Abstrak
Hasil : Dari 4062 subjek VKC, 91 (157 mata) memiliki glaukoma yang diinduksi
steroid (SIG), menunjukkan prevalensi 2,24%. Dari jumlah tersebut, 87% adalah
laki-laki. Usia rata-rata (IQR) saat onset VKC adalah 12 tahun (7-17). Pada
presentasi, durasi median VKC adalah 48 bulan (24-72) dan durasi median
penggunaan steroid adalah 24 bulan (12-36). Rasio cup-to-disc (CDR) rata-rata
adalah 0,9 (0,7-0,9) dan rata-rata deviasi adalah -21,9 dB (-30,0 hingga -10,2).
TIO dikontrol secara medis pada 66% mata (104/157) dan 34% mata (53/157)
membutuhkan operasi glaukoma. TIO presentasi tinggi (OR: 1,04; p = 0,05) dan
peningkatan durasi penggunaan steroid (OR: 1,07; p = 0,02) secara signifikan
terkait dengan kebutuhan untuk operasi glaukoma. Pada presentasi, 29/91 subjek
(31,8%) buta bilateral karena SIG. CDR yang lebih tinggi pada presentasi secara
bermakna dikaitkan dengan kebutaan pada kelompok ini (p = 0,02).
Kesimpulan : Dalam kohort VKC dengan SIG ini, penyakit ini terutama
menyerang pria remaja. sepertiga penderita glaukoma berat membutuhkan operasi
dan sepertiga buta karena SIG.
LATAR BELAKANG
India memiliki iklim tropis, dengan cuaca panas dan lembab, membuat
orang rentan terhadap beberapa penyakit mata alergi.1 Vernal keratoconjunctivitis
(VKC), penyakit mata alergi kronis memengaruhi anak laki-laki yang belum
dewasa dan sering sembuh secara spontan saat pubertas. 2–8 VKC yang parah tidak
merespons baik untuk obat anti alergi dan sering membutuhkan steroid topikal
untuk mengontrol episode akut alergi dan peradangan. Penggunaan steroid topikal
yang berkepanjangan menyebabkan mata ini mengalami komplikasi seperti
katarak, peningkatan tekanan intraokular (IOP) dan glaukoma. 5-7 Terapi alternatif
yang lebih baru, menggunakan imunomodulator seperti siklosporin A dan
tacrolimus, menghambat sel T sehingga mengurangi sitokin inflamasi. Ini
digunakan sebagai agen hemat steroid dan untuk mencegah kekambuhan.9.
Studi saat ini pada SIG mencakup beberapa etiologi dan termasuk rute yang
berbeda dari pemberian steroid.7 17-22
Karena etiologi ini berbeda dalam
kronisitasnya, lamanya pengobatan dan respon terhadap pengobatan, hasil dari
studi ini tidak memberikan pemahaman yang jelas tentang SIG dalam VKC . Oleh
karena itu, penelitian kami bertujuan untuk memahami profil demografi, hasil
manajemen dan besarnya kebutaan karena SIG dalam mata pelajaran VKC dalam
pengaturan perawatan mata tersier.
METODE
Peresepan Steroid
Pengobatan
Dari 157 mata dengan SIG, 86 (54,7%) berada di RUPS saat presentasi.
Median (IQR) jumlah RUPS saat presentasi adalah 2 (1-3) turun. Penghentian
steroid tetes mata dengan AGM jangka pendek membantu untuk mengontrol TIO
hanya dalam tiga mata dengan OHT. 154 mata yang tersisa membutuhkan AGM
topikal jangka panjang untuk kontrol TIO (98%). TIO dikontrol secara medis pada
66% (104 mata) dan 34% (53 mata) membutuhkan perawatan bedah. Dari 53 mata
yang membutuhkan pembedahan, trabeculectomy dilakukan pada 37 mata (30
menjalani trabeculectomy sendiri dan 7 memiliki trabeculectomy dengan
mitomycin C) dan aspirasi lensa gabungan, implantasi lensa intraokular dengan
operasi glaukoma dilakukan pada 16 mata. TIO menurun secara signifikan pada
ketiga kelompok intervensi: pada kelompok obat dari 22,7 ± 12,6 menjadi 15,8 ±
7,6 mm Hg (p <0,001); dalam kelompok trabeculectomy dari 30,3 ± 13,3 ke 13,1
± 3,9 mm Hg (p <0,001); dan pada kelompok operasi gabungan dari 28,9 ± 12,1
hingga 13,0 ± 2,9 (p <0,001) mm Hg. Tiga puluh empat mata memiliki TIO tinggi
tanpa kerusakan disk (OHT). Dari jumlah tersebut, TIO di 4 mata dapat dikontrol
tanpa RUPS (hanya dengan menghentikan steroid topikal) dan 19 mata
membutuhkan RUPS untuk kontrol TIO jangka panjang. Enam mata menjalani
trabeculectomy, dan lima menjalani operasi katarak dan glaukoma untuk kontrol
TIO. Semua 123 mata dengan kerusakan disk glaukoma dan IOP tinggi
membutuhkan AGM. Dari jumlah tersebut, TIO dikontrol secara medis pada 69
mata, sementara 31 mata membutuhkan trabeculectomy dan 11 mata
membutuhkan kombinasi operasi katarak dan glaukoma untuk kontrol TIO dan 12
mata mengalami peningkatan TIO menunggu intervensi bedah. Tabel 2
menggambarkan faktor-faktor yang terkait dengan kebutuhan untuk operasi di
mata dengan VKC dan glaukoma. Durasi penggunaan steroid yang lebih lama
(OR: 1,04; p = 0,02) dan TIO yang lebih tinggi (OR: 1,07; p = 0,05) secara
statistik signifikan terkait dengan kebutuhan untuk operasi (gambar 1A dan B).
Ketajaman visual median logMAR pada follow-up terakhir adalah 0,2 (0-0).
Delapan mata memiliki peningkatan ketajaman visual mulai dari 20/20 hingga
20/200 pada kunjungan terakhir karena trabeculectomy atau operasi gabungan.
Kebutaan
Enam puluh delapan mata dari 39 subjek (68 mata dari 157 mata, 43,3%)
buta pada presentasi karena kerusakan glaukoma lanjut. Dari jumlah tersebut, 32
mata buta sesuai kriteria ketajaman visual dan 36 mata buta karena kriteria VF.
Dua puluh sembilan subjek (58/157, 36,9% mata) buta bilateral, dan 10 subjek
(6,4%) buta secara sepihak. Usia rata-rata subyek buta adalah 17 tahun (IQR:
13,5-21). Tabel 3 menunjukkan faktor-faktor yang memprediksi kebutaan. Jenis
kelamin dikeluarkan dari model karena jenis kelamin laki-laki memprediksi
kebutaan dengan sempurna. CDR yang lebih tinggi pada presentasi diprediksi
kebutaan pada kohort (OR: 1117 (95% CI 2,49 hingga 5009); p = 0,02) (gambar
2). Faktor-faktor lain seperti usia saat onset VKC (p = 0,34), kategori steroid
(0,27), durasi penggunaan steroid (p = 0,26) dan penyajian TIO (p = 0,32) tidak
secara signifikan terkait dengan kebutaan pada kohort kami. Selain SIG, sequalae
yang mengancam penglihatan lain yang terlihat adalah ulkus pelindung di 2 mata,
keratoconus di 4 mata dan defisiensi sel induk limbal dengan vaskularisasi kornea
pada 10 mata. Katarak terkait ada di 85 mata (54%) di mana 16 mata memiliki
katarak signifikan secara visual yang memerlukan intervensi bedah.
Diskusi
Prolonged steroid use in VKC leads to visual impairment and blindness due
to SIG.1 11 14 Gupta et al reported blindness in 37.3%, and it was unilateral in
27% eyes. Blindness was mainly due to topical steroid use and none in the
systemic steroid category were bilaterally blind.11 In our series, 43.4% eyes were
blind at presentation, with three-fourths bilaterally blind and one-fourth
unilaterally blind. More than half or three quarters of these subjects were less than
17 years of age. Higher CDR at presentation predicted blindness in our cohort.
Gupta et al noted that 82.3% of the children who presented with SIG were
prescribed steroid medication. There were some limitations of our study. First,
being a retrospective study, information was obtained from patient records. The
duration of disease and duration of steroid use could be affected by recall bias.
Second, in more than half the subjects, the type of steroid used was not known;
this could have affected the interpretation of the relationship between the type of
steroid and the disease severity and its consequences. Most of the subjects in our
cohort had very severe disease at presentation; this could be partly due to referral
of subjects with severe disease to our tertiary eye care centre, which may be an
overestimation of disease severity. However, this cannot underestimate the
seriousness of the condition and the treatment sequel. MD as a measure of disease
severity may be confounded by other causes like cataract and corneal pathology.
The glaucoma severity was also based on the disc damage, visual acuity and MD.
As ophthalmologists, it is our responsibility to educate the patients about sight-
threatening complications of steroid medications. General physicians and general
ophthalmologists should be educated on the blinding side effects of steroid use
and the importance of monitoring when steroids are prescribed. Awareness and
use of steroid-sparing antiallergic medications can prevent these serious
complications. When steroids are indicated, prescribing a low-dose steroid and
closely monitoring for side effects like elevated IOP could prevent blindness due
to SIG in young children. Educating children and parents on the risk of
unmonitored use of steroid medications and about governmental policies and
raising public awareness on the harm of over-the-counter sale of steroid
medications would go a long way in preventing glaucoma and its blinding sequel
in children with VKC.
