Sehubungan dengan pengajuan permohonan aktivasi finansial No. HP e-Banking tersebut di atas,
Nasabah dengan ini menyatakan bahwa Nasabah bertanggung jawab sepenuhnya atas:
In connection with the request for the financial feature activation of the e-Banking Phone No. specified above, the
Customer hereby declares that the Customer is fully responsible for:
1. pengajuan permohonan aktivasi finansial No. HP e-Banking dengan nomor handphone
sebagaimana tersebut di atas;

2. kebenaran data yang diberikan oleh Nasabah, termasuk kebenaran nomor handphone yang diajukan
oleh Nasabah untuk dilakukan aktivasi finansial No. HP e-Banking;
the correctness of the data provided by the Customer, including the correctness of the mobile phone number
provided by the Customer for the financial feature activation of the e-Banking Phone No.;
3. segala akibat yang timbul sehubungan dengan pengajuan permohonan aktivasi finansial No. HP
e-Banking sebagaimana dimaksud dalam surat ini dan membebaskan BCA dari segala macam
tuntutan, gugatan, dan/atau tindakan hukum lainnya dalam bentuk apa pun dari pihak mana pun
termasuk dari Nasabah sehubungan dengan hal tersebut di atas.

instituted by any party including the Customer in connection therewith.
