gradient from the glomerulus to the Bowman
space. Glomerular pressure is primarily dependent on renal blood flow (RBF) and is controlled by
combined resistances of renal afferent and efferent arterioles. Regardless of the cause of acute kidney injury
(AKI), reductions in RBF represent a common pathologic pathway for decreasing GFR. The etiology of
AKI consists of 3 main mechanisms.
Prerenal failure - Defined by conditions with normal tubular and glomerular function; GFR is
depressed by compromised renal perfusion
Intrinsic renal failure - Includes diseases of the kidney itself, predominantly affecting the
glomerulus or tubule, which are associated with release of renal afferent vasoconstrictors;
ischemic renal injury is the most common cause of intrinsic renal failure.
Postobstructive renal failure - Initially causes an increase in tubular pressure, decreasing the
filtration driving force; this pressure gradient soon equalizes, and maintenance of a depressed GFR
is then dependent on renal efferent vasoconstriction
Patients with chronic renal failure may also present with superimposed AKI from any of the
aforementioned etiologies.
Depressed RBF eventually leads to ischemia and cell death. This may happen before frank systemic
hypotension is present and is referred to as normotensive ischemic AKI. The initial ischemic insult triggers
a cascade of events that includes production of oxygen free radicals, cytokines and enzymes, endothelial
activation and leukocyte adhesion, activation of coagulation, and initiation of apoptosis. These events
continue to cause cell injury even after restoration of RBF.
Tubular cellular damage results in disruption of tight junctions between cells, allowing back leak of
glomerular filtrate and further depressing effective GFR. In addition, dying cells slough off into the tubules,
forming obstructing casts, which further decrease GFR and lead to oliguria.
During this period of depressed RBF, the kidneys are particularly vulnerable to further insults. This is when
iatrogenic renal injury is most common. The following are common iatrogenic combinations:
Preexisting renal disease (elderly, diabetic patients, jaundiced patients) with radiocontrast agents,
aminoglycosides, atheroembolism, or cardiovascular surgery
Angiotensin-converting enzyme (ACE) inhibitors with diuretics, small- or large-vessel renal
arterial disease
Nonsteroidal anti-inflammatory drugs (NSAIDs) with congestive heart failure (CHF),
hypertension (HTN), or renal artery stenosis
Hypovolemia with aminoglycosides, amphotericin, heme pigments, or radiologic contrast agents
nephron loss causes more hyperfiltration until complete renal failure results. This has been termed the
hyperfiltration theory of renal failure and explains the scenario in which progressive renal failure is
frequently observed after apparent recovery from AKI.
Prerenal AKI
Prerenal AKI represents the most common form of kidney injury and often leads to intrinsic AKI if it is not
promptly corrected. Volume loss from GI, renal, cutaneous (eg, burns), and internal or external hemorrhage
can result in this syndrome. Prerenal AKI can also result from decreased renal perfusion in patients with
heart failure or shock (eg, sepsis, anaphylaxis).
Special classes of medications that can induce prerenal AKI in volume-depleted states are angiotensinconverting enzyme inhibitors (ACEIs) and angiotensin receptor blockers (ARBs), which are otherwise
safely tolerated and beneficial in most patients with chronic kidney disease.
Arteriolar vasoconstriction leading to prerenal AKI can occur in hypercalcemic states, with the use of
radiocontrast agents, NSAIDs, amphotericin, calcineurin inhibitors, norepinephrine, and other pressor
agents.
The hepatorenal syndrome can also be considered a form of prerenal AKI, because functional renal failure
develops from diffuse vasoconstriction in vessels supplying the kidney.
To summarize, volume depletion can be caused by the following:
Heart failure
Pulmonary embolus
Acute myocardial infarction
Severe valvular disease
Abdominal compartment syndrome (tense ascites)
Sepsis
Anaphylaxis
Anesthetics
Drug overdose
Hypercalcemia
Drugs (NSAIDs, amphotericin B, calcineurin inhibitors, norepinephrine, radiocontrast agents)
Hepatorenal syndrome
Decreased effective arterial blood volume - Hypovolemia, CHF, liver failure, sepsis
Renal arterial disease - Renal arterial stenosis (atherosclerotic, fibromuscular dysplasia), embolic
disease (septic, cholesterol)
Intrinsic AKI
Structural injury in the kidney is the hallmark of intrinsic AKI, and the most common form is ATN, either
ischemic or cytotoxic. Frank necrosis is not prominent in most human cases of ATN and tends to be patchy.
Less obvious injury includes loss of brush borders, flattening of the epithelium, detachment of cells,
formation of intratubular casts, and dilatation of the lumen (see the images below). Although these changes
are observed predominantly in proximal tubules, injury to the distal nephron can also be demonstrated. In
addition, the distal nephron may become obstructed by desquamated cells and cellular debris.
Tubular etiologies may include ischemia or cytotoxicity. Cytotoxic etiologies include the following:
Postrenal AKI
Mechanical obstruction of the urinary collecting system, including the renal pelvis, ureters, bladder, or
urethra, results in obstructive uropathy or postrenal AKI.
If the site of obstruction is unilateral, then a rise in the serum creatinine level may not be apparent due to
contralateral renal function. Although the serum creatinine level may remain low with unilateral
obstruction, a significant loss of GFR occurs, and patients with partial obstruction may develop progressive
loss of GFR if the obstruction is not relieved. Causes of obstruction include stone disease; stricture; and
intraluminal, extraluminal, or intramural tumors.
Bilateral obstruction is usually a result of prostate enlargement or tumors in men and urologic or
gynecologic tumors in women.
Patients who develop anuria typically have obstruction at the level of the bladder or downstream to it.
To summarize, causes of postrenal AKI include the following:
Ureteric obstruction (stone disease, tumor, fibrosis, ligation during pelvic surgery)
Bladder neck obstruction (benign prostatic hypertrophy [BPH], cancer of the prostate [CA prostate
or prostatic CA], neurogenic bladder, tricyclic antidepressants, ganglion blockers, bladder tumor,
stone disease, hemorrhage/clot)
Urethral obstruction (strictures, tumor, phimosis)
Intra-abdominal hypertension (tense ascites)
Renal vein thrombosis
Diseases causing urinary obstruction from the level of the renal tubules to the urethra include the following:
Tubular obstruction from crystals (eg, uric acid, calcium oxalate, acyclovir, sulfonamide,
methotrexate, myeloma light chains)
Ureteral obstruction - Retroperitoneal tumor, retroperitoneal fibrosis (methysergide, propranolol,
hydralazine), urolithiasis, or papillary necrosis
The patient's age has significant implications for the differential diagnosis of AKI.
Newborns and infants
In newborns and infants, causes of prerenal AKI include the following:
ATN - Can occur in the setting of perinatal asphyxia; ATN also has been observed secondary to
medications (eg, aminoglycosides, NSAIDs) given to the mother perinatally
ACEIs - Can traverse the placenta, resulting in a hemodynamically mediated form of AKI
Acute glomerulonephritis - Rare and most commonly the result of maternal-fetal transfer of
antibodies against the neonate's glomeruli or transfer of chronic infections (syphilis,
cytomegalovirus) associated with acute glomerulonephritis
Congenital malformations of urinary collecting systems should be suspected in cases of postrenal AKI.
Children
Prerenal AKI can be caused by the following:
Intrinsic AKI
Acute poststreptococcal glomerulonephritis - Should be considered in any child who presents with
HTN, edema, hematuria, and renal failure.
HUS - Often is cited as the most common cause of AKI in children.
The most common form of HUS is associated with a diarrheal prodrome caused by Escherichia coli
O157:H7. These children usually present with microangiopathic anemia, thrombocytopenia, colitis, mental
status changes, and renal failure.
Epidemiology
Incidence in the United States
Approximately 1% of patients admitted to hospitals have acute kidney injury (AKI) at the time of
admission. The estimated incidence rate of AKI is 2-5% during hospitalization.
AKI develops within 30 days postoperatively in approximately 1% of general surgery cases [3] ; it develops
in up to 67% of intensive care unit (ICU) patients.[4] Approximately 95% of consultations with
nephrologists are related to AKI.
Feest and colleagues calculated that the appropriate nephrologist referral rate is approximately 70 cases per
million population.[5]
Race predilection
No race predilection is recognized in AKI.
Sex predilection
Males and females are affected equally by AKI
he prognosis for patients with AKI is directly related to the cause of renal failure and, to a great extent, to
the duration of renal failure prior to therapeutic intervention.
On long-term followup (1-10 years), approximately 12.5% of survivors of AKI are dialysis dependent
(rates range widely, from 1-64%, depending on the patient population) and 19-31% of them have chronic
kidney disease.[4]
Mortality rates and associated factors
If AKI is defined by a sudden increment of serum creatinine of 0.5-1 mg/dL and is associated with a mild to
moderate rise in creatinine, the prognosis tends to be worse. (Increments of 0.3 mg/dL in serum creatinine
have important prognostic significance.)
Even if renal failure is mild, however, the mortality rate for patients is 30-60%. If these patients need
dialytic therapy, the mortality rate is 50-90%.
The mortality rate is 31% in patients with normal urine sediment test results and is 74% in patients with
abnormal urine sediment test results.
The survival rate is nearly 0% among patients with AKI who have an Acute Physiology and Chronic Health
Evaluation II (APACHE II) score higher than 40; the survival rate is 40% in patients with APACHE II
scores of 10-19.
The in-hospital mortality rate for AKI is 40-50%.
The mortality rate for patients in the ICU is higher in those who have AKI, especially when AKI is severe
enough to require dialysis treatment; the mortality rate in patients in intensive care settings with AKI is 7080%.
In addition, evidence suggests that the relative risk of death is 4.9 in patients in the ICU who have renal
failure that is not severe enough to require dialysis. This reflects that the high mortality rate in patients with
AKI who require dialysis may not be related to the dialysis procedure or accompanying comorbidities and
that AKI alone may be an independent indicator of mortality.
In one published post hoc analysis of the Fluid and Catheter Treatment Trial (FACTT), which examined
liberal versus conservative fluid management in intubated ICU patients, fluid balance and diuretic use were
identified as prognostic factors for mortality in individuals with AKI. Specifically, greater cumulative fluid
accumulation over an average of 6 days was associated with a higher mortality (10.2 L vs 3.7 L in the
liberal vs conservative group), and higher furosemide use was associated with a lower mortality
(cumulatively 562 mg vs 159 mg). Of note, more than one half of the individuals had Stage 1 AKI (AKIN
criteria) CKD, so whether these results apply to more severe stages of AKI is not clear. One interpretation
of this study is that patients who can be stabilized with less volume resuscitation fare better. From a
practical standpoint, one conclusion is that aggressive prolonged volume resuscitation does not improve
prognosis in AKI in the ICU setting.[6]
Additional prognostic factors
Other prognostic factors include the following:
Older age
Multiorgan failure (ie, the more organs that fail, the worse the prognosis)
Oliguria
Hypotension
Vasopressor support
Number of transfusions
Noncavitary surgery
Prerenal azotemia due to volume contraction is treated with volume expansion; if left untreated for a
prolonged duration, tubular necrosis may result and may not be reversible.
Postrenal AKI, if left untreated for a long time, may result in irreversible renal damage. Procedures such as
catheter placement, lithotripsy, prostatectomy, stent placement, and percutaneous nephrostomy can help to
prevent permanent renal damage.
Timely identification of pyelonephritis, proper treatment, and further prevention using prophylactic
antibiotics may improve the prognosis, especially in females. Early diagnosis of crescentic
glomerulonephritis via renal biopsy and other appropriate tests may enhance early renal recovery, because
appropriate therapy can be initiated promptly and aggressively. The number of crescents, the type of
crescents (ie, cellular vs fibrous), and the serum creatinine level at the time of presentation may dictate
prognosis for renal recovery in this subgroup of patients.
Approximately 20-60% of patients experiencing AKI require dialysis during their hospital stay. The
majority of these patients recover, with only 25% requiring long-term renal replacement therapy.
A large cohort study demonstrated that proteinuria coupled with low baseline GFR is associated with a
higher incidence of AKI and should be considered as an identifying factor for individuals at risk. An
occurrence of AKI by itself also has significant negative prognostic implications. [7]
One published study examining AKI after elective surgery in more than 200,000 patients older than 66
years suggested that patients taking statins had a lesser incidence and severity of AKI and lower mortality
than individuals not on statins. Furthermore, the incidence and severity correlated with the potency of the
statin as well. As the study was a retrospective review, the authors were not able to recommend routine
preoperative administration of statins; however, the study certainly suggests that statins should not be
routinely discontinued prior to elective surgery.[8]
Educating patients about the nephrotoxic potential of common therapeutic agents is always helpful. A good
example is NSAIDs; most patients are unaware of their nephrotoxicity, and their universal availability
makes them a constant concern.

History
A detailed and accurate history is crucial to the diagnosis of the type of acute kidney injury (AKI) that a
patient has and to determining the diseases subsequent treatment.
Distinguishing AKI from chronic renal failure is important, yet making the distinction can be difficult. A
history of chronic symptomsfatigue, weight loss, anorexia, nocturia, and pruritussuggests chronic renal
failure.
Take note of the following findings during the physical examination:
Hypotension
Volume contraction
Congestive heart failure
Nephrotoxic drug ingestion
History of trauma or unaccustomed exertion
Blood loss or transfusions
Evidence of connective tissue disorders or autoimmune diseases
Exposure to toxic substances, such as ethyl alcohol or ethylene glycol
Exposure to mercury vapors, lead, cadmium, or other heavy metals, which can be encountered in
welders and miners
People with the following comorbid conditions are at a higher risk for developing AKI:
Hypertension
Congestive cardiac failure
Diabetes
Multiple myeloma
Chronic infection
Myeloproliferative disorder
Urine output history can be useful. Oliguria generally favors AKI. Abrupt anuria suggests acute urinary
obstruction, acute and severe glomerulonephritis, or embolic renal artery occlusion. A gradually
diminishing urine output may indicate a urethral stricture or bladder outlet obstruction due to prostate
enlargement.
Because of a decrease in functioning nephrons, even a trivial nephrotoxic insult may cause AKI to be
superimposed on chronic renal insufficiency.
Acute kidney injury (AKI) has a long differential diagnosis. History can help classify the pathophysiology
of AKI as prerenal, intrinsic renal, or postrenal failure, and it may suggest some specific etiologies.
Prerenal failure
Patients commonly present with symptoms related to hypovolemia, including thirst, decreased urine output,
dizziness, and orthostatic hypotension.
Elders with vague mental status change are commonly found to have prerenal or normotensive ischemic
AKI.
Ask about volume loss from vomiting, diarrhea, sweating, polyuria, or hemorrhage.
Patients with advanced cardiac failure leading to depressed renal perfusion may present with orthopnea and
paroxysmal nocturnal dyspnea.
Insensible fluid losses can result in severe hypovolemia in patients with restricted fluid access and should
be suspected in elderly patients and in comatose or sedated patients.
Intrinsic renal failure
Patients can be divided into those with glomerular etiologies and those with tubular etiologies of AKI.
Nephritic syndrome of hematuria, edema, and HTN indicates a glomerular etiology of AKI. Query about
prior throat or skin infections.
ATN should be suspected in any patient presenting after a period of hypotension secondary to cardiac
arrest, hemorrhage, sepsis, drug overdose, or surgery.
A careful search for exposure to nephrotoxins should include a detailed list of all current medications and
any recent radiologic examinations (ie, exposure to radiologic contrast agents).
Pigment-induced AKI should be suspected in patients with possible rhabdomyolysis (muscular pain, recent
coma, seizure, intoxication, excessive exercise, limb ischemia) or hemolysis (recent blood transfusion).
Allergic interstitial nephritis should be suspected with fevers, rash, arthralgias, and exposure to certain
medications, including NSAIDs and antibiotics.
Postrenal failure
Postrenal failure usually occurs in older men with prostatic obstruction and symptoms of urgency,
frequency, and hesitancy. Patients may present with asymptomatic, high-grade urinary obstruction because
of the chronicity of their symptoms.
A history of prior gynecologic surgery or abdominopelvic malignancy often can be helpful in providing
clues to the level of obstruction.
Flank pain and hematuria should raise a concern about renal calculi or papillary necrosis as the source of
urinary obstruction.
Use of acyclovir, methotrexate, triamterene, indinavir, or sulfonamides implies the possibility of tubular
obstruction by crystals of these medications.
Physical Examination
Obtaining a thorough physical examination is extremely important when collecting evidence about the
etiology of acute kidney injury (AKI).
Skin
Skin examination may reveal the following:
Livido reticularis, digital ischemia, butterfly rash, palpable purpura - Systemic vasculitis
Maculopapular rash - Allergic interstitial nephritis
Track marks (ie, intravenous drug abuse) - Endocarditis
Petechiae, purpura, ecchymosis, and livedo reticularis provide clues to inflammatory and vascular causes of
AK.
Infectious diseases, TTP, DIC, and embolic phenomena can produce typical cutaneous changes.
Eyes
Eye examination may reveal the following:

The most important part of the physical examination is the assessment of cardiovascular and volume status.
The physical examination must include pulse rate and blood pressure recordings measured in the supine
position and the standing position; close inspection of the jugular venous pulse; careful examination of the
heart, lungs, skin turgor, and mucous membranes; and assessment for the presence of peripheral edema.
In hospitalized patients, accurate daily records of fluid intake and urine output and daily measurements of
patient weight are important. Hypovolemia leads to hypotension; however, hypotension may not
necessarily indicate hypovolemia.
Severe CHF may also cause hypotension. Although patients with CHF may have low blood pressure,
volume expansion is present and effective renal perfusion is poor, which can result in AKI.
Severe hypertension with renal failure suggests renovascular disease, glomerulonephritis, vasculitis, or
atheroembolic disease.
Abdomen
Abdominal examination may reveal the following:
Abdominal examination findings can be useful to help detect obstruction at the bladder outlet as the cause
of renal failure, which may be due to cancer or an enlarged prostate.
The presence of tense ascites can indicate elevated intra-abdominal pressure that can retard renal venous
return and result in AKI.
The presence of an epigastric bruit suggests renal vascular hypertension, which may predispose to AKI.

Diagnostic Considerations
Although acute kidney injury (AKI) potentially is a reversible condition, it can occur in patients with
chronic renal failure. Every effort should be made to identify reversibility, even if improvement in renal
function is marginal. The best way to identify reversibility is by tracking the rate of deterioration of renal
function. If the rate of worsening renal function accelerates, the cause should be sought and treated.

Differentials
Several laboratory tests are useful for assessing the etiology of AKI, and the findings can aid in proper
management. These tests include complete blood count (CBC), serum biochemistries, urine analysis with
microscopy, and urine electrolytes.
In some cases, renal imaging is useful, especially if renal failure is secondary to obstruction. The American
College of Radiology recommends ultrasonography, preferably with Doppler methods, as the most
appropriate imaging method in AKI.[9]
Overview
Lack of a systematic definition of acute renal failure (ARF) previously led to significant confusion
clinically and in the medical literature. In 2004, the Acute Dialysis Quality Initiative (ADQI) group
published the RIFLE classification of ARF, based on changes from the patient's baseline either in serum
creatinine level, glomerular filtration rate (GFR), or urine output (UO).
The RIFLE classification of ARF is as follows[1] :
Risk (R) - Increase in serum creatinine level X 1.5 or decrease in GFR by 25%, or UO < 0.5
mL/kg/h for 6 hours
Injury (I) - Increase in serum creatinine level X 2.0 or decrease in GFR by 50%, or UO < 0.5
mL/kg/h for 12 hours
Failure (F) - Increase in serum creatinine level X 3.0, decrease in GFR by 75%, or serum
creatinine level 4 mg/dL with acute increase of >0.5 mg/dL; UO < 0.3 mL/kg/h for 24 hours, or
anuria for 12 hours
Loss (L) - Persistent ARF, complete loss of kidney function >4 weeks
End-stage kidney disease (E) - Loss of kidney function >3 months
Since baseline serum creatinine level and GFRs may not be readily available, the consensus committee
recommended the use of the Modification of Diet in Renal Disease (MDRD) equation to estimate the
patients GFR/1.73 mm based on serum creatinine level, age, gender, and race. The proportional decrease in
GFR should be calculated from 75 mL/min per 1.73 mm2, the agreed upon lower limit of normal.
ARF is a common entity in the emergency department (ED). Emergency physicians play a critical role in
recognizing early ARF, preventing iatrogenic injury, and reversing the course of ARF.
Imaging studies in acute renal failure (ARF) are most important in the emergent workup of suspected
postrenal azotemia.
The distinction between community- and hospital-acquired acute renal failure (ARF) is important for the
differential diagnoses, treatment, and eventual outcome of patients with ARF.

d. obati ISK dg.antibiotik non-nefrotoksik
e. Obati asidosis metabolik dg NaHCO3 tab/I.v.
f. Obati hiperurisemi/kel.sendi dg.diet&obat
a. diet rendah protein(GFR 5-10% 40-50g/h; GFR 4-5% protein 20-30 g/h; kalori harus> 2500
kal/hari
b. Asam amino esnsial
c. Gatal(pruritus): Diet TKRP, radiasi UV,difenhidramin paratiroidektomi, transplantasi ginjal
d. Kel.GIT: kadang membaik dg diet TKRP,memperbaiki asidosis dengan NaHCO3, obat anti
