Pelan-pelan tersebut di atas, speksifikasi dan dokumen sebutharga yang lain boleh dilihat
di tempat yang dinyatakan dalam Notis sebutharga dalam masa waktu pejabat pada mana-
mana hari bekerja hingga tarikh akhir yang ditetapkan bagi penyerahan sebutharga

dan menawar untuk melaksanakan dan menjalankan kerja-kerja dan peruntukan-
peruntukan dan membekalkan semua buruh, bahan dan logi-logi, tanda-tanda amaran,
mesin-mesin, pen&ahayaan waktu malam dan pada keadaan-keadaan yang
memerlukan' Kubuan dan lain-lain yang diperlukan termasuk perlindungan insuran
kerja, pampas an pekerja, tanggungan awam dan lain-lain risiko am dan bayaran
&ukai dan le(i berkaitan kepada pihak berkuasa yang berkaitan dan segala benda dari
tiap-tiap jenis yang masing-masing disebut dan lain-lain benda yang diperlukan dalam
menjalankan kerja-kerja dengan &ara yang selamat dan betul, yang samada ditunjuk,
diperihal dan dimaksudkan dalam atau yang hendak ditakrifkan daripada Dokumen
sebutharga, yang hendaklah dilaksanakan dan dibekalkan oleh pihak kontraktor, bagi
kerja-kerja yang diperihalkan diatas, dengan menepati Dokumen Sebutharga tersebut
dengan kadar harga yang disebut di bawah ini'

syarat-syarat kontrak dan spesifikasi tersebut dan bersetuju bahawa jika sebutharga

menambah *ad jumlah *arga Kumulatif, maka kontrak ini akan luput dengan
tersendirinya atau dengan notis daripada Pegawai Penguasa, apabila yang ditentukan
telah di&apai'
5' ,ahawasanya adalah diketahui bahawa "ang Dipertua sentiasa berhak menyetujuterima
atau menolak Sebutharga ini, samada ianya lebih rendah atau lebih tinggi daripada
sebutharga-sebutharga yang lain atau sama amaunnya' "ang bertandatangan di bawah
ini bersetuju yang sebutharga ini akan berterusan sah dan tarikh tidak akan ditarik balik
dalam tempoh sembilan puluh (54 hari dari tarikh akhir ditetapkan bagi penyerahan
sebutharga dan bersetuju bahawa tiada apa-apa had, syarat atau janjian lain akan
dikenakan oleh kami selepas tarikh tersebut'

terhadap ben&ana kepada orang-orang dan kerosakan kepada

b' Nombor pendaftaran di bawah Skim Keselamatan Sosial Pekerja
(P=9K=S> ! dan / atau
&' Polisi ;nsuran Pampasan Pekerja atau Nota <iputan berserta
dengan resit bagi premium yang telah dibayar'
"ang bertandatangan di bawah ini selanjutnya bersetuju akan mendapatkan polisi
insuran tersebut daripada Panel-panel ,roker insuran yang boleh diterima pakai dan
akan mendeposit Polisi insuran bagi kerja-kerja tersebut kepada Pegawai Penguasa
sebelum melaksanakan kerja' "ang bertandatangan di bawah ini juga akan
melaksanakan perjanjian kontrak yang formal dalam masa yang munasabah'

1. HAK MAJLIS PERBANDARAN KAJANG (MPKj) UNTUK
MENERIMA/MENOLAK SEBUT HARGA
Majlis Perbandaran Kajang (MPKj) adalah tidak terikat untuk (eneri(a sebut
harga $ang terendah atau (ana'(ana sebut harga atau (e(beri a"a'a"a sebab
di atas "enolakan sesuatu sebut harga Ke"utusan /a!atankuasa &ebut %arga
MPKj adalah (ukta(ad
2. CARA-CARA MELENGKAPKAN DOKUMEN SEBUT HARGA
2.1 Pene!"##n Se$%& H#'(#
Kontraktor adalah dikehendaki (engisi dengan dak!at segala (aklu(at
berikut dengan se"enuhn$a:'
(a) %arga dan tandatangan Kontraktor di *ingkasan &ebut %arga,
(b) %arga, te("oh dan tandatangan dala( Borang &ebut %arga,
(0) &enarai Kerja Dala( Tangan (jika ada),
(d) /adual Kadar %arga (jika ada),
(e) Butir'butir &"esi)ikasi (jika ada)
2.2 Pene'#)#n D*+%,en Se$%& H#'(#
(a) Doku(en &ebut %arga $ang telah diisi dengan lengka"
hendaklah di(asukkan ke dala( sa("ul surat berlakri $ang
ber0atatkan dengan bilangan sebut hargaserta

(b) &alinan Pen$ata Bulanan #kaun Bank &$arikat bagi Tiga (3) Bulan

2. PERBELANJAAN PENYEDIAAN SEBUT HARGA

4. TEMPOH SAH SEBUT HARGA
&ebut harga ini sah sela(a se(bilan "uluh (6-) hari dari tarikh tutu"

7 Pengiraan harga doku(en sebut harga hendaklah dibuat berdasarkan

1. PEMERIKSAAN TAPAK BINA
Kontraktor disi)atkan telah (e(eriksa dan (eneliti ta"ak bina dan
sekitarn$a, bentuk dan jenis ta"ak bina, takat dan jenis kerja, bahan dan
barang $ang "erlu bagi (en$ia"kan Kerja, 0ara'0ara "erhubungan dan
laluan (asuk ke ta"ak bina dan hendaklah (enda"atkan sendiri segala
(aklu(at $ang "erlu tentang risiko, luar jangkaan dan segala hal'
keadaan $ang (e("engaruhi dan (enjejas sebut hargan$a &ebarang

"elaksanaan Kerja ini Kontraktor hendaklah juga (enge(ukakan

++ Kontraktor hendaklah (enge(ukakan ke"ada Pega!ai ,nden se(ua
"olisi insurans dan No(bor Kod Penda)taran dengan P9*K9&: $ang

(e(ulakan Kerja sahaja Nota'nota Perlindungan dan resit'resit ba$aran

5 Potong jika tidak di"erlukan
0. KEGAGALAN KONTRAKTOR MEMULAKAN KERJA

tarikh akhir te("oh (ula kerja $ang din$atakan dala( inden, tan"a
sebab'sebab $ang (unasabah, inden akan dibatalkan oleh Pega!ai
,nden dan tindakan tatatertib akan dia(bil terhada" Kontraktor
2. SUB-SE5A DAN MENYERAHHAK KERJA
Kontraktor tidak dibenarkan (engsub'se!akan Kerja ke"ada Kontraktor'
kontraktor lain Kontraktor tidak boleh (en$erahhak a"a'a"a )aedah di
ba!ah ,nden ini tan"a terlebih dahulu (enda"atkan "ersetujuan bertulis
dari"ada Pega!ai ,nden
4. PENOLAKKAN BAHAN6 BARANG DAN MUTU HASIL KERJA OLEH
PEGA5AI INDEN
48 Pega!ai ,nden atau ;akiln$a berhak (enolak bahan, barang dan (utu
hasil kerja dari jenis "ia!aian $ang tidak (ene"ati se"erti di"erihalkan
dala( s"esi)ikasi Kontraktor hendaklah, a"abila di(inta oleh Pega!ai
,nden, (e(beri ke"adan$a bau0ar'bau0ar dan/atau "erakuan ujian
"engilang untuk (e(buktikan baha!a bahan'bahan dan barang'barang
itu (e(enuhi s"esi)ikasi Bahan, barang dan kerja'kerja $ang ditolak
hendaklah diganti dan sebarang kos ta(bahan $ang terlibat hendaklah
ditanggung oleh Kontraktor sendiri
4+ Kontraktor hendaklah dengan se"enuhn$a atas "erbelanjaan sendiri

7. RINGKASAN SEBUT HARGA

sia" Bagi kerja'kerja (ekanikal dan elektrikal di(ana te("oh !aranti ke
atas alat'alat dan loji'loji adalah dua belas (8+) bulan dan dala( kes'kes
tertentu oleh kerana jenis dan keru(itan kerja, te("oh tanggungan
ke0a0atan $ang lebih la(a dari"ada ena( (4) bulan boleh dikenakan
8++ Kontraktor di"ertanggungja!abkan untuk (e(baiki ke0a0atan,
ketidakse("urnaan, keke0utan atau a"a'a"a jua kerosakan lain $ang
