DEFINISI
SOP Melepas selang sonde yang telah terpasang melalui lubang hidung

TUJUAN
Sebagai acuan penerapan langkah-langkah untuk melakukan tindakan
melepas naso gastric tube (NGT)

PROSEDUR PELAKSANAAN

Persiapan Alat
1. Perlak dan pengalas
2. Plastik sampah infeksius
3. Tissue
4. Spuit 10 cc

Persiapan Pasien
1. Jelaskan prosedur akan tindakan yang akan dilakukan
2. Atur posisi pasien senyaman mungkin

Persiapan Petugas
Sarung tangan bersih
Pelaksanaan Tindakan
1. Perawat memperkenalkan diri kepada pasien dan keluarga serta
menjelaskan mengenai prosedur yang akan dilakukan
2. Perawat meminta persetujuan tindakan secara lisan kepada
pasien/keluarganya.
3. Perawat menjaga privacy pasien dengan cara memasang tirai
4. Perawat melakukan identifikasi pasien sesuai dengan prosedur
5. Perawat melakukan kebersihan tangan sesuai dengan prosedur
6. Perawat mengenakan APD sesuai dengan prosedur
7. Perawat melakukan cek residu dengan spuit 10 cc untuk memastikan
selang sonde sudah kosong dari sisa makanan
8. Perawat melepas plester pada hidung
9. Perawat menarik secara perlahan NGT keluar sambil melihat respon
pasien
10. Perawat membuang selang pada sampah infeksius
11. Perawat membersihkan sekitar lubang hidung dengan tissue
12. Perawat merapikan alat yang telah dipakai dan membuang sampah
sesuai dengan prosedur
13. Perawat menjelaskan kepada pasien/keluarga bahwa tindakan selesai
dilakukan dan mohon undur diri
14. Perawat melepas APD sesuai dengan prosedur
15. Perawat melakukan kebersihan tangan sesuai prosedur
16. Perawat melakukan dokumentasi pelaksanaan tindakan di dalam
catatan perkembangan terintegrasi

AIM
As a reference for the implementation of measures to undertake the act of

PROCEDURE OF IMPLEMENTATION
Tools preparation
1. Perlak and pemalas
2. Plastic waste infectious
3. Tissue
4. Spuit 10 cc

Patient Preparation
1. Explain the procedure for the action to be taken
2. Set the patient's position as comfortable as possible

Officer Preparation

Implementation of Action
1. Nurses introduce themselves to patients and families and explain the
procedures to be performed
2. The nurse requests verbal approval of the action to the patient /
family.
3. Nurses maintain the privacy of patients by installing a curtain
4. The nurse identifies the patient according to the procedure
5. Nurses perform hand hygiene according to the procedure
6. Nurses wear PPE according to procedure
7. The nurse performs a residue check with a 10 cc syringe to make sure
the sonde hose is already empty from the rest of the food
8. The nurse removes the plaster on the nose
9. The nurse pulls slowly out the NGT while looking at the patient's
response
10. Nurse throw the hose on the infectious litter
11. The nurse cleans around the nostrils with a tissue
12. Nurses tidy up used tools and dispose of waste in accordance with the
procedure
13. The nurse explains to the patient / family that the action is done and
please retreat
14. The nurse removes the PPE in accordance with the procedure
15. Nurses perform hand hygiene according to procedure
16. Nurses perform documentation of action implementation in
integrated development records
