Kesakitan dan kematian akibat hipoksemia dan asidosis. Kerusakan SSP akibat dari :
Hipoksemia berkelanjutan selama episode tenggelam (cedera primer) Aritmia cedera pulmoner, gangguan reperfusi, ataupun disfungsi multiorgan (cedera sekunder), khususnya hipoksia jaringan memanjang.
Berdasarkan derajat hipoksemia dan perubahan keseimbangan asam basa, dapat terjadi disfungsi miokard dan instabilitas elektrik, henti jantung, dan iskemia SSP. Fluid aspirated into the lungs produces vagally mediated pulmonary vasoconstriction and hypertension.

Air tawar
Dapat masuk ke sirkulasi pulmoner secara osmosis. Hemodilusi menyebabkan hemolisis. Peningkatan K+ plasma dan penurunan kadar Na+ menyebabkan perubahan aktivitas elektrik jantung sehingga terjadi Ventrikel fibrilasi. Gagal ginjal akut dapat terjadi oleh karena penumpukan hasil hemolisis di ginjal, dan dapat terjadi henti jantung bila air dingin masuk ke aliran darah.
Air asin
Bersifat hipertonik. Osmosis menyebabkan darah masuk ke dalam paru-paru, sehingga menyebabkan darah menjadi lebih kental.
Freshwater moves rapidly across the alveolarcapillary membrane into the microcirculation. Freshwater is considerably hypotonic relative to plasma and causes disruption of alveolar surfactant. Destruction of surfactant produces alveolar instability, atelectasis, and decreased compliance, with marked ventilation/perfusion (V/Q) mismatching. As much as 75% of blood flow may circulate through hypoventilated lungs.
Saltwater, which is hyperosmolar, increases the osmotic gradient and therefore draws fluid into the alveoli, diluting surfactant (surfactant washout). Protein-rich fluid then exudates rapidly into the alveoli and pulmonary interstitium. Compliance is reduced, the alveolar-capillary basement membrane is damaged directly, and shunting occurs. This results in rapid induction of serious hypoxia.

Special care has to be taken for people with suspected spinal injuries, and a back board (spinal board) may be needed for the rescue. In water, CPR is ineffective, and the goal should be to bring the person to a stable ground quickly and then to start CPR. Once on ground chest compressions are performed if the patient is pulseless, and if they are not breathing rescue breaths.
The Heimlich maneuver is not recommended; the technique may have relevance in situations where airways are obstructed by solids but not fluids. Performing the manoeuver on drowning people not only delays ventilation but may induce vomiting, which if aspirated will place the patient in a far worse situation.
Resusitasi Inisial
Resusitasi mulut ke mulut dimulai sejak masih berada di dalam air. Dengan hati-hati sangga leher korban untuk mencegah terjadinya cedera spinal. CPR penuh harus segera dilakukan saat korban telah dibawa ke daratan. Resusitasi harus dilanjutkan sekurang-kurangnya hingga tubuh pasien kembali hangat. Lepaskan pakaian yang basah, keringkan tubuh korban dan berikan hembusan O2 hangat.
