 Virus hepatitis C (HCV) dikenal sebagai penyebab utama penyakit hati kronis dan
dilaporkan menginfeksi 170 juta orang di seluruh dunia. Infeksi HCV yang persisten
menyebabkan hepatitis kronis, sirosis hati, dan hepatoseluler karsinoma. Standar terapi
perawatan untuk infeksi HCV adalah interferon pegilasi dengan ribavirin. Bersama
dengan manfaat terapeutik, persiapan interferon alfa dikaitkan dengan sifat beragam efek
samping. Sebagian besar efek samping interferon adalah terjadi pengurangan 50%
aktivitas biologis. Penyebab mendasar dari setiap efek buruk interferon alfa adalah waktu
paruh serum pendek, sifat pleiotropik, kehadiran reseptornya pada banyak sel dan
kapasitas untuk melepaskan banyak sitokin.
Dalam penelitian ini difokuskan untuk meningkatkan terapi interferon dengan
mengatasi keterbatasan efek samping. Hipotesisnya adalah bahwa interferon alfa 2b (IFN
α 2b) diproduksi oleh perpaduan domain Latency related protein (LAP) dari TGF β dan
IFN α 2b memiliki situs pembelahan HCV NS3 protease sebagai linker yang akan
diaktifkan hanya di situs target (hati) oleh virus protease (HCV NS3 protease) hadir di
permukaan sel yang terinfeksi

Metode dan Hipotesis
A. Strategi
Strategi yang dapat digunakan untuk mengatasi efek samping dari obat terapeutik atau
protein yaitu dengan menargetkan obat tersebut pada lokasi terjadinya penyakit atau
digunakan sebagai pro-drug. Pendekatan dengan penargetan memberikan kelebihan dalam
penggunaan terapi lokal biomolekul, mempertahankan efikasi dan menghindari terjadinya
ikatan non-spesifik pada sel yang bukan target terapi. Pendekatan yang digunakan untuk
pengiriman interferon ke hati yang ditargetkan meliputi: penggabungan interferon alfa-2
dengan antibodi spesifik terhadap reseptor asialoglycoprotein dan galactosyl spesifik
interferon yang dimodifikasi terhadap pengikatan dengan reseptor asialoglycoprotein spesifik
hepatosit.
B. Pendekatan
Pendekatan kedua untuk mengatasi efek samping dari pengiriman sitokin sistemik yaitu
dengan merekayasa obat tersebut dalam bentuk laten yang akan aktif hanya pada lokasi
targetnya saja dengan adanya pembelahan enzimatik. Protein terapeutik dibuat dalam
bentuk laten dengan cara: penggabungan terminal-N ke terminal-C melalui suatu
penghubung, penggabungan inhibitor peptida dan penggabungan protein yang
menghubungkan TGFβ. Penghubung yang dilaporkan dapat digunakan yaitu Matrix
Metalloprotease (MMP), Human Immunodefciency Virus (HIV) protease dan situs
pembelahan protease HCV NS3. Pada penghilangan sel yang terinfeksi HCV, HCV NS3
protease mengaktifkan toksin rekombinan “zymoxin” yang akan dikirimkan ke sel yang
terinfeksi tersebut dalam bentuk laten dengan pengabungan antitoksin.
C. Metode
Metode yang digunakan untuk meningkatkan farmakokinetika dan untuk
mengatasi efek pleiotropik sistemik pada pengiriman inteferon yaitu dengan
merekayasa interferon alfa-2b dalam bentuk laten, dimana terdapat lapisan
pelindung latensi yang terhubung dengan protein TGFβ yang telah menyatu
dengan N atau C-terminal melalui situs pembelahan HCV NS3 sebagai
penghubung.

D. Hipotesis
Hipotesisnya yaitu interferon alfa-2b laten akan diaktifkan oleh HCV NS3
protease yang berada di permukaan hepatosit yang telah terinfeksi HCV.
