Latar
Belakang
BAB 1 PENDAHULUAN
Masalah Kesehatan Global Stroke serta Outcomenya
• Stroke adalah cedera pembuluh darah otak akut yang mengakibatkan kerusakan
pembuluh darah bahkan kecacatan maupun kematian jaringan secara permanen.
• Menurut World Health Organization (WHO) stroke merupakan penyakit di urutan
ke-dua sebagai penyebab kematian terbanyak di dunia. Menurut Riskesdas stroke
menduduki posisi nomor satu sebagai penyebab kematian terbanyak di Indonesia.
• Outcome stroke secara umum merupakan hemiparesis kontralateral, disartria,
hemihipestesi kontralateral, dan atau hemianopsia homonimus kontralateral.
Outcome ini dapat diukur dengan modified Rankin Scale (mRS) yang merupakan
skala ordinal antara 0 (tidak ada gejala paska-stroke) sampai 6 (mati).
Aritmia dan Komplikasinya
• Aritmia merupakan ritme abnormal jantung yang disebabkan oleh gangguan sistem
elektrik jantung. Aritmia dibagi menjadi bradikardia dan takikardia.
• Komplikasi aritmia tersering merupakan stroke kardioembolik dengan jenis aritmia
yang menjadi penyebab tersering adalah atrial fibrilasi (AF).
• Patomekanisme yang dapat menjelaskan hubungan antara aritmia dan stroke
kardioembolik adalah Triad Virchow dan disfungsi kontraksi atrium.
• Kejadian stroke dengan faktor risiko aritmia sebesar 1% - 20% secara global.
• American Heart Association (1996) menyatakan bahwa outcome pasien stroke
dengan risiko aritmia memiliki peningkatan mortalitas, angka kekambuhan, dan
tampilan klinis serta status fungsional yang lebih buruk.
