Epidemiologi
Amerika Serikat :
– Insidensi kecelakaan lalu lintas setiap 15 detik
– Korban fatal hampir tiap 12 menit
– Kematian karena cedera kepala ± 50%
– ≥ 60% fatality rate pada kecelakaan lalu lintas
berhubungan dengan cedera kepala
Bandung – 1986 rata-rata 1800 penderita per tahun
yang dirawat karena cedera kepala

Foto Polos Kepala
• Bila terdapat jejas / vulnus di kepala
• Posisi AP dan lateral
• Foto polos kepala tidak merubah
penatalaksanaan pasien. Bila pasien
mengalami kemunduran, membutuhkan
pemeriksaan brain imaging (The Skull X-Ray
Refferal Criteria Panel, 1987)
Foto Servikal
• Bila diduga ada cedera di daerah Servikal
• Atau terdapat tetraparese
CT scan kepala
Dilakukan bila :
• GCS ≤14
• GCS 15 dengan :
- Riwayat gangguan kesadaran
- Post traumatic amnesia
- Defisit neurologis fokal
- Tanda-tanda fraktur basis kranii

Diffuse injury II Tampak sisterna, midline shift < 5mm dengan/ tanpa
densitas lesi, tidak ada hiperdensitas atau densitas
campuran lesi > 25cc, dapat termasuk pecahan
tulang atau benda asing
Diffuse injury III Sisterna terdesak atau hilang, midline shift 0-5mm,
(Swelling) tdk terdapat hiperdensitas atau lesi campuran >25
cc
Diffuse injury IV (shift) Midline shift >5mm, tidak ada hiperdensitas atau lesi
campuran >25mm
Evacuated Mass Setiap lesi surgical yang dapat dievakuasi
Tujuan utama :
- Menghilangkan
Kerusakan otak
- Mencegah karena proses
- Meminimalkan sekunder
PENATALAKSANAAN
Riwayat
• Nama, umur, sex, pekerjaan
• Mekanisme injury
• Waktu kejadian injury
• Kehilangan kesadaran segera setelah injury
• Berkurangnya kewaspadaan
• Amnesia : retrograde, anterograde
• Nyeri kepala : ringan, sedang, berat
• Bangkitan kejang
• Pemeriksaan umum untuk menyingkirkan
kemungkinan injury sistemik
• Pemeriksaan neurologik
• Foto Servikal atau lainnya sesuai indikasi
• Pemeriksaan alkohol darah atau zat
toksik dalam urine
• CT-Scan kepala kalau mungkin kecuali
bila tidak ada keluhan dan neurologis
normal
Dirawat untuk observasi
• CT-Scan tidak dilakukan/tidak ada/ abnormal
• Ada luka tembus kepala
• Amnesia
• Riwayat hilang kesadaran
• Kemunduran kesadaran
• Nyeri kepala sedang / berat
• Alkoholik / intoksikasi obat
• Fraktur tengkorak
• Kebocoran cairan serebrospinal, rinorrhea atau otorrhea
• Luka di tempat lain yang bermakna
• Tidak ada teman di rumah
• Tidak mungkin segera kembali ke RS
Penderita boleh pulang bila :
• Tidak ditemukan kriteria untuk dirawat
• Segera kembali ke RS bila problem
berkembang
• Jadwalkan untuk kontrol ke klinik dalam 1
minggu
Peringatan pada cedera kepala ringan
yang tidak dirawat
Segera kembali ke RS apabila terdapat gejala :
- Tampak mengantuk & sulit dibangunkan
- Mual dan muntah
- Timbul kejang
- Perdarahan / keluar cairan dari hidung / telinga
- Nyeri kepala berat
- Kelemahan atau hilang perasaan pada lengan /
tungkai
- Tampak bingung / tingkah laku aneh
- Pupil anisokor, pengelihatan ganda
- Denyut nadi terlampau lambat atau cepat, atau pola
nafas terganggu

Lesion is completely or incompletely irreversible
Symptoms of cord compression
Continuous pain
Worsening neurological deficits
Incomplete neurological deficits
New neurological deficits
Bilateral cervical dislocation arouse from unilateral one with
locked facet
Spinal Cord Injury (SCI)
Symptoms of SCI
Symptoms varies
Depend on completeness of the lesion
Complete lesion :
All sensory and motor function at lesion area loss
Autonomic reflex unihibitted (hypereflexia)
Clinical picture may varied according to location
Incomplete lesion– related to site of lesion
Spinal Cord Injury (SCI)
Symptoms of SCI
1. Spinal Shock
From a few minute to 6 – 12 weeks.
Loss of autonomic function
(esp. lesion above T4 emergency !!
(cardiovascular threatening)
3 – 4 minute after trauma BP to 200 – 300 mm Hg.
Sympathetic stimulation (preganglionic)
Neurogenic shock : TRIAD
Hypotension ( SBP 60 – 70 % of previous SBP )
Bradycardia
Hypothermia (below lesion)
a part of spinal shock : loss of sympathetic function
Symptoms of SCI
2. Respiratory problem
Often occurs on lesion of cervical or upper thoracal segment
Worse if positive abdominal distension
Spinal shock ends :
Recovery of motor and sensory function started cranially
Vasomotor tone : recover about 4 months in complete lesion.
The completeness of lesion at spinal cord is difficult to
assess clinically
Distribution :
all over the body
(cranial, spinal, autonomic nerves)
Main manifestation : lower motor neuron type
Classification according to Seddon :
Neuro praxia : merely functional disturbance
Axonotmesis axonal damage
Neurotmesis axon dan myelin sheath damage

Loss of sensibility extensor area at lower arm and radial
side dorsum of hand
Lower lesion
Dejerine – Klumpke type
Inferior trunk ( C8 – Th1 )
Muscles weakness and atrophy of hand and fingers
Loss of sensibility on medial side of upper arm, lower arm, and
ulnar side of hand
Ulnar reflex / -
Ipsilateral Horner’ syndrome also common
Brachial Plexopathy
Plexus lesion at the level of fascicle ( cord )
Similar to proximal lesion to Median, Ulnar, and Radial nerves
Cause :
- Neoplasma
- Surgery (around pelvic area)
- Trauma
- Diabetics
- Fracture
- Idiopathic
Lumbosacral Plexopathy
Lumbal plexus lesion
Inability in
Knee extension
Thigh adduction
Loss of sensibility from inguinall, genitalia, lateral
anterior and medial thigh, and medial leg
Quadriceps reflex (KPR) and cremasteric reflex / -
Lumbosacral Plexopathy
Weakness or paralysis in
Knee flexor
Foot dorsiflexion
Foot eversion
Loss of sensibility : end of the leg, foot and toes
Achilles reflex (APR ) / -
