ABSTRACT
The initial assessment is a sheet of stages in the evaluation process by doctors and nurses on
patient data, both subjective and objective, regarding the patient's health status. It is still found that the
patient's initial assessment form is filled out incompletely in the important report section. The initial
assessment of inpatients obtained as a result of observations carried out by researchers on June 22
20223 in the field was found to be incomplete in filling out the initial assessment form (100%). The aim
of the research is to determine the completeness of filling out the initial assessment for inpatients at
Aisyiah Hospital in Padang. This type of quantitative research with a descriptive approach, this
research was conducted at Aisyiah Padang Hospital in February-July 2023, data was collected on July
10-12 2023. The population was 412 medical records and a sample of 80 inpatient medical records
were taken using a sampling technique. namely accidental sampling, data collection was carried out by
observation using a checklist table, data was analyzed univariately and processed using
computerization. The research results were obtained from 80 initial inpatient assessment medical
records that identity was complete (100%), important reports were incomplete (100%), authentication
was complete (100%) and record review was complete (98.8%) incomplete (1.3 %). Based on the
results of the research, it can be concluded that there is still incompleteness in filling out the patient's
initial assessment form due to dual factors, so it is hoped that the hospital will monitor the filling in of
the medical record file. Pay more attention so that all sections in the initial assessment are filled in and
that there are no remaining sections that are left unfilled.
