Ibu mengatakan bahwa ini adalah kelahiran anak kedua dengan jenis kelamin laki-laki.
Lahir dengan berat bada 1950kg. Bayi lahir pada tanggal 12 Desember 2021 pukul
04.38 di RSUD Pademangan, Lahir secara spontan, dengan G2 P1 A0 pada usia
kehamilan 34 minggu. Lalu dilakukan rujukan post rawat di perina selama 2 har di
RSUD Koja pada tanggal 12 Desember 2021 sampai 14 Desember 2021 karena sesak.
Setelah dilakukan pemeriksaan didapatkan hasil kesdaran umum klien stabil, akral
hangat, bayi tampak kemerahan.

R : bayi dpat tidur lama dalam hubungannya
dengan fototerafi, meningkatkan resiko
dehidrasi bila jadwal pemberian makan yang
setring tidak dipertahankan
3. Perhatikan warna dan frekuensi defkasi dan urine
R : defeksi encer, sering dan kehijauan serta urine
kehijauan menandakan keefektifan fototerapi
dengan pencahayaan dan ekresi bilirubin.

4. Tinkatkan pemasukan cairan peroral
R : meeningkatkan input cairan sebagai kompensasi

indikator adanya kekurangan vol cairan dalam
tubuh bayi
6. Berikan cairan per parenteral sesuai indikasi
R : mungkin perlu untuk memperbaiki atau

R : peningkatan suhu tubuh dapat terjadi karena

sehingga akan mempengaruhi nadi dan respirasi
3. Monitor intake dan output
R : intake yang cukup dan output yang seimbang
dengan intake cairan dapat membeantu
mempertahankan suhu tubuh dalam batas normal
4. Pertahankan suhu tubuh, jika demam lalukan kompres
axila
R : suhu dalam batas normal dapat mencegah
terjadinya cold/ heat stress
5. Cek tanda-tanda vital setial 2-4 jam sesuai yang
dibutuhkan
R : untuk mengetahui keadaan umum bayi sehingga
memungkinkan pengambilan tindakan yang cepat
ketika terjadi sesuatu keabnormalan dalam tanda-
tanda vital
6. Kolaborasi pemberian antiseptik jika demam
R : antipiretik cepat membantu menurunkan demam
bayi
