Embriologi Sistem Urinarius 2 macam perkembangan sistem urinarius yaitu organogenesis ginjal dan maturasi ginjal. Organogenesis ginjal terdiri melalui 3 tahapan secara berurutan yaitu : pronefros, mesonefros, dan metanefros Organogenesis Ginjal Usia gestasi Organogenesis minggu ke- 3 - Sistem pertama yaitu bentuk pronefros dan duktusnya. - Pronefros perlahan akan berdegenerasi dan duktusnya akan menjadi duktus Wolfii dan bagian kaudal dari mesonefros. 4 Sistem pronefros mengalami regresi, saluran ekskresi mesonefros pertama mulai tampak. 5 - Perkembangan dari sistem metanefros (ginjal tetap), berawal dari tunas ureter yang berkembang dari tonjolan saluran mesonefros di dekat muaranya ke kloaka. - Tunas ureter ini menembus jaringan metanefros, yang menutup ujung distalnya sebagai topi melebar membentuk pelvis renalis primitif terbagi kranial dan kaudal kaliks mayor. - Tiap-tiap kaliks membentuk 2 tunas baru yang akan membelah terus hingga 12 generasi atau lebih. - Generasi ketiga dan keempat kaliks minor. Generasi seterusnya piramida ginjal 7-8 Dimulainya nefrogenesis sampai 34-36 minggu. Kemudian nefron berkembang terus dan ukurannya bertambah sesuai dengan pertambahan ukuran ginjal dan perkembangan fungsinya 12-14 Pembentukan pelvis renalis serta kaliks mayor dan minor selesai sebelum masa gestasi ini Antenatally diagnosed hydronephrosis In utero, hydronephrosis is defined as fetal renal pelvic diameter >5 mm in the second trimester and >7 mm in the third trimester. Urine formation is thought to begin between the 5th and 8th weeks of gestation. At 20 weeks, the fetus produces approximately 5 cc of urine per hour and by the 40th week this increases to 50 cc per hour. The fetal bladder can be observed to empty on ultrasound by the 15 th week. The bladder capacity which is approximately 10 cc at 30 weeks increases to 50 cc at term Antenatally diagnosed dilatation of the urinary tract can result from either impairment of urine flow or retrograde reflux of urine. Urine flow impairment can occur at any level in the urinary tract, and may affect one or both sides. When urine flow impairment is present early in pregnancy, the renal parenchyma develops dysplasia10, and in severe cases there is a reduction of the ipsilateral glomerular filtration rate (GFR). Where outflow of urine from both kidneys is markedly impaired, this will result in fetal oliguria or anuria. While fetal urine is not a major contributor to amniotic fluid volume in the first trimester, oliguria or anuria thereafter leads to oligo- or anhydramnios
