EXHIBIT B – INDEPENDENT CONTRACTOR HEALTH,
ENVIRONMENTAL AND KESEHATAN, KESELAMATAN DAN LINGKUNGAN
SAFETY GUIDELINES
1. RESPONSIBILITY FOR COMPLIANCE
1.1 Contractor shall comply (and ensure that all
members of Contractor Group comply) with all
applicable laws and these Independent Contractor
Health, Environmental and Safety Guidelines
(“Guidelines”) within the Area of Operations. These
Guidelines are intended to promote a safe and
healthful workplace where the Services are
performed without incident and in an
environmentally-sound manner.
1.2 These Guidelines are a minimum standard and are
intended to supplement, not replace, Contractor’s
safety program. Contractor may implement
additional measures, as necessary, to assure
workplace health, environmental and safety
protection, and shall implement all additional
measures required by law.
1.3 The requirement for Contractor to comply with these
Guidelines does not alter Contractor’s status as an
independent contractor, does not change the rights
or obligations Contractor has as an independent
contractor, and does not amend or restrict
Contractor’s liabilities and indemnities provided in
this Contract. All costs associated with compliance
are included in the compensation provided for under
this Contract, and Contractor has no right to claim
any additional payment not specifically provided for
in this Contract because of the requirement for
compliance with these Guidelines.
1.4 Contractor shall communicate these Guidelines to its
employees and to members of the Contractor Group
prior to entering the Area of Operations. Contractor
shall maintain written documentation of its actions
undertaken to fulfill these responsibilities and
maintain a copy of those written documentations on
site.

1. TANGGUNG-JAWAB PEMATUHAN
1.1 Kontraktor wajib mentaati (dan menjamin bahwa
semua anggota Kelompok Kontraktor mentaati)
semua ketentuan perundangan yang berlaku dan
Pedoman Kesehatan, Keselamatan dan
Lingkungan Kontraktor Mandiri ini (“Pedoman”) di
Daerah Operasi. Pedoman ini dimaksudkan untuk
mewujudkan lingkungan kerja yang selamat dan
sehat, dimana Jasa-jasa dilaksanakan tanpa
insiden dan secara ramah lingkungan.
1.2 Pedoman ini adalah suatu acuan minimum dan
dimaksudkan untuk melengkapi, tidak mengganti,
program keselamatan Kontraktor. Kontraktor dapat
menerapkan ketentuan tambahan, sebagaimana
diperlukan, guna menjamin perlindungan
kesehatan, keselamatan dan lingkungan tempat
kerja, dan menerapkan segala ketentuan
tambahan sebagaimana diwajibkan oleh ketentuan
hukum.
1.3 Keharusan Kontraktor untuk mematuhi Pedoman
ini, tidak mengubah status Kontraktor sebagai
kontraktor mandiri, tidak mengubah hak dan
kewajiban Kontraktor sebagai suatu kontraktor
mandiri, dan tidak menambah atau membatasi
tanggung-jawab dan ganti rugi Kontraktor
sebagaimana dimaksud dalam Kontrak. Semua
biaya yang timbul sehubungan dengan pematuhan
ketentuan ini sudah termasuk dalam imbalan yang
diatur dalam Kontrak ini, dan Kontraktor tidak
berhak untuk menuntut imbalan tambahan yang
tidak secara khusus diatur dalam Kontrak ini
sehubungan dengan kewajiban untuk mematuhi
Pedoman ini.
1.4 Kontraktor harus mengkomunikasikan Pedoman ini
kepada pegawai-pegawainya dan kepada anggota-
anggota Kelompok Kontraktor, sebelum memasuki
Daerah Operasi. Kontraktor harus memelihara
dokumentasi tertulis tentang tindakan-tindakan
yang dilakukan untuk memenuhi kewajiban-
kewajiban ini dan menyimpan salinan dokumentasi
tersebut di tempat kerja.
1.5 Kontraktor akan memberitahu Perusahaan, nama

1.6 Company or its representatives may inspect the
performance of the Services in the Area of
Operations at any time to determine compliance with
these Guidelines and prescribe measures to
Contractor to achieve compliance. Contractor or
Contractor Group shall implement steps to achieve
compliance with these Guidelines.
2. HEALTH, ENVIRONMENTAL AND SAFETY WORK
PLAN
2.1 On or before the Effective Date of the Contract,
Company will advise Contractor whether a Health,
Environmental and Safety Work Plan (“Plan”) is
required, and if so, of the scope of the Plan.
Contractor shall prepare the Plan on or before the
Effective Date of the Contract.
2.2 The Plan developed by Contractor shall describe the
health, environmental and safety issues associated
with the Services, and the mitigation measures
required to address these issues before Contractor
or Contractor Group enters the Area of Operations.
Contractor shall ensure that the Plan is based on,
and complies with applicable laws, decrees, codes,
standards, administrative rules and regulations,
relevant Company policy, processes and standards,
these Guidelines and generally accepted good
international industry practice for the industry in
which the Services are to be provided. If an
inconsistency or conflict exists between components
of these laws, decrees, codes, standards, rules,
regulations, policy, these Guidelines and practices,
Contractor shall ensure that it complies with the most
stringent to the extent that it is legal.
2.3 Contractor shall submit Contractor’s Plan to
Company’s Representative for review. Company
has the right, but not the obligation, to review
Contractor’s Plan and either approve the Plan or
return it to Contractor with notice of deficiencies.

2. RENCANA KERJA KESEHATAN, KESELAMATAN
DAN LINDUNGAN LINGKUNGAN
2.1 Pada atau sebelum Tanggal Berlaku Kontrak,
Perusahaan akan memberitahu Kontraktor apakah
suatu Rencana Kerja Kesehatan, Keselamatan dan
Lindungan Lingkungan (“Rencana Kerja”)
diperlukan, dan jika diperlukan, akan memberitahu
lingkup Rencana Kerja. Kontraktor akan
menyiapkan Rencana Kerja pada atau sebelum
Tanggal Berlaku Kontrak.
2.2 Rencana Kerja yang dibuat oleh Kontraktor harus
merinci masalah-masalah kesehatan, lindungan
lingkungan dan keselamatan sehubungan dengan
Jasa-jasa, dan upaya-upaya penanggulangan yang
diperlukan untuk mengatasi masalah-masalah ini
sebelum Kontraktor atau Kelompok Kontraktor
memasuki Daerah Operasi. Kontraktor harus
menjamin bahwa Rencana Kerja didasarkan atas,
dan sesuai dengan ketentuan undang-undang,
keputuan-keputusan, pedoman-pedoman, standar-
standar, keputusan dan peraturan administrasi,
kebijakan Perusahaan terkait, proses-proses dan
standar-standar yang berlaku, Pedoman ini dan
sesuai dengan praktek industri internasional yang
baik untuk industri dimana Jasa-jasa akan
diberikan. Jika ada ketidak-sesuaian atau
pertentangan di antara bagian-bagian dari
ketentuan hukum, keputusan, undang-undang,
pedoman, peraturan, kebijakan, Pedoman ini dan
praktek-praktek, Kontraktor harus menjamin bahwa
ia mematuhi ketentuan yang paling ketat yang sah.
2.3 Kontraktor akan menyerahkan Rencana Kerja
Kontraktor kepada Perwakilan Perusahaan untuk
diperiksa. Perusahaan berhak, tapi tidak
berkewajiban, untuk memeriksa Rencana Kerja
Kontraktor dan menyetujui atau

 Contractor shall correct any deficiencies and
resubmit the Plan for Company’s review. Contractor
shall ensure that the Plan is accepted by Company
before Contractor commences the Services.
Company’s review of the Plan does not absolve
Contractor of its responsibility to develop and
implement a Plan that complies with applicable laws
or these Guidelines.
3. ENTRANCE TO PROPERTY
3.1 Before commencement of performance, Contractor
shall inspect and confirm that the work site within the
Area of Operations is safe and if not, promptly report
any unsafe condition to Company. Failure to
promptly report an unsafe condition to Company
precludes Contractor from asserting any defense or
Claim against Company in reliance on an unsafe
condition which should have been observed during
inspection.
3.2 Contractor shall comply (and ensure that all
members of Contractor Group comply) with
Company’s security requirements that Company
communicates to Contractor during the performance
of the Services.
3.3 Contractor shall ensure that only those employees or
personnel of Contractor or Contractor Group having
authorized business arising out of this Contract are
allowed in the Area of Operations.
4. MOTOR VEHICLE SAFETY
4.1 Contractor shall have and comply (and ensure that
all members of Contractor Group comply) with its
own motor vehicle safety plan (“Motor Vehicle Safety
Plan”) unless the Services provided do not require
the operation of motor vehicles and equipment
(“vehicle”). Company Representative may review
this Motor Vehicle Safety Plan and, if required,
Contractor shall implement improvements, such as a
provision for defensive driving training.

tidak membebaskan Kontraktor dari kewajiban
untuk membuat dan melaksanakan suatu Rencana
Kerja yang sesuai dengan ketentuan perundangan
yang berlaku atau Pedoman ini.
3. MASUK KE AREA
3.1 Sebelum dimulainya pelaksanaan Jasa-jasa,
Kontraktor harus memeriksa dan memastikan
bahwa tempat kerja di Daerah Operasi dalam
keadaan selamat, dan jika tidak, segera
melaporkan setiap keadaan tidak selamat kepada
Perusahaan. Kegagalan untuk segera melaporkan
keadaan yang tidak selamat kepada Perusahaan
menghapuskan hak Kontraktor untuk
menggunakan keadaan tidak selamat tersebut,
yang seharusnya ditemui pada saat pemeriksaan,
dalam tangkisan atau Tuntutan terhadap
Perusahaan.
3.2 Kontraktor harus mematuhi (dan menjamin bahwa
semua anggota Kelompok Kontraktor mematuhi)
ketentuan keamanan Perusahaan yang
diberitahukan oleh Perusahaan kepada Kontraktor
selama pelaksanaan Jasa-jasa.
3.3 Kontraktor harus menjamin bahwa hanya pegawai-
pegawai atau personel Kontraktor atau Kelompok
Kontraktor, yang bertugas dan berwenang
sehubungan dengan Kontrak ini, diperbolehkan
masuk ke Daerah Operasi.
4. KESELAMATAN KENDARAAN BERMOTOR
4.1 Kontraktor harus mempunyai dan tunduk (dan
memastikan bahwa semua anggota Kelompok
Kontraktor tunduk) pada rencana keselamatan
kendaraan bermotor (“Rencana Keselamatan
Kendaraan Bermotor”) kecuali Jasa-jasa yang
disediakan tidak memerlukan pengoperasian
kendaraan bermotor dan alat-alat bermotor
(“Kendaraan”). Perwakilan Perusahaan dapat
memeriksa Rencana Keselamatan Kendaraan
Bermotor ini dan, apabila diperlukan, Kontraktor

4.2 Contractor shall comply (and ensure that all
members of Contractor Group comply) with rules
and regulations, including written instructions
prescribed by Company and communicated to
Contractor, relating to vehicle safety. These include
observation of the posted speed limit, or if not
posted, a safe speed with regard to existing
conditions.
4.3 Contractor shall ensure that all vehicles are in safe
operating condition and operators must be properly
trained, qualified, licensed and/or certified.
Contractor shall ensure that these vehicles are
equipped with seat belts for driver and passengers
and that operator and passengers use seat belts at
all times.
4.4 Except as provided in this Section, the operator of a
vehicle may not use a cellular telephone or any other
communication device, in either hand-held or hands-
free mode, while the vehicle is in motion. These
telephones or communication devices may be left in
an “on” position to alert drivers of an incoming call;
however, calls must not be answered until the
vehicles have been stopped in a safe location. This
guideline does not apply to Contractor’s use of
facility mobile equipment (e.g., forklifts, electric carts,
fire trucks, etc.), dispatch or emergency response
communications, or citizen band radios if previously
approved to do so in writing by Company.
4.5 On or before the Effective Date of the Contract,
Company will advise Contractor whether a driver
monitoring system is required. If a driver monitoring
system is required, Contractor shall have, or develop
and implement such a system. Company may
request review of the system’s monthly data and
recommend measures to improve Contractor’s
vehicular safety performance.

4.3 Kontraktor harus menjamin bahwa semua

berijazah. Kontraktor harus menjamin bahwa
Kendaraan-kendaraan ini dilengkapi dengan sabuk
pengaman untuk pengemudi dan penumpang-
penumpang, dan pengemudi dan penumpang-
penumpang selalu mengenakan sabuk pengaman.
4.4 Kecuali ditentukan dalam Pasal ini, operator
Kendaraan tidak diperkenankan untuk
menggunakan telpon seluler atau alat komunikasi
lain, baik dengan menggenggam atau secara
“hands-free”, selama Kendaraan dalam keadaan
bergerak. Telpon-telpon atau alat-alat komunikasi
ini dapat dibiarkan dalam posisi menyala untuk
menandakan kepada pengemudi tentang adanya
panggilan masuk; namun, panggilan-panggilan
masuk tidak boleh dijawab sampai kendaraan
terhenti di tempat yang aman. Pedoman ini tidak
berlaku untuk penggunaan alat-alat fasilitas mobil
oleh Kontraktor (seperti forklift, kendaraan listrik,
mobil pemadam kebakaraan dsb.), komunikasi
dispatch atau komunikasi tanggap darurat, atau
radio Citizen Band (CB) jika telah disetujui secara
tertulis sebelumnya oleh Perusahaan.
4.5 Pada atau sebelum Tanggal Berlaku Kontrak,
Perusahaan akan memberitahu Kontraktor tentang
perlu tidaknya suatu sistem pemantauan
pengemudi. Jika suatu sistem pemantauan
pengemudi diperlukan, Kontraktor harus
mempunyai, atau mengembangkan dan
menerapkan sistem tersebut. Perusahaan dapat
meminta untuk memeriksa data bulanan dari
sistem tersebut dan mengusulkan upaya-upaya
untuk meningkatkan kinerja keselamatan

4.6 Contractor shall ensure that motorcycles are not
used for performing the Services except if approved
in writing by Company.
5. SMOKING
5.1 Smoking is prohibited in all areas containing crude
oil or fuel storage, gas processing and compression
equipment and separation or treating equipment.
Smoking is only permitted in designated, authorized
areas.
5.2 Company has the right to order Persons observed
smoking in unauthorized areas to cease or to leave
the Area of Operations.
5.3 Smoking is prohibited in Company-provided aircraft,
vessels, light vehicles, buses, heavy equipments,
buildings, offices and warehouses.
6. PERMIT TO WORK
6.1 The “Permit to Work” (“PTW”) applies to work that
requires a specific permit to perform the work. PTW
requires identification of job task hazards, evaluation
of job task risks, specification for control measures to
track performance and use of those control
measures to prescribe improvements to
performance. The purpose of PTW is to prescribe
documented practices to manage and control risks
associated with the particular work. Contractor shall
submit its practice(s) for Company’s review. If
Company determines that Company’s documented
practices for substantially the same work are more
stringent than Contractor’s documented practices,
Company Representative may require Contractor to
follow Company’s documented practices.
6.2 Prior to starting any non-routine, safety critical work
or field activities, Contractor shall perform a Job
Hazard Analysis (“JHA”). Contractor shall ensure
that the JHA accomplishes all of the following:

5. MEROKOK
5.1 Merokok dilarang di semua area dimana ada

bakar, pemrosesan gas dan peralatan kompresi
dan pemisahan, atau peralatan perawatan.
Merokok hanya diperbolehkan di tempat yang
ditetapkan boleh merokok.
5.2 Perusahaan berhak memerintahkan Orang yang
merokok di area tidak boleh merokok untuk
menghentikan merokok atau meninggalkan Daerah
Operasi.
5.3 Merokok dilarang di pesawat terbang, kapal-kapal,
kendaraan kecil, bus, peralatan berat, bangunan,
kantor dan gudang yang disediakan Perusahaan.
6. IJIN BEKERJA
6.1 Ijin Bekerja (“PTW”) berlaku untuk pekerjaan yang
memerlukan ijin khusus untuk melaksanakan
pekerjaan. Untuk mendapatkan PTW diperlukan
identifikasi bahaya pekerjaan, evaluasi resiko tugas
pekerjaan, rincian upaya-upaya pengendalian
untuk memantau kinerja dan penggunaan upaya
pengendalian tersebut guna menentukan
perbaikan kinerja. Tujuan PTW adalah untuk
menentukan cara kerja terdokumentasi untuk
mengelola dan mengontrol resiko-resiko yang
terkait dengan pekerjaan tertentu. Kontraktor harus
menyerahkan cara-kerjanya kepada Perusahaan
untuk diperiksa. Jika Perusahaan menentukan
bahwa cara kerja Perusahaan untuk pekerjaan
yang sama lebih ketat daripada cara kerja
Kontraktor, maka Perwakilan Perusahaan dapat
mewajibkan Kontraktor agar mengikuti cara kerja
Perusahaan.
6.2 Sebelum memulai pekerjaan atau kegiatan
lapangan yang rawan keselamatan dan bersifat
tidak rutin, maka Kontraktor harus melaksanakan
Analisa Bahaya Pekerjaan (“JHA”). Kontraktor
harus menjamin bahwa JHA memuat semua hal

(A) Study and record each step of any non-routine,
safety critical work or field activities.
(B) Identify existing or potential equipment,
environmental or action-generated job hazards.
(C) Determine the best way to perform the job and
mitigate or eliminate hazards and risks.
6.3 Prior to starting any non-routine, safety critical work
or field activities, Contractor shall communicate the
JHA to its employees and those members of
Contractor Group who will perform the non-routine,
safety critical work.
6.4 Before hot work operations are conducted,
Contractor shall obtain a determination from
Company whether a hot work permit is required and
if required, shall develop, implement and comply with
all conditions of any hot work permit.
6.5 Contractor shall develop, implement and comply with
(and ensure that all members of Contractor Group
comply with) written safe-entry procedures for any
Services involving entry into confined spaces, limited
access vessels or below grade pits.
6.6 Contractor shall develop, implement and comply with
an energy isolation system (lock-out/tag-out) before
any member of Contractor Group performs work on
equipment or machinery.
7. PRESSURE TESTING
7.1 Contractor shall ensure that its employees or
Contractor Group do not test pipes or pressure
vessels with or without compressed air, gas or
service fluid, without Company’s prior written
approval.

tidak rutin, Kontraktor harus memberitahu JHA
kepada pegawainya dan anggota Kelompok
Kontraktor yang akan melaksanakan pekerjaan
atau kegiatan lapangan yang rawan keselamatan
dan bersifat tidak rutin.
6.4 Sebelum operasi hot work dilaksanakan,
Kontraktor harus mendapatkan pengarahan dari
Perusahaan apakah suatu ijin hot work diharuskan,
dan apabila diharuskan, harus membuat,
melaksanakan dan mentaati semua ketentuan ijin
hot work.
6.5 Kontraktor harus membuat, menerapkan dan
mentaati (dan menjamin bahwa semua anggota
Kelompok Kontraktor mentaati) prosedur tertulis
tentang masuk secara selamat untuk setiap Jasa-
jasa yang menyangkut masuk ke ruang tertutup,
jalan masuk terbatas bejana atau kolam-kolam
bawah permukaan.
6.6 Kontraktor harus membuat, menerapkan dan
mantaati sistem penonaktifan energi secara total
(lock-out/tag-out) sebelum salah satu anggota
Kelompok Kontraktor melaksanakan pekerjaan
pada peralatan atau mesin.
7. UJI TEKAN
7.1 Kontraktor harus menjamin bahwa pegawai-
pegawainya atau Kelompok Kontraktor tidak
menguji pipa-pipa atau bejana-bejana tekan
dengan atau tanpa udara bertekanan, gas atau
cairan pemeliharaan (service fluid), tanpa
persetujuan tertulis terlebih dahulu dari

8. EXPLOSIVES AND HAZARDOUS MATERIALS
8.1 If explosives are required in the performance of the
Services, Contractor shall train its employees and
provide Company with written notification regarding
the proposed use, storage and handling of
explosives prior to the start of the blasting. Further,
Contractor shall ensure that its employees are
qualified to perform this type of work.
8.2 Contractor shall notify and receive Company’s prior
written approval before chemicals or hazardous
substances are brought to the Area of Operations.
8.3 Contractor shall ensure that all hazardous
substances used in the performance of the Services
are handled, stored, transported, identified,
documented and secured in a safe and
environmentally-sound manner.
8.4 Contractor shall maintain (and ensure that all
members of Contractor Group maintain) Material
Safety Data Sheets or their equivalent (“MSDSs”) in
the Area of Operations for all chemicals and other
hazardous substances used in performance of the
Services and perform all the Services consistent with
instructions contained in these MSDSs. MSDSs shall
be available in English and in the language of
Indonesia.
9. FIRE PROTECTION AND EMERGENCY
RESPONSE
9.1 Contractor shall take (and ensure that all members
of Contractor Group take) reasonable precautions to
prevent fires. Contractor shall ensure that
contaminated paper, rags, trash and other
combustible are disposed of in safe containers in
compliance with applicable laws and generally
accepted good international industry practice.
9.2 Contractor shall ensure that flammable liquids, such
as, gasoline, kerosene and fuel oil, are transported

Perusahaan.
8. BAHAN PELEDAK DAN BAHAN BERBAHAYA
8.1 Jika bahan peledak diperlukan dalam pelaksanaan
Jasa-jasa, Kontraktor harus melatih pegawai-
pegawainya dan memberikan pemberitahuan
tertulis kepada Perusahaan perihal rencana
penggunaan, penyimpanan dan penanganan

8.2 Kontraktor harus memberitahu dan mendapatkan
persetujuan tertulis dari Perusahaan sebelum
bahan-bahan kimia atau bahan berbahaya dibawa
masuk ke Daerah Operasi.
8.3 Kontraktor harus menjamin bahwa semua bahan
berbahaya yang digunakan dalam pelaksanaan
Jasa-jasa ditangani, disimpan, diangkut,
diidentifikasi, didokumentasi dan diamankan
dengan cara yang selamat dan ramah lingkungan.
8.4 Kontraktor harus menyimpan (dan menjamin
bahwa semua anggota Kelompok Kontraktor
menyimpan) Material Safety Data Sheet atau yang
sejenis (“MSDS”) di Daerah Operasi untuk semua
bahan kimia dan barang berbahaya lainnya yang
digunakan dalam pelaksanaan Jasa-jasa, dan
harus melaksanakan semua Jasa-jasa secara
konsisten dengan cara penggunaan spesifikasi dan
informasi dalam MSDS. MSDS harus tersedia
dalam bahasa Inggris dan dalam bahasa
Indonesia.
9. PENCEGAHAN KEBAKARAN DAN TANGGAP
DARURAT
9.1 Kontraktor harus mengambil (dan menjamin bahwa
semua anggota Kelompok Kontraktor mengambil)
tindakan pencegahan yang layak untuk mencegah
kebakaran. Kontraktor harus menjamin bahwa
kertas, kain lap, sampah dan bahan mudah
terbakar lainnya dibuang hanya ke dalam kontainer
yang aman sesuai ketentuan perundangan yang
berlaku dan sesuai cara kerja industri internasional
yang diakui secara umum.
9.2 Kontraktor harus menjamin bahwa semua cairan
mudah terbakar, seperti bensin, minyak tanah dan

that are designed for these purposes. Contractor
shall ensure that these liquids are stored away from
possible sources of ignition.
9.3 Contractor shall ensure that fire protection
equipment is not tampered with. Contractor shall
ensure that hydrants or main water valves are not
opened or closed without Company’s written
approval, except in the case of an emergency.
9.4 Contractor shall immediately report all leaks or other
indications of gas escaping around piping, vessels or
equipment to Company. Contractor shall cease (and
ensure that all members of Contractor Group cease)
all work in the area near the leak upon discovery of
the hazard.
9.5 Contractor shall ensure that its employees use only
non-toxic cleaning solvents with a high flash point
(above 140°F or 60°C) for cleaning purposes.
9.6 Contractor shall ensure that its employees receive
the fire protection, safety and other emergency
training required under all applicable laws and these
Guidelines.
9.7 Contractor shall provide its own fire protection
equipment for the use of its employees and
personnel of Contractor Group unless otherwise
agreed by Company in writing.
9.8 Contractor shall shut down (and ensure that all
members of Contractor Group shut down) internal
combustion engines before refueling, except where
the refueling point is sufficiently remote from the
engine to allow safe refueling.
9.9 In case of fire or other emergency situation,
Contractor shall immediately take (and ensure that
all members of Contractor Group take) appropriate

9.4 Kontraktor harus segera melaporkan segala
kebocoran atau tanda-tanda keluarnya gas di
sekitar pipa-pipa, bejana-bejana atau peralatan,
kepada Perusahaan. Kontraktor harus
menghentikan (dan menjamin bahwa semua
anggota Kelompok Kontraktor menghentikan)
semua pekerjaan di daerah yang dekat dengan
kebocoran pada saat ditemui bahaya tersebut.
9.5 Kontraktor harus menjamin bahwa pegawai-
pegawainya hanya menggunakan larutan
pembersih tidak beracun dengan titik nyala yang
tinggi (di atas 140°F atau 60°C) untuk keperluan
pembersihan.
9.6 Kontraktor harus menjamin bahwa pegawai-
pegawainya mendapatkan pelatihan perlindungan
kebakaran, keselamatan dan pelatihan
pengamanan darurat lainnya sebagaimana
diperlukan oleh ketentuan perundangan yang
berlaku dan Pedoman ini.
9.7 Kontraktor harus menyediakan sendiri peralatan
perlindungan kebakaran untuk digunakan oleh
pegawai-pegawainya atau personel Kelompok
Kontraktor kecuali disetujui lain secara tertulis oleh
Perusahaan.
9.8 Kontraktor harus mematikan (dan menjamin bahwa
semua anggota Kelompok Kontraktor mematikan)
mesin-mesin pembakar internal sebelum pengisian
bahan bakar, kecuali apabila titik pengisian terletak
cukup jauh dari mesin untuk memungkinkan
pengisian secara aman.
9.9 Dalam hal terjadi kebakaran atau keadaan darurat
lainnya, Kontraktor harus segera mengambil (dan
menjamin bahwa semua anggota Kelompok

 measures to protect the safety of personnel and to
extinguish the fire or otherwise handle the
emergency situation even where the cause is
unrelated to the Services. The first priority must be
the safety of all personnel. Contractor shall notify
Company Representative about the fire or other
emergency situation immediately, but not later than
the period of time after discovering the emergency
incident permitted under the Plan.
9.10 Contractor shall shut down (and ensure that all
members of Contractor Group shut down) and
remove all equipment from the area in and around
the fire and other emergency situation to the extent
possible.
9.11 Use of motors, compressors, pumps and other
equipment inside tank dike areas by Contractor is
permitted only in accordance with Company
procedures.
9.12 Contractor shall ensure that “strike anywhere”
matches and plastic disposable lighters are not
carried or used in hazardous areas. Safety matches
are preferred, but facility or work site rules apply and
control in case of conflict.
10. MEDICAL AID
10.1 Contractor shall provide first aid personnel,
equipment and supplies for its employees and
personnel of Contractor Group unless otherwise
agreed by Company in writing.
10.2 “First aid personnel” as the term is used in
Section 10.1 means a person who is a member of
Contractor Group and is working in the Area of
Operations. In addition to this person’s work
assignment, this person is also capable of providing
first aid. Contractor shall provide a minimum of one
first aid personnel for every twenty employees who
are working in the Area of Operations.

dan memindahkan semua peralatan dari daerah
sekitar kebakaran atau keadaan darurat lainnya,
sejauh mana memungkinkan.
9.11 Penggunaan motor-motor, kompresor-kompresor,
pompa-pompa dan peralatan lainnya di dalam
daerah tanggul tangki oleh Kontraktor hanya
diperbolehkan apabila sesuai dengan prosedur
Perusahaan.
9.12 Kontraktor harus menjamin bahwa pemantik api
yang dapat digunakan dimana saja dan pemantik
api plastik habis-buang, tidak dibawa atau
digunakan di daerah berbahaya. Pemantik api
keselamatan sebaiknya digunakan, tetapi
ketentuan fasilitas atau tempat kerja diberlakukan
apabila terjadi pertentangan dengan ketentuan
Pasal ini.
10. BANTUAN MEDIK
10.1 Kontraktor, untuk keperluan pertolongan pertama
pada kecelakaan (“P3K”), harus menyediakan
personel, peralatan dan bahan-bahan sendiri untuk
menangani pegawai-pegawainya dan personel
Kelompok Kontraktor kecuali disetujui lain secara
tertulis oleh Perusahaan.
10.2 “Personel P3K” sebagaimana istilah tersebut
digunakan dalam Pasal 10.1 berarti seseorang
yang merupakan anggota dari Kelompok
Kontraktor dan sedang bekerja di Daerah Operasi.
Sebagai tambahan terhadap penugasan kerja
orang tersebut, orang tersebut harus juga mampu
memberikan P3K. Kontraktor harus menyediakan
minimum satu orang personel P3K untuk setiap
dua puluh orang pegawai yang sedang bekerja di
Daerah Operasi.

11. PERSONAL PROTECTIVE EQUIPMENT
11.1 Personal Protective Equipment (“PPE”) must be
provided by Contractor for its employees and
personnel of Contractor Group if required by
applicable laws, decrees, codes, standards,
administrative rules and regulations, relevant
Company policy or generally accepted good
international industry practice for the industry in
which the Services are to be provided. Contractor
shall provide this equipment at its own cost unless
otherwise agreed by Company in writing.
Contractor’s employees must wear (and ensure that
all members of Contractor Group wear) this
equipment within the Area of Operations, as
required.
11.2 All PPE must be used and maintained by Contractor
in compliance with applicable laws and
manufacturer’s instructions.
12. HOUSEKEEPING
12.1 Contractor shall maintain good housekeeping at all
times and keep all work sites clean and free from
obstructions. Contractor shall mark and identify all
tripping hazards.
12.2 Contractor shall keep (and ensure that members of
Contractor Group keep) the access to emergency
exits clear at all times.
12.3 Contractor shall ensure that all ditches, holes,
excavations, overhead work and other impediments
connected with the Services are properly barricaded,
and are provided warning signs or lights where
necessary.
13. INCIDENT AND SAFETY REPORTING
13.1 Contractor shall report all on-the-job accidents or
injuries arising from the Services to the proper
governmental authorities, where required, and to
Company Representative. Contractor shall also
report all theft or other incidents of a criminal or
security nature, such as, a hijacking or assault.

11.1 Peralatan Perlindungan Pribadi (“PPE”) harus
disediakan oleh Kontraktor untuk pegawai-
pegawainya dan personel Kelompok Kontraktor
bilamana diharuskan oleh ketentuan undang-
undang, keputusan-keputusan, pedoman-
pedoman, standar-standar, keputusan dan
peraturan administrasi, kebijakan Perusahaan
terkait yang berlaku atau sesuai praktek industri
internasional yang baik yang diterima secara
umum dalam lingkup Jasa-jasa diberikan.
Kontraktor harus menyediakan peralatan ini atas
biaya dan tanggung-jawabnya sendiri, kecuali
disetujui lain secara tertulis oleh Perusahaan.
Pegawai-pegawai Kontraktor harus mengenakan
(dan menjamin bahwa semua anggota Kelompok
Kontraktor mengenakan) peralatan ini selama
berada di Daerah Operasi, sebagaimana
diharuskan.
11.2 Semua PPE harus digunakan dan dipelihara oleh
Kontraktor sesuai dengan ketentuan perundangan
yang berlaku dan keterangan dari pabrik.
12. PENATAAN KERAPIHAN
12.1 Kontraktor harus menata kerapihan secara baik
setiap saat dan harus tetap memelihara agar
semua tempat kerja bersih dan bebas dari
rintangan. Kontraktor harus menandai dan
mengenali semua bahaya sandungan.
12.2 Kontraktor harus menjaga (dan menjamin bahwa
semua anggota Kelompok Kontraktor menjaga)
akses pintu darurat bebas setiap saat.
12.3 Kontraktor harus menjamin bahwa semua parit,
lubang, galian, pekerjaan kawat listrik dan
rintangan lain sehubungan dengan Jasa-jasa
secara tepat diberi pembatas, dan bila diperlukan,
diberi tanda atau lampu peringatan.
13. PELAPORAN KEJADIAN DAN KESELAMATAN
13.1 Kontraktor harus melaporkan semua kecelakaan
kerja atau luka badan sehubungan dengan
pelaksanaan Jasa-jasa kepada instansi pemerintah
berwenang, jika diperlukan, dan kepada
Perwakilan Perusahaan. Kontraktor juga harus
melaporkan semua pencurian atau tindak pidana
atau keamanan lainnya, seperti pembajakan atau

involving personnel injury or Property damage that is
filed with Contractor’s (or any member of Contractor
Group’s) insurance company or representative or
that are reportable under the Occupational Safety
and Health Administration’s (“OSHA”) recordkeeping
regulations (or their equivalent in the Area of
Operations).
13.2 Contractor shall maintain and file (and ensure that all
members of Contractor Group maintain and file)
accident and incident reports required under this
Contract or as required by applicable laws, decree,
codes, administrative rules, these Guidelines and
regulations, and furnish copies to Company.
13.3 Contractor shall immediately and verbally report all
accidents and incidents to Company and confirm the
report in writing within the time limit specified by
Company in the Plan. This includes fatalities,
injuries, fires, spills, motor vehicle accidents,
damage to Company Property and other reports
required in this Contract.
13.4 Contractor shall report all accidents and incidents
arising from the Services that affect health, the
environment and safety (including spills). These
reports must be delivered to Company
Representative on a monthly basis unless otherwise
specified by Company in the Plan.
13.5 If required by Company Representative, Contractor
shall also prepare monthly reports regarding total
hours worked, number and type of incidents that
occurred during the report period or other statistic
required by Company. Contractor shall deliver these
reports to Company Representative by a specified
date each month.

Kontraktor memelihara dan menyimpan) laporan-
laporan kecelakaan dan insiden di bawah Kontrak
ini, atau sebagaimana diharuskan oleh undang-
undang, keputusan-keputusan, pedoman-
pedoman, peraturan administrasi yang berlaku,
Pedoman dan peraturan-peraturan ini, dan
menyediakan salinannya kepada Perusahaan.
13.3 Kontraktor harus segera dan secara lisan
melaporkan semua kecelakaan dan insiden kepada
Perusahaan dan mengkonfirmasi laporan lisan
tersebut secara tertulis dalam jangka waktu yang
ditetapkan oleh Perusahaan dalam Rencana Kerja.
Hal mana termasuk kematian, luka badan,
kebakaran, tumpahan, kecelakaan kendaraan,
kerusakan Harta Benda Perusahaan dan laporan
lainnya sebagaimana diharuskan oleh Kontrak ini.
13.4 Kontraktor harus melaporkan semua kecelakaan
dan kejadian yang timbul dari Jasa-jasa yang
mempengaruhi kesehatan, lingkungan dan
keselamatan (termasuk tumpahan). Laporan-
laporan ini harus diserahkan kepada Perwakilan
Perusahaan secara bulanan, kecuali ditetapkan
lain oleh Perusahaan dalam Rencana Kerja.
13.5 Jika diminta oleh Perwakilan Perusahaan,
Kontraktor harus membuat laporan bulanan
tentang jumlah jam kerja, jumlah dan jenis insiden
yang terjadi selama masa laporan atau statistik
lainnya sebagaimana diminta oleh Perusahaan.
Kontraktor harus menyerahkan laporan-laporan ini
kepada Perwakilan Perusahaan pada tanggal
tertentu setiap bulannya.

14. BEHAVIOR-BASED SAFETY
14.1 Contractor shall develop, implement and comply with
(and ensure that all members of Contractor Group
implement) a behavior-based safety process (“BBS”)
to provide for observation and feedback on
employee behaviors and to track and analyze these
observations in an effort to identify and implement
actions for improvement. BBS is intended to prevent
injury and incident through reduction of at-risk
behaviors in the workplace.
15. ENVIRONMENTAL PROTECTION
15.1 Contractor shall prevent (and ensure that all
members of Contractor Group prevent) spills or other
releases of oil or chemical substances during the
performance of Services. Contractor shall develop,
implement and comply with a pollution prevention
plan. Company has the right, but not the obligation
to review this pollution prevention plan.
15.2 Contractor shall exercise (and ensure that all
members of Contractor Group exercise) the
necessary care to protect and preserve the
environment, including flora, fauna and other natural
resources or assets at any location where the
Services are performed. Contractor shall mitigate
(and ensure that all members of Contractor Group
mitigate) adverse impacts to the environment. This
includes proper disposal of all hazardous and non-
hazardous wastes such as oil, chemicals, sewage
and garbage. Contractor shall comply with any
environmental practices specified by Company and
included as a condition of this Contract. In the event
Contractor discovers or is notified of 1) any condition
or situation on, in or around the Area of Operations
which may constitute a release of Hazardous
Substances or a violation of any law, or 2) any
threatened or actual lien, action or notice that the
Area of Operation is not in compliance with any law,
the party discovering the condition shall immediately
notify the Company. Contractor shall then
immediately take reasonable measures to remediate
the conditions and notify any other appropriate
governmental authorities.

14. KESELAMATAN BERDASARKAN PERILAKU
14.1 Kontraktor harus membuat, melaksanakan dan
mentaati (dan menjamin bahwa semua anggota
Kelompok Kontraktor melaksanakan) sebuah
proses Keselamatan Berdasarkan Perilaku (“BBS”)
untuk memungkinkan pemantauan dan masukan
balik dari perilaku pegawai dan untuk mengikuti
serta untuk menganalisa pantauan-pantauan ini
dalam rangka untuk menemukan dan menerapkan
tindakan-tindakan untuk perbaikan. BBS
dimaksudkan untuk mencegah terjadinya luka
badan dan insiden melalui pengurangan perilaku
beresiko di tempat kerja.
15. LINDUNGAN LINGKUNGAN
15.1 Kontraktor harus mencegah (dan menjamin bahwa
semua anggota Kelompok Kontraktor mencegah)
tumpahan atau pembuangan minyak atau bahan
kimia lainnya selama pelaksanaan Jasa-jasa.
Kontraktor harus membuat, melaksanakan dan
mentaati suatu rencana pencegahan polusi.
Perusahaan berhak, tetapi tidak berkewajiban,
untuk memeriksa rencana pencegahan polusi ini.
15.2 Kontraktor harus melaksanakan (dan menjamin
bahwa semua anggota Kelompok Kontraktor
melaksanakan) pemeliharaan yang diperlukan
untuk melindungi dan melestarikan lingkungan,
termasuk flora, fauna dan sumber daya atau aset-
aset alam lainnya di tiap lokasi dimana Jasa-jasa
dilaksanakan. Kontraktor harus mengurangi (dan
menjamin bahwa semua anggota Kelompok
Kontraktor mengurangi) dampak buruk kepada
lingkungan. Hal mana termasuk pembuangan yang
memadai untuk limbah bahan berbahaya dan tidak
berbahaya, seperti limbah minyak, bahan kimia,
limbah rumah tangga dan sampah. Kontraktor
harus mematuhi segala ketentuan lindungan
lingkungan yang ditentukan oleh Perusahaan dan
dijadikan ketentuan dalam Kontrak ini. Dalam hal
Kontraktor menemukan atau diberitahu tentang
adanya 1) segala keadaan atau situasi, di atas, di
dalam atau di sekitar Daerah Operasi yang bisa
jadi merupakan pelepasan Bahan Berbahaya atau
pelanggaran ketentuan hukum yang berlaku, atau
2) segala ancaman atau telah dilakukan sita,
perintah atau pemberitahuan bahwa Daerah
Operasi tidak memenuhi ketentuan undang-undang
manapun, pihak yang menemukan keadaan
demikian harus segera memberitahu Perusahaan.

15.3 Contractor shall assess the environmental hazards
of materials and supplies used in conjunction with
the Services and substitute materials presenting less
risk whenever possible. Contractor shall not use the
following materials, as well as any other materials
specified by Company, in the Area of Operations
without Company’s written approval:
(A) Polychlorinated Biphenyls (PCBs).
(B) Asbestos.
(C) Chlorinated solvents and thinners.
(D) Halon and other chlorinated fluorocarbons.
15.4 Contractor shall use only properly grounded above-
ground steel tanks for fuel storage. Contractor shall
not use bladder, fiberglass, plastic and other types of
fuel storage tanks without Company’s written
approval. Contractor shall ensure that loading and
drainage connections to fuel storage tanks are either
plugged or locked in the closed position when not in
use, and equipped with self-closing (“dead-man’s
valve”) fuel dispensing nozzles.
15.5 Contractor shall ensure that all onshore, above-
ground fuel, oil and chemical storage tanks used in
connection with the Services have a secondary
containment mechanism with a minimum capacity
equal to 110% of the capacity of the largest single
tank. Secondary containment impounds may have a
drain connection for removal of storm water if the
drain discharge is normally plugged or equipped with
a valve that is generally locked closed.
15.6 Contractor shall ensure that its employees do not
hunt, disturb or capture native birds, fish or other

ini, demikian juga material-material lainnya yang
ditentukan Perusahaan, di Daerah Operasi, tanpa
persetujuan tertulis dari Perusahaan:
(A) Poliklorinat Bifenil (PCB).
(B) Asbestos.
(C) Pelarut dan pengencer berklorin.
(D) Halon dan Fluorokarbon berklorin lainnya.
15.4 Kontraktor hanya akan menggunakan tangki-tangki
baja di atas permukaan yang di-tanahkan
(grounded) secara layak. Kontraktor tidak akan
menggunakan penyimpan bahan bakar tipe
bladder, serat kaca, plastik atau tipe lainnya tanpa
persetujuan tertulis dari Perusahaan. Kontraktor
harus menjamin bahwa sambungan pengisian dan
pembuangan ke/dari tangki penampungan bahan
bakar ditutup atau dikunci dalam posisi tertutup jika
tidak sedang digunakan, dan dilengkapi dengan
nozzle pengalir bahan bakar dengan katup
berpenutup otomatis (dead-man’s valve).
15.5 Kontraktor harus menjamin bahwa semua tangki
penyimpan bahan bakar, minyak dan bahan kimia,
yang berada di atas permukaan di daratan, yang
digunakan sehubungan dengan pelaksanaan Jasa-
jasa, mempunyai mekanisme penyimpanan
sekunder dengan kapasitas setara dengan 110%
dari kapasitas tangki tunggal terbesar. Daerah
penampungan sekunder dapat mempunyai
sambungan pembuangan untuk pembuangan air
hujan jika saluran pembuangan biasanya tertutup
atau dilengkapi dengan katup yang biasanya
tertutup dan terkunci.
15.6 Kontraktor harus menjamin bahwa pegawai-
pegawainya tidak berburu, mengganggu atau

 animals. Contractor may allow its employees to fish
at certain times and in certain places in the Area of
Operations if permitted by local regulations in the
Area of Operations.
15.7 Contractor shall ensure that trees and vegetation are
not removed to an extent greater than is necessary
to perform the Services. Contractor shall ensure that
topsoil is stockpiled for subsequent use in site
restoration unless Company provides in writing for
an alternative course of action.
15.8 Contractor shall ensure that fossils and antiquities
found at work sites are protected from damage or
disturbance. Contractor shall report the location of
these fossils and antiquities to Company and
suspend work at that location pending further
instructions from Company. Contractor is not
entitled to compensation for the period of the
suspension but Company shall reimburse Contractor
for all of the following expenses of Contractor which
are actual, direct and non-recoverable:
(A) Expenses incurred by Contractor as a consequence
of the suspension which are reasonably necessitated
by the suspension.
(B) Expenses otherwise incurred by Contractor during
the period of the suspension which Contractor could
not reasonably have avoided in order to be able to
re-commence performance of the Services upon the
suspension being lifted.
16. SHORT SERVICE EMPLOYEE PROGRAM
16.1 On or before the Effective Date of the Contract,
Company will advise Contractor whether a Short
Service Employee (“SSE”) program is required. If
required, Contractor shall prepare and implement its
SSE program. This SSE program applies to those
employees engaged by Contractor or any member of
Contractor Group who have less than six months
experience in the same job type.

15.8 Kontraktor harus menjamin bahwa fosil-fosil dan
barang-barang antik yang ditemukan di tempat
kerja, dilindungi dari kerusakan atau gangguan.
Kontraktor harus melaporkan lokasi penemuan
fosil-fosil dan barang-barang antik ini ke
Perusahaan dan menghentikan pekerjaan
sementara di tempat penemuan tersebut sambil
menunggu perintah selanjutnya dari Perusahaan.
Kontraktor tidak berhak atas imbalan selama
penghentian sementara pekerjaan ini, namun
Perusahaan akan mengganti biaya-biaya
Kontraktor berikut ini, yang sifatnya aktual,
langsung dan tidak dapat dimintakan
pengembalian (non recoverable):
(A) Biaya-biaya yang dikeluarkan Kontraktor sebagai
akibat dari penghentian yang diperlukan secara
layak dalam penghentian.
(B) Biaya-biaya lain yang dikeluarkan Kontraktor
selama masa penghentian sementara, yang tidak
dapat dihindari secara layak oleh Kontraktor, dalam
rangka untuk dapat memulai kembali pelaksanaan
Jasa-jasa saat penghentian sementara dinyatakan
berakhir.
16. PROGRAM PEGAWAI JANGKA PENDEK
16.1 Pada atau sebelum Tanggal Berlaku dari Kontrak,
Perusahaan akan memberitahu Kontraktor apakah
suatu program Pegawai Jangka Pendek (“SSE”)
diperlukan. Jika diperlukan, Kontraktor harus
menyiapkan dan menerapkan program SSE.
Program SSE ini diterapkan kepada pegawai yang
dipekerjakan oleh Kontraktor atau anggota
Kelompok Kontraktor yang mempunyai
pengalaman kerja yang kurang dari enam bulan di

16.2 Minimum requirements of a SSE program include all
of the following:
(A) SSE personnel shall be visibly identified.
(B) The number of SSEs in any work crew shall be
limited to the extent possible.
(C) A list of any high-risk work activities or areas in
which SSEs are not allowed to work.
17. STOP WORK AUTHORITY AND SUSPENSION OF
WORK
17.1 Stop Work Authority. Contractor shall stop work
when an imminent hazard to persons, Property or
the environment is identified and immediately notify
Company Representative that work has stopped, the
reasons for stopping the work and an estimate of
when the work will resume. Contractor shall take all
appropriate measures to abate the imminent hazard
and limit the duration of the stop work and
coordinate efforts with Company Representative to
mitigate the effect of this stop work authority.
17.2 Suspension of Work. Company has the right to
immediately suspend the performance of the
Services if Company, in its sole judgment,
determines that any member of Contractor Group is
failing to comply with these Guidelines or with written
instructions provided to Contractor under these
Guidelines or with applicable safety laws and
regulations while in the Area of Operations.
Suspension under this Section 17.2 continues until
Company notifies Contractor that the suspension is
lifted. Contractor acknowledges that Company has
no obligation to lift the suspension until it is satisfied
that Contractor will thereafter comply with these
Guidelines. Contractor is not entitled to
compensation for the period of the suspension or to
reimbursement of any expenses incurred during the
suspension.

bahaya mendesak terhadap orang, Harta Benda
atau lingkungan, dan segera memberitahu
Perwakilan Perusahaan bahwa pekerjaan
dihentikan, alasan penghentian pekerjaan dan
perkiraan kapan pelaksanaan pekerjaan akan
dipulihkan. Kontraktor harus megambil segala
langkah yang layak untuk mengatasi bahaya
mendesak dan membatasi jangka waktu
penghentian pekerjaan dan mengkoordinasikan
dengan Perwakilan Perusahaan untuk mengurangi
dampak dari kewenangan penghentian pekerjaan
ini.
17.2 Penghentian Pekerjaan. Perusahaan berhak untuk
segera menghentikan sementara pelaksanaan
Jasa-jasa, jika Perusahaan atas pertimbangannya
sendiri, menentukan bahwa anggota manapun dari
Kelompok Kontraktor gagal untuk memenuhi
ketentuan Pedoman ini atau perintah tertulis yang
diberikan kepada Kontraktor sesuai Pedoman ini
atau ketentuan dan peraturan keselamatan yang
berlaku selama berada di Daerah Operasi.
Penghentian sementara di bawah Pasal 17.2 ini
tetap berlangsung sampai Perusahaan
memberitahu Kontraktor bahwa penghentian
sementara telah berakhir. Kontraktor mengakui
bahwa Perusahaan tidak berkewajiban untuk
mengakhiri penghentian sementara sampai
Perusahaan yakin bahwa Kontraktor akan
selanjutnya mematuhi Pedoman ini. Kontraktor
tidak berhak atas imbalan selama jangka waktu
penghentian sementara atau atas penggantian
biaya yang timbul selama penghentian sementara.

18. TRAINING
18.1 Contractor shall ensure its employees are trained
(and ensure that all members of Contractor Group
are trained) in compliance with appropriate health,
safety and environmental codes, standards, laws
and regulations of all governmental or regulatory
agencies having jurisdiction over the Services or the
Area of Operations. Contractor shall ensure that this
training also addresses potentially dangerous
conditions, safe work practices and procedures,
including safety training to ensure the proper use of
any PPE required to perform the Services. At
Company’s request, Contractor shall provide
Company with a certification that training
requirements are met and maintained.
18.2 Prior to commencement of the Services, Contractor
shall participate (and ensure that all members of
Contractor Group participate) in a health,
environmental and safety orientation with Company
Representative.
19. MISCELLANEOUS
19.1 Contractor shall secure (and ensure that all
members of Contractor Group secure) compressed
gas cylinders in place on a regular cart or chained to
a support in an upright position. Contractor shall
ensure that these cylinders are protected when not
in use with protective valve caps. Furthermore,
Contractor shall ensure that compressed oxygen and
flammable gases are not stored together or near
combustible materials, but stored in accordance with
written instructions provided by Company or, if no
instructions are provided, in accordance with
generally accepted good international industry
practice.
19.2 Contractor shall ensure that no firearms, ammunition
or deadly weapons are stored, brought upon or used
in the Area of Operations except as may be
authorized by Company (e.g., for security purposes
to ensure protection of Contractor’s personnel and
Property).

18. PELATIHAN
18.1 Kontraktor harus menjamin bahwa pegawai-
pegawainya terlatih (dan menjamin bahwa semua
anggota Kelompok Kontraktor terlatih) untuk
mematuhi segala pedoman, standar, undang-
undang dan peraturan mengenai kesehatan,
keselamatan dan lindungan lingkungan yang layak
yang diberlakukan oleh semua instansi yang
mempunyai kewenangan terhadap pelaksanaan
Jasa-jasa atau atas Daerah Operasi. Kontraktor
harus menjamin bahwa pelatihan ini juga
mencakup keadaan-keadaan yang berpotensi
bahaya, kebiasaan dan prosedur kerja selamat,
termasuk pelatihan keselamatan untuk menjamin
pemakaian PPE secara benar yang dibutuhkan
untuk pelaksanaan Jasa-jasa. Atas permintaan
Perusahaan, Kontraktor harus membuktikan
kepada Perusahaan dengan sertfikasi bahwa
kebutuhan pelatihan telah dipenuhi dan
dipertahankan.
18.2 Sebelum dimulainya Jasa-jasa, Kontraktor akan
ikut serta (dan menjamin bahwa semua anggota
Kelompok Kontraktor ikut serta) dalam orientasi
kesehatan, lindungan lingkungan dan keselamatan
dengan Perwakilan Perusahaan.
19. LAIN-LAIN
19.1 Kontraktor akan mengamankan (dan menjamin
bahwa semua anggota Kelompok Kontraktor
mengamankan) bejana gas bertekanan di tempat,
di atas kereta biasa atau dirantai pada penopang
dalam posisi tegak. Kontraktor harus menjamin
bahwa bejana-bejana ini dilindungi dengan
pengaman katup pada saat tidak dipakai.
Selanjutnya Kontraktor harus menjamin oksigen
bertekanan dan gas-gas mudah terbakar, tidak
disimpan di tempat yang sama atau dekat material-
material yang mudah meledak, tetapi disimpan
sesuai perintah tertulis dari Perusahaan atau, jika
tidak ada perintah Perusahaan, maka disimpan
sesuai kebiasaan industri internasional yang baik.
19.2 Kontraktor harus menjamin bahwa tidak ada
senjata api, amunisi atau senjata mematikan yang
disimpan, dibawa atau digunakan di Daerah
Operasi kecuali sebagaimana diperbolehkan oleh
Perusahaan (seperti untuk keperluan pengamanan
untuk menjamin perlindungan personel atau Harta

19.3 Contractor shall ensure that all of its safety
equipment are properly maintained and in operable
condition in accordance with manufacturer’s
instructions, Company requirements and applicable
laws.
19.4 Contractor shall review the final or completed work
site and undertake any remedial measures required
to remove hazards and restore the work site in
accordance with Company requirements.
20. OTHER SPECIFIC PROVISIONS
20.1 In addition to Section 11.1, if the following PPE are
required to be provided by Contractor, such PPE
shall meet the following standards:
(A) Hard hat shall comply with ANSI Z89.1, “American
National Standard for Head Protection”.
(B) Safety eyeglasses shall meet the requirement of
ANSI Z87.1 ”Practice for Occupational and
Educational Eye and Face Protection”.
(C) Safety shoes shall meet the requirement and
specification of ANSI Z41 “Protective Footwear”.
20.2 Contractor shall use (and ensure that all members of
Contractor Group use) the following additional PPE
when they are working in specific areas:
(A) Fire retardant clothes (NFPA 1975) in the flame or
flash fire hazard areas (only for Kalimantan
Operations).
(B) Flash suit for working under electrical hazard.
(C) Ear protection (The National Institute for
Occupational Safety and Health [“NIOSH”])
approved) for areas > 85 dBA TWA.
(D) Personal fall arrest equipment (ANSI Z359.1-1999)
for working at height over 1.8 meters.

19.3 Kontraktor harus menjamin bahwa semua
peralatan keselamatannya dipelihara secara layak
dan berada dalam kondisi layak operasi sesuai
instruksi pabrikan, ketentuan Perusahaan dan
perundang-undangan yang berlaku.
19.4 Kontraktor harus memeriksa tempat kerja terakhir
atau yang telah diselesaikan, dan mengambil
langkah-langkah perbaikan yang diperlukan untuk
menyingkirkan bahaya-bahaya dan memulihkan
tempat kerja sesuai ketentuan Perusahaan.
20. KETENTUAN-KETENTUAN KHUSUS LAINNYA
20.1 Sebagai tambahan terhadap Pasal 11.1, jika PPE
berikut ini harus disediakan oleh Kontraktor, PPE
tersebut harus memenuhi standar sebagai berikut:
(A) Topi keras harus memenuhi ANSI Z89.1, “Standar
Nasional Amerika untuk Perlindungan Kepala”.
(B) Kacamata keselamatan harus memenuhi
persyaratan ANSI Z87.1 “Kebiasaan untuk
Perlindungan Mata dan Muka untuk Pekerjaan dan
Pendidikan”.
(C) Sepatu keselamatan harus memenuhi persyaratan
dan spesifikasi ANSI Z41 “Sepatu Keselamatan”.
20.2 Kontraktor harus menggunakan (dan menjamin
bahwa seluruh anggota Kelompok Kontraktor
menggunakan) PPE tambahan berikut ini ketika
mereka sedang bekerja di daerah khusus:
(A) Baju tahan api (NFPA 1975) di daerah bahaya
kobaran atau semburan api (hanya untuk Daerah
Operasi Kalimantan).
(B) Pakaian anti percikan listrik untuk pekerjaan di
daerah bahaya listrik.
(C) Perlindungan telinga (yang disetujui oleh Institut
Nasional untuk Keselamatan dan Kesehatan Kerja
[“NIOSH”]) untuk wilayah dengan tingkat
kebisingan > 85 dBA TWA.
(D) Peralatan pelindung jatuh pribadi (ANSI Z359.1-
1999) untuk bekerja di ketinggian lebih dari 1,8
meter.
