1. Berperan dalam LTP dan LTD, ini adalah efek biphasic pada eksitabilitas, kognisi dan memori
pada ritmik diurnal dan setelah stres tipe acute non-painful (Diamond et al 1996).
2. Berparitisipasi dalam neurogenesis di girus dentatus dengan eksitabilitas asam amino. Pada
keadaan akut stres dapat menghambat neurogenesis yang berlangsung (Galea et al 1996),
terutama menyangkut fear-related learning and memory karena secara anatomi dan fungsional
terdapat hubungan antara amigdala dan girus dentatus (Ikegaya et al., 1997).
3. Partisipasi pada stress-induced atrophy pada girus dentatus dan regio CA3 pada hipokampus
yang berakibat penurunan fungsi memori (McEwen., 1999).
Stres akan mempengaruhi baik fungsi maupun struktur hipokampus, beberapa studi
menunjukkan korelasi ke duanya, sebagai contoh stres dapat mengakibatkan demensia (Kaplan et al.,
2001), depresi ataupun skizofrenia (Heim et al., 2004). Stres pada saat prenatal dapat menyebabkan
pengecilan ukuran pada hipokampus yang nantinya mengakibatkan penurunan fungsi kognisi (Kim et
al., 2002). Akibat stres baik akut atau kronik akan mengaktivasi beberapa gen yaitu CREB dan Fos
yang menyebabkan terjadinya perubahan tingkah laku dan memori (Blank et al., 2004, Chen et al.,
2006).
Rangsangan HPA aksis dengan dilepaskannya adrenal steroid akan berperan dalam plastisitas
di hipokampus melalui NMDA dan beberapa asam amino, di saat yang sama juga dapat menimbulkan
efek destruksi sehingga berakibat penurunan fungsi kognisi.
Glucocorticoid, CAMs dan Faktor Neurotropik
Stresor mempunyai dampak di tingkat biomolekuler, serta dapat mengakibatkan gangguan aktivitas
neuronal, yaitu dengan ditemukannya ekspresi neuronal cell adhesion molecules (NCAM) yang
berperan dalam formasi sinaps dan stabilisasi transmisi sinaps (Kiss, 2001).
Ekspresi kronik glucocorticoid akan mengakibatkan perubahan kadar CAM pada beberapa area di otak.
Hal ini disebabkan karena adrenosteroid mempunyai efek langsung terhadap transkripsi gen dari CAM,
yaitu dengan melakukan interaksi bersama dua faktor transkripsi yaitu nuclear factor-B atau NF-B
dan activator protein-1 atau AP-1 (Vreugdenhil, 2001). Penelitian Datson (2001) menggunakan
teknik Serial Analysis of Gene Expression (SAGE) menemukan bahwa NCAM-140 tersupresi oleh
reseptor glucocorticoid (GR) tetapi tidak oleh reseptor mineralocorticoid (MR)
Rangsangan Glucocorticoid pada kondisi stres kronis akan mengakibatkan peningkatan ekspresi dari
NMDA reseptor dari glutamat, disertai penurunan respons AMPA reseptor di hipokampus (Bartanusz,
1995). NMDA akan mengaktivasi NCAM-140, sedangkan AMPA akan merangsang NCAM-180 tetapi
bukan NCAM-140, NCAM-140 isoform yang mempunyai peran dalam stabilisasi sinaps.(Touyarot,
2002). Jadi setiap isoform dari NCAM mempunyai efek yang berbeda.
Rangsangan glucocorticoid akan menurunkan kadar brain derived neurotrophic factor (BDNF) (Smith
et al., 1995) dan sebaliknya terjadi peningkatan ekspresi fibroblast growth factor-2 (FGF-2). Hal ini
diduga merupakan mekanisme neuroprotektif. FGF reseptor mempunyai efek interaksi secara langsung
ke NCAM-140 yang penting untuk neuriteoutgrowth (Niethammer, 2002). NCAM-140 juga mempunyai
fungsi sebagai reseptor dari glial cell line-derived neurotrophic factor (GDNF) yang mempunyai peran
penting dalam perkembangan akson di hipokampus (Paratcha, 2003). Beberapa studi menunjukkan
bahwa pada kondisi distress juga akan mempengaruhi intracellular tranduction pathway dari
neurotrophin, yaitu kaskade dari Ras-mitogen acctivated protein kinase (MAPK), yang berperan
penting dalam sinaptik plastisitas dan ketahanan neuronal (Meller, 2003). Stimulasi GR yang berlebihan
akan mengakibatkan defisit energi dari hipokampus berupa penurunan ATP sehingga aktivitas
proteolitik dari NCAM meningkat serta penurunan ikatan membran dari NCAM (Skladchikova, 1999).
Jadi kondisi distress yang berkepanjangandapat mengakibatkan penurunan biosintesis NCAM disertai
peningkatan katalisis.
Peran Adaptasi Otak
Dengan adanya suatu stres, maka otak akan memberikan 2 macam bentuk respon, yaitu respon cepat
dan lambat. Respon cepat dari otak berupa pelepasan corticotropin-releasing hormone (CRH) yang
merupakan motor sistem simpatik melalui CRH-1 reseptor (CRHR1) yang akan mengakibatkan
respon fight-flight. CRHR1 akan merangsang HPA aksis yang nantinya akan menghasilkan
mineralocorticoid. Sedangkan pada respon lambat ini dapat terjadi mekanisme koping dan adaptasi
melalui perangsangan CRHR2, yang nantinya akn merangsang pelepasan glucocorticoid (Heinrichs et
al., 2004) (Gambar.3).
Corticosteroid mempunyai binary action terhadap suatu stres yaitu respon akut dan respon recovery
atau adaptasi. Akibat dari stres maka baik MR maupun GR akan menyebabkan ekspresi 70-100 gen di
hipokampus antara lain: biogenik amine, neuropeptide (Sabban et al., 2001), neurotrophic factor atau
growth factor (Schaaf et al 1997), dan CAMs (Sandi et al., 2004) yang nantinya akan mengakibatkan
perubahan baik pada tingkat molekular, selular dan behavioral .
Kortikosteroid mempunyai sifat sebagai pleiotropic agents, mempunyai efek sebagai hormon yang
dapat mengatur transkripsi beberapa gen. Beberapa protein dan neurotransmiter diantaranya voltage
gated ion-channel, G protein-couple receptor, serta serotonin 5-HT reseptor, kadarnya terpengaruh
akibat rangsangan corticosteroid dan stres. Meskipun bersifat pleiotropic, steroid tidak mempengaruhi
seluruh tingkat seluler, misalnya: meskipun L-type Ca2+ channel sangat terpengaruh akan tetapi
potassium dan sodium channel kurang terpengaruh (de Kloet et al., 2005).
Gagalnya mekanisme koping
Respon dari suatu stres dipengaruhi oleh predisposisi genetik dan juga pengalaman sebelumnya
terutama pada saat usia muda, hal ini merupakan seleksi genetik dimana terdapat dysregulasi dari HPA
dan sistem biogenic amine pada individu tertentu (Veenema et al., 2203, Degen et al., 2004) .
Beberapa studi menunjukkan bahwa adanya korelasi antara disregulasi HPA aksis dengan risiko
terjadinya depresi, meskipun sering kali tidak ditemukan hiperkortisolemia pada pemeriksaan basal
cortisol level, akan tetapi pada pemeriksaan cortisol urine per hari terlihat peningkatan yang jelas (de
Kloet et al., 2005).
Gangguan pada aksis HPA juga merupakan risiko untuk terjadi relaps pada pasien depresi (Zobel,
2001). Penelitian pada individu yang sehat menunjukkan bahwa perubahan yang subtle dari fungsi
aksis HPA merupakan suatu pertanda untuk terjadinya suatu depresi atau penyakit lain
akibat distress (Sullivan, 2000).
Kesimpulan
Kortikosteroid merupakan mediator penting dalam respon terhadap stres. Ke dua resptornya yaitu MR
dan GR, mempunyai fungsi yang sangat kompleks, yang dapat mengakibatkan perubahan baik di
tingkat molekuler, seluler maupun tingkah laku.
Adanya perubahan yang sangat kecil pada fungsi HPA aksis dapat mengakibatkan gangguan pada
mood dan tingkah laku. Hal ini disebabkan karena adanya interaksi antara hipokampus, korteks pre-
frontal dan amigdala dengan HPA aksis.
Profil genetik, kepribadian dan pengalaman masa kecil berperan pada mekanisme koping individu
terhadap stres.
