General structure

conversation in the dialogue text above, would you mind helping us to explain

Hi Frank, I am so happy that you and Hannah ask me about either the
previous conversation or video. Well, I’ll try to answer your questions one by
one. Firstly you should know that the previous material is called as
registration form. Registration form is a document (printed or electronic)
with spaces in which we can write or enter data.
a. If we talk about the purpose toward both the pervious materials. It can
be classified into some purposes because there are many kinds of forms;
therefore, there will be many different purposes too. Commonly, Form is
having some purposes or functions, such as:
 To record a customer enquiry
 To apply for something
 To make a booking reservation
 To ask people for their opinions ( questionnaires)
b. As I have said before that there are many kinds of form. E.g.,
 Purchase form
 Order form All those form can be used depending on
 Withdrawal slip the situation you deal with. For instance,
 Deposit slip when you want to stay at a hotel, you
 Reservation slip should make a hotel reservation, before
 Medical form staying.

We got it Professor Norman. I think this material is quite interesting for


us, then, I have a question. What should we do to make a form? Should we
know parts of form first?

Nice question, Hannah. Um surely you should know part of a form. I will
explain it for you. Generally there are some formats of the form that
you should know.
a. The name of the organization and/or department. The address is
normally included only if the form will be outside the organization
b. Instructions on how to complete the form. Some forms put the

a. There are several rules when you want to make a form:
 You should give clear instructions. Most forms state if black pen
should be used. They may also specify block capitals because
the form is then much easier to read.
 Include a space for a mobile phone number and email address.
It is sensible if you may want to contact the person who
completed it. Long forms are normally divided into sections,
under headings, so that people can focus their mind on the
different types of information that is required.
 Option choices are often given.
To select an option, we can use:
- By putting a tick or cross on a dotted line or in a box.
- By deleting the option that does not apply
- By writing “yes” or “no” as appropriate
Generally these options should be never mixed on one form.
Choose one method and stick to it and put an appropriate
instruction at the top of the form.
An alternative is to put an asterisk (*) against all the options
with the instructions at the bottom.
Example: *Delete an appropriate.

Moreover, if you want to complete a form, you should:


a. Read the form through first
b. Study all the instructions carefully. Make sure you have the right
type of pen, know whether you must print everything and have
