Insiden cedera stres pada atlet yang lebih tua terus meningkat, terkait dengan populasi
lansia yang semakin meningkat. Prinsip yang sama berlaku untuk pencegahan dan
pengobatan, tetapi atlet yang lebih tua biasanya menunjukkan hasil yang lebih
buruk. Adalah wajib untuk meningkatkan kesadaran umum yang sering diabaikan
ini.
 PENDAHULUAN

● Stress fraktur disebabkan oleh ketidakseimbangan antara proses reabsorpsi tulang dan pembentukan tulang. Ini terjadi
karena dominasi aktivitas osteoklas atas aktivitas osteoblas biasanya karena latihan berintensitas tinggi dengan beban
submaksimal pada tulang tanpa istirahat
● Hal ini biasanya terjadi pada orang berusia muda yang aktif, seperti atlet
● Dengan stres yang berkelanjutan, mikrofraktur terjadi dan mungkin membesar sehingga menjadi menyebabkan stress
fractures
● Stress fractures mewakili sekitar 10% dari semua lesi yang terlalu sering terkena dalam latihan olahraga dan umumnya
terutama pada atlet atletik (kejadian tahunan yang didokumentasikan hingga 20% di pelari)
● Hampir semua publikasi laporan stress fractures dalam kisaran usia 10-30 tahun, dengan studi pada atlet yang lebih
tua menjadi langka
● Dengan peningkatan partisipasi olahraga pada populasi yang lebih tua, kejadian patah tulang ini menjadi lebih relevan,
dan diperlukan lebih banyak penelitian tentang topik ini.
Informasi • Seorang pasien pelari wanita berusia 48 tahun dengan
keluhan nyeri mekanis di kalkanealis kanan dan daerah
pra-tibial, berlangsung selama sekitar 7 minggu dan tanpa
episode traumatis.

obat analgesik sendiri dan fisioterapi yang diresepkan
nonmedis.

• Pemeriksaan klinis menunjukkan sedikit edema sisi

lateral, tanpa tanda peradangan lain. Ada nyeri di
peroneus distal dan retinakulum ekstensor, saat palpasi.

• Radiografi didokumentasikan sejajar, melintang, ketiga

distal, stres fraktur fibular dengan kalus tulang yang jelas

• Ultrasonografi mengkonfirmasi adanya fraktur fibula dan

lesi yang dieksklusi pada jaringan lunak yang berdekatan
• Pada riwayat kesehatan sebelumnya, pasien melaporkan
ketidakteraturan dalam siklus menstruasinya dan sudah ditangani oleh
asisten reumatologi

• Pasien dipersilahkan ke rumah dan disuruh untuk mempertahankan
non-weight bearing regimen pada tungkai kanan selama 2 minggu.

dengan pendapat medis, berlari 7 km sehari dan hanya melaporkan
nyeri ringan

• Evaluasi radiografi menunjukkan kalus hipertrofik, tanpa berderetan.

• Tindak lanjut pada 6 bulan menunjukkan atlet bebas nyeri, mampu

melakukan latihan dasar tanpa gejala. Radiografi mendokumentasikan
reabsorpsi kalus dan remodeling tulang, dengan konsolidasi fraktur

• Hampir setiap tulang rentan terhadap stres fraktur, tetapi 95%
dilaporkan terjadi pada ekstremitas bawah
• Pada pelari, stres fraktur biasanya terjadi di bagian kortikal tulang

kelelahan termasuk berikut: tibia (33%), tulang tarsal (20%),
metatarsal (20%), femur (11%), fibula (7%), dan panggul (7%).

• Insiden cedera stres pada atlet yang lebih tua terlihat meningkat. Kunci
untuk diagnosis yang efektif dari potensi fraktur stres adalah tinjauan
yang cermat terhadap riwayat fraktur pasien dan pemeriksaan klinis.
Faktor risiko tampaknya sama seperti untuk atlet yang lebih muda,
dengan fisiologi cedera diyakini multifaktorial dalam kedua kasus.
• Prinsip yang sama berlaku untuk pencegahan dan pengobatannya, tetapi
penerapannya harus ditekankan dan diawasi, karena atlet yang lebih tua
