Pasien dengan permasalahan tulang merupakan fokus utama dalam bidang orthopedi.
Perlakuan yang diberikan sebagai perawatan terhadap pasien dengan patah tulang di
antaranya adalah pemasangan alat bantu serta operasi pada lokasi patah tulang, salah
satunya adalah Open Reduction and Internal Fixation (ORIF). Perlakuan ORIF berisiko
Perbandingan insiden infeksi luka operasi pada pasien patah tulang tertutup
dengan pasien patah tulang terbuka yang sama-sama diberi perlakuan ORIF menjadi
dasar dalam menentukan arah penelitian ini. Sehingga, tujuan umum dari penelitian ini
adalah untuk mengetahui perbedaan antara insiden infeksi luka operasi yang terjadi
terhadap pasien patah tulang terbuka dengan pasien patah tulang tertutup yang sama-
sama diberi perlakuan ORIF serta mengetahui karakteristik bakteri yang dikultur dari
spesimen yang diambil dari para pasien tersebut sehingga diharapkan hal ini dapat
menunjukkan perbedaan insiden infeksi luka operasi antara pasien patah tulang terbuka
mengikuti perkembangan alur perawatan yang didapat oleh pasien serta mengevaluasi
Pada hasil penelitian ini, dalam rentang waktu 10 Februari 2017 sampai dengan
14 November 2017, insiden infeksi luka operasi diperkiraan setidaknya sebesar 2,73%
(12/440) dengan rincian 1,82% (8/440) merupakan pasien patah tulang tertutup dan
0,91% (4/440) merupakan pasien patah tulang terbuka. Penyebab patah tulang
terbanyak adalah kecelakaan lalu lintas (KLL) dan neoplasma pada tulang. Sebagian
besar pasien (75%) merupakan pria. Enam pasien (60%) masuk kelompok usia remaja,
yaitu rentang 12 s.d. 25 tahun. Sebelum mengalami insiden infeksi luka operasi, durasi
operasi rata-rata yang terdapat perlakuan ORIF pada seluruh pasien adalah 4 jam 3
menit +1 jam 38 menit. Spesies bakteri terbanyak yang ditemukan setelah kultur sampel
berupa deep swab pada luka dan/atau aspirat pus berturut-turut adalah Staphylococcus
spesies bakteri patogen yang spesifik terhadap masing-masing jenis patah tulang.

Insiden infeksi luka operasi pada penelitian ini serupa dengan beberapa

ABSTRACT
Objective: To find out the difference between the incidence of surgical site infections
that occurred in open fracture patients and the incidence of surgical site infections that
occurred in closed fracture patients who were both given ORIF treatment; to know the
characteristics of bacteria cultured from specimens taken from these patients so that
this can be expected to show a difference in incidence of surgical site infections
between open fracture patients and closed fracture patients.
Methods: This study was a non-experimental descriptive epidemiology study and was
carried out by following the development of the patient's treatment pathways and
evaluating the patient's medical records.
Results: The incidence of surgical site infections was estimated at 2.73% (12/440).
Only 1.82% (8/440) was from closed fracture patients and 0.91% (4/440) was from
open fracture patients. The cause of most fractures was road traffic accidents and
neoplasms in bone. Most patients (75%) were men. Six patients (60%) belonged to a
group of teenagers, namely the range of 12 s.d. 25 years. Before the incidence of
surgical site infections, the average duration of surgery which includes ORIF in all
patients was 4 hours 3 minutes +1 hours 38 minutes. The most bacterial species found
after sample culture in the form of swabs in wound and/or pus aspirate were
Staphylococcus aureus, Acinetobacter baumannii, and Pseudomonas aeruginosa.
There were no species of pathogenic bacteria that were specific to each type of fracture.
Antibiotics that had the highest resistance numbers were Ampicillin, Chloramphenicol,
and Tetracyclin. Meanwhile, some antibiotics that had the highest sensitive numbers
were Gentamicin, Cotrimoxazole, Clindamycin, and Levofloxacin. All isolates of
Acinetobacter baumannii found were multidrug-resistant organisms (MDRO).
Conclusion: The incidence of surgical site infections in this study was similar to some
studies that had been carried out in other countries. The average duration of surgery in
this study was quite long, thus increasing concerns about the incidence of surgical site
infections. In addition, all isolates of Acinetobacter baumannii found were MDRO.
Therefore, further research is needed which includes the aim of supporting the
effectiveness of surgery, developing guidelines for prevention of the incidence of
surgical site infections, preventing transmission from the health facility environment,
and preventing antibiotic resistance.
