 KETENTUAN DAN SYARAT – SYARAT
TERMS AND CONDITIONS

1. Jasa kami hanya terbatas pada pengujian dan kalibrasi yang tertera pada konfirmasi order ini.
Our services are subject to terms and conditions described in the order confirmation.
2. 2Biaya pengujian / kalibrasi DIBAYAR DI MUKA, kecuali ada perjanjian khusus.
Testing and calibration fees are subject to PAYMENT IN ADVANCE, except when prior arrangement have been made with the customer.
3. Contoh yang sudah diterima sesudah jam 14.00 dihitung pada hari kerja berikutnya. Sabtu, Minggu dan hari libur resmi tidak dihitung hari
kerja.
Sample (s) received after 2:00 P.M will be considered as submission on the following working day. Saturday, Sunday and other public
holiday are not working days.
4. Beberapa pengujian dan kalibrasi tertentu waktu penyelesaian dapat dipercepat dengan ketentuan :
Menjadi 3-4 hari kerja dengan tambahan biaya 75%
Menjadi 1-2 hari kerja dengan tambahan biaya 150%
Express services are available for certain test. Results could be obtained within :
3-4 working days available at a surcharge of 75%
1-2 working days available at a surcharge of 150%
5. Pengambilan contoh di lokasi dan penyampaian hasil pengujian dan kalibrasi ke alamat pelanggan dapat dilayani pada area yang masih
dapat terjangkau oleh laboratorium. Biaya yang timbul karena hal ini menjadi tanggungan pelanggan dan akan ditagih bersamaan dengan
penagihan biaya pengujian dan kalibrasi.
The service of taking samples and delivering of the result to customer are available at extra charge to the customer.
6. Laporan hasil penguijan dan kalibrasi akan diterbitkan setelah seluruh pengujian dan kalibrasi selesai di kerjakan.
Test result will be issued after all testing and calibration are completed.
7. Sisa contoh akan dimusnahkan paling lama 30 hari kalender terhitung sejak tanggal penerimaan , khusus order lab teknik, sampel akan
disimpan maksimal selama 3 bulan. Untuk contoh yang mudah mengalami perubahan sifat fisika, kimia dan mikrobiologinya, setelah
selesai pengujian segera dimusnahkan.
Remaining samples which do not deteriorate easily, will be kept after analysis for maximum of 30 days effective from the date of receipt,
specially for technical laboratory orders, samples will be kept for maximum of 3 months after that time the samples will be disposed off
without notice they can. Samples which deteriorate easily will be directly destroyed after being analyzed.
8. Permintaan pengujian ulang atas sisa contoh yang masih disimpan oleh laboratorium jika diperoleh hasil pengujian yang sama dengan
sebelumnya akan dikenakan biaya seperti contoh baru.
A request for re-test of retaining sample (s) in the laboratory, will be treated as a new sample and subject to all the above charges, except
when the results obtained are not the same.
9. Bagi pelanggan yang memerlukan faktur pajak, harus memberikan NPWP. Kami tidak melayani penerbitan faktur pajak susulan. Untuk
perbaikan invoice dan faktur pajak maksimal 1 x 24 jam setelah diterima.
Customer should be submitted tax number in order to issue tax receipt, for revision of invoice and tax maximal 1 x 24 hour after being
received.
10. Pembatalan atas permintaan pelanggan maksimal 1x24 jam. Apabila lebih dari 1x24 jam maka pengujian tetap dilakukan dan di tagihkan
100%.
Cancellation at customer request maximum 1x24 hours. If more than 1x24 hours, testing will still be carried out and billed 100%.
11. Contoh yang dikirim harus dalam kemasan yang sesuai dan tertutup rapat untuk menjamin keamanan penanganannya oleh karyawan
laboratorium. Jika contoh mengandung bahan-bahan berbahaya, pelanggan harus memberikan cara penanganannya kepada Customer
Service Officer laboratorium.
Sample (s) to be sent shall be placed in suitable container and properly closed to be ensure its safely handling by laboratory’s personnel. If
the sample (s) contains hazardous materials, customer shall inform on its handling methods to the laboratory’s Customer Service Officer.
12. Pelanggan harus menjamin keselamatan dan kesehatan kerja karyawan laboratorium yang melakukan kegiatan pengujian/ sampling/
kalibrasi di lokasi pelanggan.
Customer shall ensure the health and safety of the laboratory’s personnel (s) who is conducting the testing/ sampling/ calibrating activities
on the customer’s site.
13. Ketentuan dan syarat-syarat Laboratorium Sucofindo tunduk kepada Syarat dan Ketentuan Umum Layanan Jasa PT. SUCOFINDO yang
dapat diakses di website kami www.sucofindo.co.id .
Terms and condition Sucofindo laboratory refer to General Terms and Condition PT. Sucofindo and can also be accesed at our website
www.sucofindo.co.id .
14. Kami senantiasa berupaya untuk menjaga integritas dan kredibilitas dalam pelaksanaan jasa pengujian/kalibrasi yang kami berikan, untuk
itu kami sangat menghargai apabila Pelanggan tidak memberikan/menerima tanda terima kasih dalam bentuk apapun yang dapat
menurunkan kredibiltas jasa pelayanan kami. . Jika terindikasi adanya praktek dimaksud, maka pelaksanaan pekerjaan akan ditunda dan
para pihak sepakat untuk melakukan investigasi terhadap dugaan penyuapan di masing-masing Perusahaan dan siap untuk melalui proses
hukum sesuai dengan peraturan perundang-undangan yang berlaku.
We always strive to maintain integrity and credibility in the implementation of the testing/calibration services that we provide, for that we
really appreciate it if the customer does not give/receive a sign of gratitude in any form that can reduce the credibility of our services. . If
there are indications of such practice, the execution of the work will be postponed and the parties agree to investigate the alleged bribery
in each company and are ready to go through legal processes in accordance with the applicable laws and regulations.
