PRAKATA
Bismillahirrahmanirrahim.
ASSALAMULAIKUM W.B.T

Alhamdulillah, marilah kita bersama-sama merafakkan setinggi-tinggi kesyukuran ke hadrat


Allah S.W.T kerana dengan limpah izin dan kurniaNYA jua Buku Panduan Penyediaan
Jadual Harga bagi Kontrak Pengurusan Fasiliti dan Penyenggaraan Bagi Bangunan
Kerajaan ini dapat dihasilkan.

Untuk makluman semua, Buku Panduan ini telah dibangunkan selari dengan keperluan
kepada PWD Facilities Management & Maintenance (FMM) 2016, FMM Technical
Specification, pekeliling dan surat arahan Perbendaharaan, pekeliling dan surat arahan
Ketua Pengarah Kerja Raya yang berkaitan.

Beberapa siri mesyuarat dan bengkel telah diadakan bagi tujuan pembangunan Buku
Panduan ini dimana Bahagian Perundingan Pengurusan Aset CPAB telah menjemput
pejabat Pakar dan pejabat operasi JKR seperti Cawangan Senggara Fasiliti Bangunan, JKR
Wilayah Persekutuan Putrajaya dan JKR Wilayah Persekutuan Kuala Lumpur. Keahlian
mesyuarat dan bengkel adalah terdiri dari pegawai-pegawai yang mempunyai pengalaman
kerja dan pengetahuan dalam pelaksanaan kontrak FMM.

Objektif Buku Panduan ini adalah sebagai rujukan dan panduan dalam proses menyediakan
Jadual Harga tender oleh pejabat yang menyedia dan memanggil tender (PMT) bagi
Kontrak Pengurusan Fasiliti dan Penyenggaraan. Selain daripada dokumen rujukan, Buku
Panduan ini juga mempunyai objektif untuk menwujudkan keseragaman dalam amalan
penyediaan Jadual Harga bagi Kontrak Pengurusan Fasiliti dan Penyenggaraan serta
membantu dalam usaha mengumpul dan membina satu data kos yang boleh dijadikan
rujukan bagi skop kerja Kontrak Pengurusan Fasiliti dan Penyenggaraan kepada Jabatan.

Adalah diharapkan Buku Panduan ini dapat membantu pegawai dalam meningkatkan
keberkesanan proses kerja dan kemahiran dalam penyediaan tender dan anggaran tender
Kontrak Pengurusan Fasiliti dan Penyenggaraan khususnya.

1.0 PENGENALAN

Buku Panduan ini adalah sebagai dokumen rujukan kepada Pejabat


Mengurus Tender (PMT) dalam menyediakan Jadual Harga bagi perolehan
kontrak Pengurusan Fasiliti dan Penyenggaraan (Facilities Management and
Maintenance (FMM)). Dokumen ini dibangunkan sebagai langkah
penambahbaikan kepada perlaksanaan tender sedia ada yang mana berlaku
ketidakseragaman di dalam penyediaan Jadual Harga bagi kontrak FMM oleh
pejabat yang memanggil tender di JKR.

Syarat-syarat Kontrak FMM 2016 klausa 1 dan klausa 30 memperuntukkan


Jadual Harga (Schedule of Price) merupakan sebahagian dari dokumen
kontrak dan menjadi asas kepada harga sesebuah kontrak FMM. Ia
mengandungi huraian serta butiran kerja bagi kontrak FMM tersebut. Adalah
penting Jadual Harga disediakan dengan tepat, jelas dan merangkumi
keseluruhan skop kerja bagi mengelakkan kesamaran, percanggahan atau
kesilapan yang boleh menimbulkan pertikaian antara kontraktor dan Kerajaan.

Dokumen ini boleh dijadikan template kepada penyediaan Jadual Harga


Senarai Kuantiti FMM dan Pejabat Mengurus Tender (PMT) boleh meminda,
menambah atau mengurangkan deskripsi atau kuantiti semasa penyediaan
Jadual Harga Senarai Kuantiti mengikut kehendak dan keperluan premis
masing-masing.

2.0 OBJEKTIF

i) Mewujudkan keseragaman dalam amalan penyediaan Jadual Harga bagi


kontrak FMM;
ii) Memastikan semua petender menghargakan tender berdasarkan asas
yang sama;
iii) Memastikan penilaian dan pengurusan prestasi kerja kontraktor dapat
diukur dengan lebih mudah dan tepat;
iv) Membantu ke arah penyediaan data kos Pengurusan Fasiliti dan
Penyenggaraan lebih efektif dan tepat di peringkat perancangan dan
kawalan kos.

3.0 SKOP DOKUMEN

Dokumen ini mengandungi panduan penyediaan Jadual Harga Senarai Kuantiti


bagi dokumen kontrak utama FMM dan dokumen sub kontrak perkhidmatan
Penjagaan Fasiliti merujuk kepada perkara-perkara berikut:

i) Dasar Pengurusan Aset Kerajaan;


ii) Pekeliling Am. Bil 1 Tahun 2009 : Manual Pengurusan Aset Menyeluruh
Kerajaan;
iii) Pekeliling Am. Bil 2 Tahun 2012 : Tatacara Pengurusan Aset Tak Alih
Kerajaan;
iv) Arahan Perbendaharaan dan Pekeliling Perbendahaan;
v) Surat Kementerian Kewangan - Hala Tuju Dasar Perolehan Kerajaan
berkaitan Kontrak Pengurusan Fasiliti dan Penyenggaraan Dalam Projek
Kerajaan rujukan 3/K.KEW/PK/MOF/D/29 SK.11(33) bertarikh 9 Disember
2016;
vi) Syarat-syarat Kontrak PWD Facilities Management & Maintenance Form
2016;
vii) Garis Panduan Perolehan dan Pentadbiran Kontrak Pengurusan Fasiliti

4.0 PANDUAN PENYEDIAAN JADUAL HARGA

4.1 PENGENALAN

Buku Panduan ini menyediakan dua (2) template Jadual Harga Senarai Kuantiti iaitu:

a. Untuk Kontrak Utama - Jadual Harga Pengurusan Fasiliti dan


Penyenggaraan (FMM);
b. Untuk Sub-Kontrak – Jadual Harga Perkhidmatan Penjagaan Fasiliti iaitu
Perkhidmatan Pembersihan dan Mencuci (Housekeeping & Cleaning
Services) dan Perkhidmatan Kawalan Serangga Perosak (Pest Control
Services).

Penyediaan Jadual Harga Senarai Kuantiti hendaklah dibaca bersama dengan


GBSMG Edisi 2019 atau yang terkini dan dokumen-dokumen rujukan lain seperti
manual yang dikeluarkan oleh pengilang dan lain-lain. Ini bagi membantu
menghasilkan keterangan kerja dan anggaran jabatan berdasarkan kekerapan yang
dicadangkan.

Selain itu, keterangan kerja juga perlu merujuk kepada Senarai Aset, Laporan
Pemeriksaan Bangunan (Facility Conditions Assessment (FCA)), lukisan siap bina,
dan lain-lain rujukan bagi menghasilkan keterangan yang komprehensif.

PMT boleh mengubah/meminda keterangan kerja dan kekerapan dalam template ini
mengikut keperluan dan keadaan aset/premis yang berkaitan.

Tempoh kontrak yang diguna bagi contoh pengiraan kuantiti dalam penyediaan
templete jadual harga ialah 3.25 tahun iaitu termasuk 3 bulan tempoh pengurusan
peralihan masuk.

4.2 JADUAL HARGA PENGURUSAN FASILITI DAN PENYENGGARAAN (FMM)


KONTRAK UTAMA

i) Preambles

Menerangkan skop dan cara menghargakan kadar harga bagi memastikan


petender mengambilkira semua faktor yang mempengaruhi sesuatu harga.
Preambles ini hendaklah dimasukkan dalam penyediaan Jadual Harga dan
menjadi sebahagian daripada dokumen kontrak.

ii) Kerja-kerja Permulaan (Preliminaries)

Senarai Kuantiti ini mengandungi kehendak kontraktual dan keperluan


permulaan sesuatu kontrak seperti Bon Perlaksanaan, Insurans dan lain- lain.
PMT hendaklah menyatakan keperluan pejabat tapak dan peralatan PPF
(FSO site office and equipment) mengikut keperluan pentadbiran kontrak
premis. Penyediaan peruntukan bagi kerja-kerja Permulaan hendaklah
dirujuk bersama dengan pekeliling dan arahan semasa yang sedang berkuat
kuasa dalam Jabatan.

iii) Pengurusan Peralihan (Transition Management)

Senarai Kuantiti ini mengandungi keperluan semasa Tempoh Peralihan


Masuk (Transition In) iaitu 3 bulan dari Tarikh Mula Kontrak dan Tempoh
Peralihan Keluar (Transition Out) iaitu 3 bulan sebelum Tarikh Tamat Kontrak.
Pengiraan kuantiti bagi Tempoh Peralihan Masuk diukur secara kuantiti
sementara kerana ianya bergantung kesediaan pejabat FSO dan tarikh tamat
kontrak sedia ada. Pengiraan kuantiti Tempoh Peralihan yang diguna sebagai
contoh dalam penyediaan Templete Jadual Harga ini adalah 3 bulan.
Diantara aktiviti yang disenaraikan dibawah Pengurusan Peralihan ini ialah ;

a) Peralihan Masuk
Mengadakan pejabat tapak.
Mengadakan Sistem Pengurusan Maklumat (Management
Information System[MIS] samaada membangun, mengguna,
mengurus dan menyenggara MIS atau mengguna, mengurus

dan menyenggara MIS sedia ada sebagaimana yang


ditetapkan dalam kontrak.
Mengurus, mengemaskini, menyenggara dan menyelaras
inventori dan data aset menggunakan MIS yang
dibangunkan/sedia ada.
Melaksanakan Facilities Familiarization and Verification (FFV)
melalui FCA.
Hadir dan menyaksikan pengujian, pentauliahan dan
penyerahan yang dilakukan oleh pihak yang terlibat sama ada
pemilik premis atau kontraktor sedia ada.
Menghadiri semua latihan dan aktiviti yang ditetapkan dalam
kontrak dan seperti yang diarah oleh PPF.
Mengenal pasti dan menyenarai kecacatan baharu yang tidak
terkandung dalam FCA .
Sentiasa memaklumkan kepada PPF sekiranya ada aktiviti
FMM yang sedang dilaksanakan tidak sempurna atau diluar
skop kerja.

b) Peralihan Keluar
Mengemaskini Pelan Pengurusan Peralihan dalam tempoh
satu bulan sebelum tempoh peralihan keluar.
Memberi kerjasama kepada kontraktor baharu bagi
menjalankan FFV
Menyenaraikan baki kerja yang tidak dapat diselesaikan
Melaksanakan mesyuarat pasukan koordinasi peralihan keluar
Melaksana dan menyiap semua pengujian dan pentauliahan
serta menyerahkan sijil dan warranti berkaitan.

iv) Pelan Pengurusan Fasiliti (Facility Mangement Plan (FMP)

FMP merupakan satu dokumen yang perlu dibangunkan oleh kontraktor


mengikut skop kerja yang ditender dengan merujuk bersama kepada GBSMG
Edisi 2019 atau yang terkini dan FMM Technical Specification, merangkumi
pelaporan yang perlu disediakan. PPF hendaklah memastikan FMP yang

dikemukakan oleh kontraktor menepati Syarat-syarat kontrak FMM 2016


klausa 9 dan menggaji pekerja dibawah kontrak mengikut klausa 13 dan
keperluan yang dinyatakan di dalam FMM Technical Specification.

Sukacita diingatkan amalan menghargakan kos kakitangan pengurusan


kontraktor dibawah Human Resources Management Plan tidak lagi
diperuntukkan di bawah item ini kerana kos buruh, bahan, mesin dan perlatan
hendaklah dikira termasuk dalam kos kerja operasi dan penyenggaraan di
bawah Bill 4 - Operation and Maintenance dan Bill 5 – Repair and
Replacement.

v) Operasi dan Penyenggaraan (Operation and Maintenance)

Senarai Kuantiti operasi dan penyenggaraan merangkumi kerja sivil, struktur,


senibina, mekanikal, elektrikal, landskap, Computerizes Maintenance
Management System (CMMS) dan Energy Efficiency.

Pengukuran kuantiti pelaksanaan operasi dan penyenggaraan adalah


merujuk kepada tempoh kontrak tidak termasuk Transition In (3 bulan) serta
menjadikan kekerapan penyenggaraan pada Government Building Scheduled
Maintenance Guidelines (GBSMG) edisi 2019 atau yang terkini sebagai
rujukan.

PMT hendaklah menyenaraikan kerja operasi dan penyenggaraan bagi bil ini
mengikut premis sebagaimana Site Information and Asset Inventories
(Appendix 8) disertakan dalam tender.

vi) Repair and Replacement

Senarai Kuantiti ini perlu disedia berdasarkan laporan Facility Condition


Assessment (FCA) dan lain-lain rekod penyenggaraan yang berkaitan.

Senarai kuantiti ini diukur secara Kuantiti Sementara (Provisional Quantity)


dan hendaklah diukur semula berdasarkan kerja sebenar yang dilaksanakan
ditapak. Penerangan bagi kerja-kerja yang terlibat perlu disertakan bersama
dengan lukisan dan spesifikasi bagi kerja tersebut.

PMT hendaklah membuat anggaran kuantiti kepada repair and part


replacement, dalam keadaan dimana tiada maklumat FCA atau maklumat
tidak dapat digunakan, PMT hendaklah membuat anggaran. Bagi tujuan
penyediaan panduan jadual harga ini kita mengambil contoh 10% dari kuantiti
keseluruhan bagi setiap aset/komponen yang ditentukan.

vii) Provisional Sums

Senarai Kuantiti ini mengandungi kerja yang diperuntukkan sebagai


provisional sum di peringkat tender FMM. Di antara kerja yang diperuntukkan
sebagai Wang Peruntukan Sementara (WPS) ialah kerja Housekeeping &
Cleaning Services dan Pest Control Services dan lain-lain kerja yang tidak
dapat dianggar dan dikenalpasti di peringkat penyediaan tender.

Syarat-syarat Kontrak Pengurusan Fasiliti dan Penyenggaraan 2016 Klausa


30.3 memperuntukkan PPF boleh membelanja sebahagian atau keseluruhan
WPS yang diperuntukkan dan mengarah kontraktor untuk melaksanakan
WPS tersebut.

4.3 JADUAL HARGA PERKHIDMATAN PENJAGAAN FASILITI :


PERKHIDMATAN PEMBERSIHAN DAN MENCUCI (HOUSEKEEPING &
CLEANING SERVICES)

i) Preambles

Menerangkan skop dan cara menghargakan kadar harga bagi memastikan


petender mengambilkira semua faktor yang mempengaruhi sesuatu harga.
Preambles ini hendaklah dimasukkan dalam penyediaan Jadual Harga
dan menjadi sebahagian daripada dokumen kontrak.

ii) House Keeping & Cleaning Services

Senarai Kuantiti ini mengandungi kerja operasi perkhidmatan pembersihan


dan mencuci mengikut ruang, jenis kemasan dan ketetapan pencapaian
prestasi. Pengukuran kuantiti kepada tempoh pelaksanaan operasi
perkhidmatan adalah merujuk kepada tempoh kontrak tidak termasuk
tempoh pengurusan Peralihan Masuk (3 bulan).

4.4 JADUAL HARGA PERKHIDMATAN PENJAGAAN FASILITI :


PERKHIDMATAN KAWALAN SERANGGA PEROSAK (PEST CONTROL
SERVICES)

i) Preambles

Menerangkan skop dan cara menghargakan kadar harga bagi memastikan


petender mengambilkira semua faktor yang mempengaruhi sesuatu harga.
Preambles ini hendaklah dimasukkan dalam penyediaan Jadual Harga
dan menjadi sebahagian daripada dokumen kontrak.

ii) Perkhidmatan Kawalan Serangga Perosak

Senarai Kuantiti ini mengandungi operasi perkhidmatan kawalan serangga


perosak. Pengukuran kuantiti pelaksanaan operasi perkhidmatan adalah
merujuk kepada tempoh kontrak tidak termasuk tempoh pengurusan
Peralihan Masuk (3 bulan).

PREAMBLES TO SCHEDULE OF PRICE

1. GENERAL DIRECTIONS

1.1 The Contract comprises the management, administration, supervision,


operation and maintenance of the said FMM Services. The FMM Services
shall include the provision of all labour, material, plants, equipment,
temporary or permanent works as required in the Contract.

1.2 The Schedule of Price shall be read in conjunction with the Conditions of
Contract and its appendices i.e the Government Requirement, FMM
Technical Specifications, Performance Requirement, Site Information &
Asset Inventories, Schedule of Maintenance Frequency and Scope of
Services in the Contract Documents.

1.3 The headings, sub-headings and item descriptions in the Schedule of Price
identify the works covered. However the exact nature and extent of the
work to be carried out shall be ascertained by reference to the Contract as
a whole, in particular the Government Requirement, FMM Technical
Specifications, Performance Requirement, Site Information & Asset
Inventories and Scope of Services.

1.4 The rates and prices inserted in the Schedule of Price shall include cost of
the works described, all risks, liabilities and obligations set forth or implied
in the Contract Document and shall be deemed to include the following,
unless expressly stated otherwise:

(a) the provision of labour and all costs in connection therewith;

(b) the supply of materials, goods, equipment and storage thereof


including delivery to site and all costs in connection therewith;

(c) taking delivery of materials, goods and equipment necessary for the
FMM Services and including those supplied by others, loading and
unloading, storage and all costs in connection therewith;

(d) the provision of plants and machineries and all costs in connection
therewith;

(e) the provision of making good and all cost in connection therewith
unless separately provided for in the Schedule of Price;

(f) the provision of consumables item and all cost in connection


therewith unless separately provided for in the Schedule of Price;

(g) the fixing, erecting and installing or placing in position of materials,


goods and equipment including around obstruction, tolerances,
penetration, working space, over-breaks etc. and all costs in
connection therewith;

(h) the provision of all temporary works and all costs in connection
therewith unless separately provided for in the Schedule of Price;

(i) the phasing requirements of the Works including the effect on the
programming of the Works of all client needs, complaints and other
restrictions, alterations and modification to Work (executed under
the Contract or in conjunction with it) and all other requirements of
the Contract and all costs in connection therewith;

(j) all cost of testing to show proof of compliance with the FMM
Technical Specification on materials and workmanship inclusive of
samples of materials and submission of tests results. Should the
frequency of testing and sampling not specified, then it shall be in
accordance with the relevant Malaysian/British Standards and
Codes of Practice. Where there is no such relevant Standards and
Codes of Practice for reference, it shall then be as instructed by the
FSO.;

(k) all royalties and dues which the Contractor may be required to pay;

(l) wastage;

(m) water and electricity for the Works;

(n) establishment charges, overheads and profit;

(o) whatsoever method of work the Contractor may adopt.

3. UNPRICED ITEMS

3.1 Items against which no price or rate is entered will not be paid for when
executed and shall be deemed to be covered in other prices or rates in the
Schedule of Price.

3.2 All cost not included elsewhere but necessary for compliance with each
and every term in the Conditions of Contract shall be deemed to have been
covered by the rates and prices in the Schedule of Price.

4. TRIALS, SAMPLES, TESTS AND INSPECTION

The rates and prices shall include for tests and the effects on the progress
of the Works for the carrying out of all trials, sampling and tests whether
specified in the Contract Document or otherwise in order to ensure
compliance with the requirements of the FMM Technical Specification
except where separately itemized in the Schedule of Price.

5. PROVISIONAL SUM

All provisional sums included in the Schedule of Price are intended to be


approximate indications only of the amounts to be expended in respect of
the work described and may be omitted entirely or substantially varied

without entitling the Contractor to any claim for compensation. No part of
the Contractor’s overhead, expenses or profit should be included in these
sums when the bill is priced unless items for profit and attendance are
specifically allowed for.

6. PROVISIONAL QUANTITY

All Provisional Quantities in Provisional Quantities Bill are estimated


quantities which shall not be taken as actual and correct quantities of works
to be executed by the contractor in the fulfillment of its obligations under
the Contract. The amount to be paid shall be ascertained by re-
measurement of the works as it is actually executed.

PREAMBLES TO SCHEDULE OF PRICE

1. GENERAL DIRECTIONS

1.1 The Contract comprises the management, administration, supervision,


operation and maintenance of the said FMM Services. The FMM Services
shall include the provision of all labour, material, plants, equipment,
temporary or permanent works as required in the Contract.

1.2 The Schedule of Price shall be read in conjunction with the Conditions of
Contract and its appendices i.e the Government Requirement, FMM
Technical Specifications, Performance Requirement, Site Information &
Asset Inventories, Schedule of Maintenance Frequency and Scope of
Services in the Contract Documents.

1.3 The headings, sub-headings and item descriptions in the Schedule of Price
identify the works covered. However the exact nature and extent of the
work to be carried out shall be ascertained by reference to the Contract as
a whole, in particular the Government Requirement, FMM Technical
Specifications, Performance Requirement, Site Information & Asset
Inventories and Scope of Services.

1.4 The rates and prices inserted in the Schedule of Price shall include cost of
the works described, all risks, liabilities and obligations set forth or implied
in the Contract Document and shall be deemed to include the following,
unless expressly stated otherwise:

(a) the provision of labour and all costs in connection therewith;

(b) the supply of materials, goods, equipment and storage thereof


including delivery to site and all costs in connection therewith;

(c) taking delivery of materials, goods and equipment necessary for the
FMM Services and including those supplied by others, loading and
unloading, storage and all costs in connection therewith;

(d) the provision of plants and machineries and all costs in connection
therewith;

(e) the provision of making good and all cost in connection therewith
unless separately provided for in the Schedule of Price;

(f) the provision of consumables item and all cost in connection


therewith unless separately provided for in the Schedule of Price;

(g) the fixing, erecting and installing or placing in position of materials,


goods and equipment including around obstruction, tolerances,
penetration, working space, over-breaks etc. and all costs in
connection therewith;

(h) the provision of all temporary works and all costs in connection
therewith unless separately provided for in the Schedule of Price;

(i) the phasing requirements of the Works including the effect on the
programming of the Works of all client needs, complaints and other
restrictions, alterations and modification to Work (executed under
the Contract or in conjunction with it) and all other requirements of
the Contract and all costs in connection therewith;

(j) all cost of testing to show proof of compliance with the FMM
Technical Specification on materials and workmanship inclusive of
samples of materials and submission of tests results. Should the
frequency of testing and sampling not specified, then it shall be in
accordance with the relevant Malaysian/British Standards and
Codes of Practice. Where there is no such relevant Standards and
Codes of Practice for reference, it shall then be as instructed by the
FSO.;

(k) all royalties and dues which the Contractor may be required to pay;

(l) wastage;

(m) water and electricity for the Works;

(n) establishment charges, overheads and profit;

(o) whatsoever method of work the Contractor may adopt.

3. UNPRICED ITEMS

3.1 Items against which no price or rate is entered will not be paid for when
executed and shall be deemed to be covered in other prices or rates in the
Schedule of Price.

3.2 All cost not included elsewhere but necessary for compliance with each
and every term in the Conditions of Contract shall be deemed to have been
covered by the rates and prices in the Schedule of Price.

4. TRIALS, SAMPLES, TESTS AND INSPECTION

The rates and prices shall include for tests and the effects on the progress
of the Works for the carrying out of all trials, sampling and tests whether
specified in the Contract Document or otherwise in order to ensure
compliance with the requirements of the FMM Technical Specification
except where separately itemized in the Schedule of Price.

5. PROVISIONAL SUM

All provisional sums included in the Schedule of Price are intended to be


approximate indications only of the amounts to be expended in respect of
the work described and may be omitted entirely or substantially varied

without entitling the Contractor to any claim for compensation. No part of
the Contractor’s overhead, expenses or profit should be included in these
sums when the bill is priced unless items for profit and attendance are
specifically allowed for.

6. PROVISIONAL QUANTITY

All Provisional Quantities in Provisional Quantities Bill are estimated


quantities which shall not be taken as actual and correct quantities of works
to be executed by the contractor in the fulfillment of its obligations under
the Contract. The amount to be paid shall be ascertained by re-
measurement of the works as it is actually executed.
