TESTIS (androgen & steroidanabolik, antiandrogen,

& kontrasepsi pria)

Testis, seperti ovarium, memiliki fungsi gametogenik dan endokrin. Dimulainya
fungsi gametogenik testis terutama dikontrol olehsekresi FSH oleh hipofisis.
Untuk kesinambungan produksi spermadi tubulus seminiferosa, juga diperlukan
kadar testosteron local yang tinggi. Sel-sel Sertoli di tubulus seminiferosa
merupakan sumber estradiol yang dihasilkan oleh testis melalui aromatisasi
testosteron yang diproduksi secara lokal.Pada stimulasi LH,terbentuk testosteron
oleh sel interstisialis atau Leydig yang terdapat di ruang antara tubulus-tubulus
seminiferosa. Sel Sertoli di testis membentuk dan menyekresikan beragam
protein aktif, termasuk faktor inhibisi duktus miilleri (mullerian duct inhibitory
factor), inhibin, dan aktivin.
ANDROGEN &
STEROID ANABOLIK
Pada manusia, androgen
terpenting yang disekresikan
oleh testis adalah testosteron.

Jalur sintesis testosteron di

testis serupa dengan yang
sebelumnya diuraikan untuk
kelenjar adrenal dan ovarium.
 Pada pria, sekitar 8 mg testosteron diproduksi setiap hari. Sekitar 95% dihasilkan
oleh sel Leydig dan hanya 5% oleh adrenal.

 Testis juga mengeluarkan sejumlah kecil androgen poten lain, dihidrotestosteron,

serta androstenedion dan dehidroepiandrosteron, yaitu golongan androgen lemah.

 Pregnenolon dan progesteron sertaturunan 17-hidroksilasinya juga dikeluarkan

dalam jumlah kecil.

 Kadar plasma testosteron pada pria adalah sekitar 0,6 mcg/dL setelah pubertas dan
tampaknya menurun setelah usia 50 tahun.

 Sekitar 65% dari testosteron dalam darah terikat ke sexhormone-binding

globulin.SHBG ini meningkat konsentrasinya dalam plasma oleh estrogen, oleh
hormon timid, dan pada pasien dengan sirosis hati.

 Kadarnya berkurang oleh androgen dan hormone pertumbuhan serta lebih rendah
pada orang dengan obesitas.

 Sebagian besar dari testosteron sisanya terikat ke albumin. Sekitar 2% tetap bebas
dan dapat masuk ke dalam sel serta berikatan dengan reseptor intrasel.
 METABOLISME
Di banyak jaringan sasaran,
testosteron diubah menjadi
dihidrotestosteron oleh 5á-
reduktase. Di jaringan-jaringan ini,
dihidrotestosteron adalah androgen
aktif utama. Perubahan testosteron
menjadi estradiol oleh P450
aromatase juga terjadi di sebagian
jaringan,termasuk jaringan lemak,
hati, dan hipotalamus, serta hal ini
mungkin penting dalam pengaturan
fungsi gonad.Jalur utama untuk
penguraian testosteron pada
manusia terjadi di hati.
 
EFEK FISIOLOGIK
 Pada pria normal, testosteron atau metabolit aktifnya 5á-dihidrotestosteron berperan
dalam banyak perubahan yang terjadi saat pubertas.

 Selain sifat androgen yang mendorong pertumbuhan secara umum di jaringan-jaringan

tubuh, hormon golongan ini juga berperan dalam pertumbuhan penis dan skrotum.

 Perubahan pada kulit mencakup tumbuhnya rambut pubis, ketiak, dan janggut.

 Kelenjar sebasea menjadi lebih aktif, dan kulit cenderung menjadi lebih tebal dan
berminyak.

 Laring tumbuh dan pita suara menjadi lebih tebal, menghasilkan suara bernada rendah.

 Pertumbuhan tulang terangsang dan penutupan epifisis mengalami percepatan.

 Efek lain mencakup pertumbuhan prostat dan vesikula seminalis, menggelapnya kulit, dan
meningkatnya sirkulasi kulit.

 Androgen berperan penting dalam merangsang dan mempertahankan fungsi seksual pada
pria.
