DEFINISI

Preeklampsia merupakan hipertensi pada

kehamilan dengan gangguan multisistem yang biasanya
terjadi setelah usia kehamilan lebih dari 20 minggu.
Preeklampsia selain ditandai dengan adanya
hipertensi juga diikuti dengan proteinuria dan edema.
Proteinuria adalah gejala yang dapat menggambarkan
kebocoran sistem endothelial yang merupakan tanda
dari preeklampsia
EPIDEMIOLOGI
Hipertensi dalam kehamilan merupakan 5 - 15 % penyulit kehamilan dan
merupakan salah satu dari tiga penyebab tertinggi mortalitas dan
morbiditas ibu bersalin.

Indonesia merupakan salah satu negara berkembang dengan Angka

Kematian Ibu (AKI) dan kematian perinatal tinggi yaitu tertinggi ketiga di
%kawasan South East Asian Nation
ASEAN dan tertinggi kedua di
Regional Organization (WHO, 2013).

KLASIFIKASI
Preeklampsia Ringan

• Preeklampsia ringan adalah suatu sindroma spesifik kehamilan dengan menurunnya

perfusi organ yang berakibat terjadinya vasospasme pembuluh darah dan aktivasi
endotel.

Preeklampsia Berat

• Preeklampsia berat ialah preeklampsia dengan tekanan darah sistolik > 160 mmHg
dan tekanan darah diastolik > 110 mmHg disertai proteinuria lebih 5 g/24 jam.
• Preeklamsia berat dibagi menjadi
- Preeklamsia berat tanpa impending eclampsia.
- Preeklamsia berat dengan impending eclampsia
KLASIFIKASI
Kondisi Kriteria

Klasifikasi dan Diagnosis ACOG (2013)

FAKTOR RESIKO
• Obesitas atau DM gestasional
• Penyakit ginjal
• Usia <18 tahun atau >40 tahun
• Preeclampsia pada kehamilan sebelumnya
• Sindrom antibody antifosfolipid
• Hipertensi kronik
• Riwayat keluarga dengan preeklampsi
• Nuliparitas
• Wanita melahirkan bayi kecil sesuai masa kehamilan
• Riwayat kehamilan buruk
• IUGR, IUFD dan solusio plasenta
• DM tipe 1.
PATOFISIOLOGI
 PATOFISIOLOGI
Sirkulasi uteroplasenta pada kehamilan
normal dan preeklampsia

(A) : kehamilan normal terjadi perubahan pada

cabang arteri spiralis dari dinding otot yang tebal
menjadi dinding pembuluh darah yang lunak
sehingga memungkinkan terjadinya sejumlah aliran
darah ke uteroplasenta.

(B) : preeklampsia, perubahan arteri spiralis ini

tidak terjadi dengan sempurna sehingga dinding otot
tetap kaku dan sempit dan akibatnya akan terjadi
penurunan aliran darah ke sirkulasi uteroplasenta
yang mengakibatkan hipoksia.
 PATOFISIOLOGI

Dalam kehamilan normal perubahan besar terjadi

dalam arteri spiral untuk memungkinkan peningkatan
suplai darah ke ruang intervillous dalam rangka
memenuhi kebutuhan unit feto-plasenta selama
tahap akhir kehamilan. Pre-eklampsia ditandai oleh
kegagalan remodeling spiral arteri.
DIAGNOSIS
Anamnesis

• Hamil usia >20 minggu

• Nyeri kepala (pusing)

• Nyeri ulu hati atau nyeri pada kuadran

kanan atas abdomen
• Pandangan kabur

• BAK sedikit
• BB naik 1,5 kg atau lebih dalam
seminggu yang berulang

• Bengkak pada lengan, muka,

perut, atau edema geralisata

Rawat Jalan • Banyak istirahat (berbaring/tidur miring)

• Bila tidak ada perbaikan :

• Tekanan darah, kadar proteinuria
Rawat Inap selama 2 minggu
• Adanya satu atau lebih gejala dan
tanda-tanda preeklampsia berat
TATALAKSANA

Preeklamsia Berat

Pada kasus preeklampsia yang berat dan eklampsia,

magnesium sulfat yang diberikan secara parenteral adalah obat
anti kejang yang efektif tanpa menimbulkan depresi susunan
syaraf pusat baik bagi ibu maupun janinnya. Obat ini dapat diberi
kan secara intravena melalui infus kontinu atau intramuskular
dengan injeksi intermiten
TATALAKSANA

Preeklamsia Berat
Infus intravena kontinu :

• Berikan dosis bolus 4 – 6 gram MgSO4 yang diencerkan

dalam 100 ml cairan dan diberikan dalam 15-20 menit.
• Mulai infus rumatan dengan dosis 2 g/jam dalam 100 ml
cairan intravena selama 6 jam.
• Ukur kadar MgSO4 pada 4-6 jam setelah pemberian dan
disesuaikan kecepatan infuse untuk mempertahankan kadar
antara 4 dan 7 mEg/l (4,8-8,4 mg/l).
TATALAKSANA

Preeklamsia Berat

Injeksi intramuskular intermiten:

• Berikan 4 gram MgSO4 sebagai larutan 20% secara intravena

dengan kecepatan tidak melebihi 1 g/menit. Lanjutkan segera
dengan 10 gram MgSO4 50%, sebagian (5%) disuntikan
dalam di kuadran lateral atas bokong (penambahan 1 ml
lidokain 2 % dapat mengurangi nyeri).
• Apabila kejang menetap setelah 15 menit, berikan MgSO4
sampai 2 gram dalam bentuk larutan 20% secara intravena
dengan kecepatan tidak melebihi 1g/menit.
 TATALAKSANA
Preeklamsia Berat

Injeksi intramuskular intermiten:

• Setiap 4 jam sesudahnya, berikan 5 gram larutan MgSO4 50%

yang disuntikan dalam ke kuadran lateral atas bokong bergantian
kiri-kanan, tetapi setelah dipastikan bahwa:

 Refleks patela (+)

 Tidak terdapat depresi pernapasan (frekuensi >16x/menit)
 Pengeluaran urin selama 4 jam sebelumnya melebihi 100 ml
 Harus sedia antidotum (kalsium glukonas 10% dalam 10 cc = 1 g).
 MgSO4 dihentikan 24 jam setelah bayi lahir atau 24 jam setelah
kejang berakhir atau jika ada tanda-tanda intoksikasi.
 TATALAKSANA
Obat - obat hipertensi
a. Penghambat adrenergik
• Adrenolitik sentral
- Metildopa : 3x125 mg/hari
sampai 3x500 mg/hari.
- Klonidin : 3x0,1 mg/hari
atau 0,3 mg/500 ml
glukosa 5%/6jam
• Beta bloker
- Pindolol : 1x5 mg/hari sampai
3x10 mg/hari
• Alfa bloker
- Prazosin : 3x1 mg/hari sampai
3x5 mg/hari
• Alfa dan Beta Bloker
- Labetolol : 3x100 mg/hari
b. Vasodilator
-Hidralazin : 4x25 mg/hari atau
parenteral 2,5 mg – 5 mg
c. Antagonis kalsium
-Nifedipin : 3 x 10 mg/hari.
TATALAKSANA

Tindakan terminasi kehamilan

Pada preeklamsia berat tidak membaik setelah rawat

inap, demi keselamatan ibu dan janin biasanya
dianjurkan untuk terminasi kehamilan. Persalinan induksi
dilakukan dengan sebelumnya memberikan pematangan
paru terlebih dahulu. Jika ada tanda gagal induksi
disarankan untuk sesar
PENCEGAHAN
 Tirah baring
 Pemberian suplemen yang mengandung
minyak ikan, antioksidan, kalsium.
 Penggunaan diuretik, zinc 200 mg/hari, magnesium 365
mg/hari, Antitrombotik : aspirin dosis rendah rata – rata di
bawah 100 mg/hari, Pemberian kalsium: 1.500- 2.000 mg/
hari dapat dipakai sebagai suplemen pada risiko tinggi
terjadinya preeklampsia.
 Antioksidan, misalnya vitamin C, vitamin E, B-karoten,
CoQro, N-Asetilsistein, asam lipoik
PROGNOSIS

Pada keadaan yang tidak berkembang menjadi eklampsia, angka

mortalitas rendah, tetapi harus diingat bahwa preeclampsia
merupakan salah satu faktor yang menyebabkan kematian
maternal. Prognosis maternal dan neonatal pada preeklampsia
ditentukan oleh faktor-faktor seperti usia gestasi saat mengalami
penyakit, tingkat keparahan penyakit, kualitas manajemen dan,
adanya penyakit berat sebelumnya
 KESIMPULAN

Preeklampsia merupakan hipertensi pada kehamilan dengan gangguan

multisistem yang biasanya terjadi setelah usia kehamilan lebih dari 20 minggu.
Preeklampsia selain ditandai dengan adanya hipertensi juga diikuti dengan
proteinuria dan edema.
Etiologi dari terjadinya preeklampsi adalah Peran Prostasiklin dan
Tromboksan, Faktor Imunologis, Faktor Genetik, Iskemik dari uterus, Defisiensi
kalsium, Disfungsi dan aktivasi dari endothelial. Preeklampsia dapat diklasifikasikan
menjadi dua golongan yaitu preeklamsi ringan dan preeklamsi berat.
KESIMPULAN

Preeklamsia berat dibagi menjadi preeklamsia berat tanpa impending

eclampsia dan preeklamsia berat dengan impending eclampsia jika disertai gejala-
gejala subjektif seperti nyeri kepala hebat, gangguan visus, muntah-muntah, nyeri
epigastrium, kenaikan progresif tekanan darah.
Beberapa faktor risiko preeklampsia ialah Obesitas atau DM gestasional,
Penyakit ginjal, Usia <18 tahun atau >40 tahun, Preeklampsia pada kehamilan
sebelumnya, Sindrom antibody antifosfolipid, Hipertensi kronik, Riwayat keluarga
dengan preeklampsi, Nuliparitas, Wanita melahirkan bayi kecil sesuai masa
kehamilan, Riwayat kehamilan buruk, IUGR, IUFD dan solusio plasenta Dan DM
tipe 1.
KESIMPULAN

Untuk diagnosis preeklampsia dapat ditegakkan dari gambaran klinik dan

pemeriksaan laboratorium. Pengobatan dilakukan secara simptomatis, karena faktor
penyebab yang belum diketahui secara pasti. Tujuan dari penangannannya adalah
Mencegah terjadinya eklampsi, Anak harus lahir dengan kemungkinan hidup besar,
Persalinan harus dengan trauma yang sedikit-sedikitnya, mencegah hipertensi yang
menetap.
Tujuan utama perawatan preeklampsia adalah mencegah kejang, perdarah
an intrakranial, mencegah gangguan fungsi organ vital, dan melahirkan bayi sehat.
Pencegahan dapat dilakukan dengan cara tirah baring dan pemberian suplemen se
perti zinc, vitamin C, vitamin E, B-karoten, CoQro, N-Asetilsistein, asam lipoik.
