IMUNHEMATOLOGI
• ilmu yang mempelajari reaksi antigen (Ag) dan antibodi (Ab)
pada sel darah, khususnya sel darah merah.
• Konsep imunohematologi yang digunakan dalam bidang
pelayanan transfusi darah adalah pemeriksaan sebelum
transfusi (pre-transfusi test) dan mendeteksi adanya reaksi
transfusi yang ditandai adanya Ab terhadap sel darah.
• Selain pada proses transfusi, kondisi lain yang dapat mencetus
Ab terhadap sel darah adalah proses kehamilan.
ANTIGEN DAN ANTIBODI
• Imunohematologi yang diaplikasikan pada transfusi darah lebih
mengutamakan reaksi antara antigen (Ag) pada sel darah merah
dengan antibodi (Ab) pada serum/plasma.
• Ag merupakan unsur biologis yang mempunyai bentuk dengan
struktur kimia yang kompleks dan mempunyai berat molekul cukup
besar untuk menstimulus Ab. Oleh karena itu, umumnya jenis Ag
berasal dari molekul protein.
• Epitop (antigen determinan) merupakan bagian dari Ag yang bereaksi
dengan Ab atau dengan reseptor spesifik pada limfosit T.
• Epitop ini berada pada molekul pembawa sel darah merah, sehingga
pada permukaan membran sel darah merah, terdapat banyak epitop
yang menentukan spesifisitas dan kekuatan reaksi Ag dan Ab, seperti
terlihat pada Gambar 1.3.
