Pretreatment
Treatment with an NSAID before a procedure has been shown to have a significant benefit in
many studies but not all of them.The rationale for pretreatment is to block the development of
hyperalgesia by reducing the input from peripheral nociceptors. For patients who cannot take
NSAIDs, pretreatment with acetaminophen also has been shown to reduce postoperative
pain.Patients can be pretreated 30 minutes before the procedure with either an NSAID (e.g.,
ibuprofen 400 mg or flurbiprofen 100 mg) or with acetaminophen 1000 mg.
Flexible Plan
A third pharmacologic approach is the use of a flexible plan for prescribing analgesics .* A
flexible prescription plan serves to minimize both postoperative pain and side effects. Given
these goals, the clinician’s strategy is twofold:
(1) to achieve a maximally effective dose of the nonnarcotic analgesic (either an NSAID or
acetaminophen for patients who cannot take NSAIDs), and
(2) in the rare cases in which the patient still has moderate to severe pain, to consider adding
drugs that increase the NSAID’s analgesia.
Because of its predictive value, the presence of preoperative pain or mechanical allodynia may
be an indicator that such NSAID combinations should be considered. Studies have shown that
treatment combining an NSAID with acetaminophen 1000 mg alone (i.e., no opioid) produces
nearly twice the analgesic response as just the NSAID. Administration of ibuprofen 600 mg with
acetaminophen 1000 mg produced significant relief of posttreatment endodontic pain, compared
to ibuprofen.

Pretreatment

bahwa pengobatan yang menggabungkan NSAID dengan asetaminofen 1000 mg saja (yaitu, tidak ada
opioid) menghasilkan respons analgesik hampir dua kali lipat hanya dari NSAID.
Pemberian ibuprofen 600 mg dengan acetaminophen 1000 mg menghasilkan pengurangan nyeri endodontik

STEPS OF RETREATMENT
- Coronal Disassembly
Endodontic retreatment procedures commonly require removal of the existing coronal restoration
But in some cases access can be made through the existing restoration It is advisable to remove
the existing restoration especially if it has poor marginal adaptation, secondary caries to avoid
procedural errors. To maintain form, function and esthetics, temporary crown can be placed.
In some teeth, post and core needs to be removed for gaining access to the root canal system
However, when crown is with good marginal integrity, access can be gained without crown
removal.
Patency of canal can be regained by removing obstructions in the canal which can be in the form
of silver points, gutta percha, pastes, sealers, separated instruments and posts, etc.
» Gutta-Percha Removal
» Removal of Resilon
Resilon can be removed using combination of hand and rotary
instruments, similarly as we do for removal of guttapercha. For effective
removal of resilon, combination of chloroform dissolution and rotary
instrumentation is recommended. For removal of resilon sealer, use of
GatesGlidden drills and Hfiles is recommended.
Grasp the carrier with pliers and try to remove it using fulcrum
mechanism rather than straight pulling if from the tooth.
Use ultrasonics along the side of the carrier and thermosoften the
guttapercha. Then move ultrasonics apically and displace the carrier
coronally.
Use solvents to chemically soften the guttapercha and then use hand
files to loosen the carrier.
Use rotary instruments to remove plastic carrier from the canal.
Use instrument removal system to remove carrier especially if it is
metal carrier.
» Removal of Paste
Soft setting pastes can be removed using the normal endo dontic
instruments preferably using crown down technique. Hard setting cements
like resin cements can be first softened using solvents like xylene,
eucalyptol, etc. and then removed using endodontic files. Ultrasonic
endodontic devices can also be used to breakdown the pastes by vibrations
and thus facilitate their removal
» Separated Instruments and Foreign Objects
Broken instruments or foreign objects can be retrieved from the
canals but primary requirement for their removal is their accessibility
and visibility.
If rootcanal is obstructed by foreign object in coronalthird then
attempt retrieval, in middlethird, attempt retrieval or bypass and if it
is in apical third leave it or treat surgically.
If overpreparation of canal compromises the dentin thickness, one
should leave the instrument in place rather than compromising the
coronal dentin.
If instrument is readily accessible, remove it by holding with
instruments like Stieglitz pliers and Massermann extractor.
Massermann extractor comprises a tube with a constriction into
which a stylet is introduced to grasp the fractured instrument
After gaining access to the root canal system, with its thorough cleaning and shaping and
managing other complications, the treatment is completed using the routine procedures.
But sometimes the retreatment may become difficult due to presence of therapy resistant
microorganisms like Enterococcus faecalis.

Perangkat endodontik ultrasonik juga dapat digunakan untuk memecah pasta dengan getaran
dan dengan demikian memudahkan pelepasannya
• Instrumen yang rusak atau benda asing dapat diambil dari kanal tetapi persyaratan utama
untuk memindahkannya adalah aksesibilitas dan visibilitasnya.
• Jika saluran akar terhalang oleh benda asing di sepertiga koronal maka coba ambil kembali,
di sepertiga tengah, coba ambil atau bypass dan jika di sepertiga apikal dapat ditinggalkan
atau rawat dengan pembedahan.
• Jika preparasi saluran akar yang berlebihan mengganggu ketebalan dentin, instrumen harus
dibiarkan di tempatnya daripada membahayakan dentin koronal.
• Jika instrumen mudah diakses, lepaskan dengan memegang instrumen seperti tang Stieglitz
dan ekstraktor Massermann. Ekstraktor Massermann terdiri dari tabung dengan penyempitan
di mana stilet dimasukkan untuk memegang instrumen yang retak
Setelah mendapatkan akses ke sistem saluran akar, dengan cleaning menyeluruh dan shaping
serta penanganan komplikasi lainnya, perawatan diselesaikan dengan menggunakan prosedur

Retreatment
With age, the incidence of endodontic retreatments may increase, due to coronal leakage
from loss of seal, recurrent caries or fracture of teeth, and restorations. 111 Also, over-
looked or missed canals are a more common cause of failure in older patients. An isolated
symptom, heat sensitivity is usually indicative of a missed canal.

Endodontic surgery is defined as “removal of tissues other than the contents of root canal to
retain a tooth with pulpal or periapical involvement”. Surgical intervention is required for cases
where retreatment has failed or is not an option, and the tooth is to be retained rather than
extracted.
It is the ablation of apical portion of the root-end attached soft tissues. The purpose of root-end
resection is to remove anatomic variations, correct operator errors, evaluate and create an apical
seal, and reduce fenestration of root apices.
Root-end resection involves beveling the apical portion of the root end. Root-end resection
removes the untreated apical portion of the root and enables the surgeon to determine the cause
of failure. It also provides a flat surface that allows the operator to create a root-end cavity
preparation and place a root-end filling material.
ROOT RESECTION/AMPUTATION
Root resection is the removal of one or more roots from a multirooted tooth (Figure 24-48). It is
usually performed in maxillary molars, but can also be performed for mandibular molars. The
indications for this procedure are presence of severe bone loss in a periodontally-involved root,
untreatable roots with broken instrument, perforations, caries, resorption, vertical root fracture,
and calcified canals in a multi-rooted tooth. The contraindications for root amputation are
absence of sufficient bony support for the remaining root or roots as well as fused roots.
BICUSPIDIZATION/BISECTION
It is defined as surgical separation of a multirooted tooth into two halves and their respective
roots. Each root is then restored with a separate crown. Basically bicuspidization is done to form
a more favorable position for the remaining segments which leaves them easier to clean.
Indications for bicuspidization of mandibular molars are the presence of irreparable furcation
perforation, furcation pathosis from a periodontal abscess, presence of buccolingual cervical
caries, or a fracture in the furcation. The contraindications for bicuspidization are the presence of
deep furcation, unrestorable crowns, absence of adequate bony support for roots as well as fused
roots and presence of fused roots.
HEMISECTION
Hemisection is the surgical division of a multi-rooted tooth into two segments. It is usually
performed in mandibular molars, but can also be performed for maxillary molars. The
indications and contraindications for this procedure are very similar to those for root resection. In
mandibular molars, the tooth is divided buccolingually through the bifurcation. In maxillary
molars, the tooth is sectioned mesiodistally through the furcation. After raising a sulcular flap
without a vertical-releasing incision, hemisection is usually carried out by making a vertical cut
through the crown into the furcation. This action results in complete separation of the tooth in
two segments. The unsalvageable root and its coronal segment are then removed
PERIRADICULAR CURETTAGE
It is a surgical procedure to remove diseased tissue from the alveolar bone in the apical or lateral
region surrounding a pulpless tooth.
The goal of periapical curettage is the removal of pathological soft tissue surrounding the apex.
This procedure allows optimal access and visibility to the surgical site. Removal of a periapical
lesion reduces bleeding and provides a biopsy specimen for histological examination. The soft
tissue should be carefully peeled away with a suitable curette. The cavity should be examined
under magnification to ensure a clean bony crypt and no tissue tags
ENDODONTIC IMPLANTS
Endodontic implants are used for providing stabilization of teeth in which alveolar support is
lost due to endodontic and periodontal disease. It enhances root anchorage by extension of
artificial material beyond alveolar socket but with in range of alveolar bone

HEMISECTION

INDICATIONS
Grossman et al. provided a list of indications for endodontic surgery including the
1. presence of large and intruding periapical lesions,
2. overfilled canals,
3. incomplete apical root formation, and destruction of the apical constriction by over-
instrumentation amongst others
However, many of the previously accepted indications are no longer valid in light of current
concepts of the biological basis for endodontic treatment
Given these success rates and improved technology, there are specific indications for
periradicular surgery today. These are
(1) failure of nonsurgical retreatment (treatment has been rendered at least two times),
(2) failure of nonsurgical (initial) treatment and retreatment is not possible or practical or would
not achieve a better result, or
(3) when a biopsy is necessary.
It is paramount that these indications must be in the best interests of the patient, within the skills
of the clinician, and reflective of biological principles of endodontic therapy.
CONTRAINDICATIONS
5. Proximity to nasal floor and maxillary sinus: A careful surgical procedure is required to
avoid surgical perforation of sinus.
6. Miscellaneous

Menurut Grosman dkk. memberikan daftar indikasi untuk operasi endodontik termasuk
3. pembentukan akar apikal yang tidak lengkap, dan penghancuran penyempitan apikal oleh
instrumentasi berlebihan antara lain
Namun, banyak dari indikasi yang diterima sebelumnya tidak lagi valid mengingat konsep
dasar biologis untuk perawatan endodontik saat ini
Mengingat tingkat keberhasilan ini dan teknologi yang ditingkatkan, ada indikasi khusus untuk
operasi periradikular saat ini. Ini adalah
(1) kegagalan perawatan ulang non-bedah (pengobatan telah dilakukan setidaknya dua kali),
(2) kegagalan pengobatan dan perawatan ulang (awal) non-bedah tidak mungkin atau praktis
atau tidak akan mencapai hasil yang lebih baik, atau
KONTRAINDIKASI
Misalnya : leukemia atau neutropenia, diabetes mellitus yang tidak terkontrol, hipertensi yang
tidak terkontrol, gangguan perdarahan yang tidak terkontrol, pasien immunocompromised,
infark miokard baru-baru ini
Misalnya : akar miring ke lingual, apeks akar lebih dekat dengan kanal mandibula, adanya
pelat bukal yang terlalu tebal
Misalnya : Dokter harus benar-benar jujur tentang keterampilan dan pengetahuan bedah
mereka. Di luar kemampuan mereka, kasus harus dirujuk ke endodontis atau ahli bedah
mulut.
4. Panjang akar yang pendek di mana pencabutan apeks akar lebih lanjut mengganggu
prognosis.
5. Dekat dengan dasar hidung dan sinus maksilaris: Prosedur pembedahan yang hati-hati
diperlukan untuk menghindari perforasi sinus secara bedah.
6. Lain-lain
misalnya : gigi yang tidak dapat direstorasi, prognosis periodontal yang buruk, gigi yang
retak secara vertikal, gigi yang tidak strategis.

• Inject local anesthetic with vasoconstrictor into soft tissue. This will help in controlling
hemorrhage during surgery.
• Design flap depending upon condition of the patient and preference of the clinician.
• Expose the surgical site.
• Use the bone curette to remove the pathologic tissue surrounding the root.
• Insert the curette between the soft tissue and bone, apply the pressure against the bone.
• After removing the tissue from the bony area, grasp the soft tissue with the help of tissue
forceps. • Send the pathological tissue for histopathological examination
ROOT RESECTION/AMPUTATION
• Before root resection, endodontic treatment is done on the roots to be retained, once the canals
to be retained are obturated, the permanent restoration is done. After this, root resection is carried
out. There are basically two approaches for root resections:
1. Vertical: Here complete root is ressected along with its associated portion of crown. This
procedure also called as hemisection or trisection. It is done from mesial to distal in maxillary
molars and bucccal to lingual in mandibular molars.
2. Horizontal/Oblique: In this, root is ressected at the point where it joins to the crown. It is also
called as root resection.

• Masukkan kuret di antara jaringan lunak dan tulang, berikan tekanan pada tulang.
• Setelah mengeluarkan jaringan dari daerah yang bertulang, pegang jaringan lunak dengan
bantuan tang jaringan (tissue forceps).
• Kirim jaringan patologis untuk pemeriksaan histopatologis
ROOT RESECTION/AMPUTATION
• Sebelum tindakan root resection, perawatan endodontik harus dilakukan pada akar yang akan
dipertahankan, setelah saluran akar yang akan dipertahankan diobturasi, dilakukan restorasi
permanen. Setelah ini, reseksi akar baru bisa dilakukan. Pada dasarnya ada dua pendekatan untuk
reseksi akar:
1. Vertikal: Di sini akar lengkap direseksi bersama dengan bagian mahkota yang terkait.
Prosedur ini disebut juga dengan hemiseksi atau triseksi. Dilakukan dari mesial ke distal pada
molar rahang atas dan bukal ke lingual pada molar rahang bawah.
2. Horizontal/Oblique: Dalam hal ini, akar direseksi pada titik di mana ia bergabung dengan
mahkota. Ini juga disebut sebagai reseksi akar.
ENDODONTIC IMPLANTS
Prognosis gigi yang telah mengalami amputasi akar, hemiseksi untuk bikuspidisasi bervariasi
sesuai dengan situasi. Jika prosedur ini dilakukan dengan benar dan gigi direstorasi dengan
benar, faktor utama yang mempengaruhi keberhasilan adalah kebersihan mulut pasien. Pasien
harus mau dan mampu melakukan prosedur ekstra untuk mencegah akumulasi plak. Kegagalan
untuk melakukannya akan mengakibatkan kegagalan prosedur.
PERIAPICAL CURRETAGE
Prognosis baik, Para pasien bebas dari rasa sakit dan gejala infeksi lainnya setelah satu
pengobatan. Pada pemeriksaan radiografi yang dilakukan dari 6 sampai 12 bulan dan 3 tahun
kemudian, ditemukan bahwa 50% dari akar kuret menunjukkan penyembuhan lengkap, 33%
tidak pasti dan 17% tidak memuaskan. Pembentukan tulang paling cepat selama tahun pertama
pasca operasi, setelah itu menurun. Pembentukan tulang juga paling cepat pada lesi yang lebih
besar tetapi menurun, bahkan pada lesi ini, ketika berkembang ke sekitar akar.
ENDODONTIC IMPLANT THERAPY
Dalam sebuah studi, Sebanyak 13.434 catatan pasien yang memiliki implan (33,6%) atau terapi
saluran akar (66,4%) dimasukkan. Analisis tingkat kelangsungan hidup dan tabel Kaplan-Meier
mengungkapkan sebagian besar implan dilepas dalam tahun pertama (58,8%), sementara hanya
35,2% perawatan saluran akar yang gagal dalam periode waktu yang sama. Tingkat
kelangsungan hidup secara keseluruhan secara signifikan (p<0,001) lebih tinggi untuk terapi
implan (98,3%) dibandingkan dengan perawatan saluran akar (72,7%). Sebuah hubungan yang
signifikan secara statistik ditemukan antara pengobatan (p<n0,001), usia (p<0,001) dan
kecemasan (p=0,004) dengan hasil pengobatan.
Terapi implan menunjukkan kegagalan yang jauh lebih rendah jika dibandingkan dengan
perawatan saluran akar, tetapi pemilihan salah satu perawatan harus didasarkan pada beberapa
faktor. Usia dan kecemasan yang lebih tinggi juga secara signifikan terkait dengan saluran akar
dan kegagalan perawatan implan.
APICOECTOMY
Menurut studi dari Shimon Firiedman ini, 37-91% gigi dapat diharapkan untuk sembuh,
sementara hingga 33% masih dapat disembuhkan beberapa tahun setelah operasi. Yang penting,
80-94% gigi dapat tetap berfungsi tanpa gejala, bahkan jika tidak dapat disembuhkan.

Prognosis of teeth that have had root amputation, hemi section for bicuspidization varies
according to the situation. If these procedures are performed correctly and the tooth is restored
properly, the major factor affecting success is the patient’s oral hygiene. The patient must be
willing and able to perform extra procedures to prevent plaque accumulation. Failure to do so
will result in the failure of the procedure.
PERIAPICAL CURRETAGE
The patients were free from pain and other symptoms of infection after one treatment. In the
radiographic examinations carried out from 6 to 12 months and 3 years later, it was found that
50% of curetted roots showed complete healing, 33% were uncertain and 17% were
unsatisfactory. Bone formation was most rapid during the first postoperative year, after which it
decreased. The bone formation was also most rapid in larger lesions but decreased, even in these,
when it advanced to the vicinity of the root.
ENDODONTIC THERAPY
A total of 13,434 records of patients who had implant (33.6%) or root canal therapy (66.4%)
were included. The survival rate analysis and Kaplan-Meier table revealed the majority of the
implants were removed within the first year (58.8%), while only 35.2% of the root canal
treatments failed in the same time period. The overall survival rate was significantly (p < 0.001)
higher for implant therapy (98.3%) compared to root canal treatment (72.7%). A statistically
significant association was found between treatment (p <n0.001), age (p < 0.001) and anxiety
(p = 0.004) with treatment outcome
Implant therapy exhibited significantly lower failures when compared to root canal treatment,
but the selection of either treatment should be based on multiple factors. Higher age and anxiety
were also significantly associated with root canal and implant treatment failure.
APICOECTOMY
According to these studies from Shimon Firiedman, 37–91% of teeth can be expected to be
healed, while up to 33% can still be healing several years after surgery. Importantly, 80–94% of
teeth can remain in symptom-free function, even if they are not healed.
