MUSKULOSKELETAL
1
• Orthopaedics is concerned with bones, joints,
muscles, tendons and nerves – the skeletal
system and all that makes it move.
(Apley’s System of Orthopaedics and Fractures)
ILMU GERAK
• KINESIOLOGI :
Adalah ilmu yang mempelajari tubuh manusia
pada waktu melakukan gerakan. Beberapa
disiplin ilmu yang terlibat dalam kinesiologi
adalah anatomi, fisiologi neuromuskular dan
biomekanika

SISTEM PENGUNGKIT PADA
TUBUH MANUSIA
• Pengungkit adalah
batang yang kaku,
berputar pada suatu
aksis
• Fulcrum (axis)- titik
tumpu, aksis rotasi
• “Moment arm”- jarak
tegak lurus dari garis
aksi gaya ke axis
rotasi
6
SISTEM PENGUNGKIT
•TIPE I
•TIPE II
•TIPE III
7
Pengungkit Tipe I
• Titik penyokong terletak di
antara gaya dan beban
• Gaya dan beban mempunyai
arah yang sama
• Contoh: articulatio
atlantooccipitalis
8
Pengungkit Tipe II
9
Pengungkit Tipe III
• Beban & gaya terdapat pada sisi
yang sama terhadap titik penyokong
• Arah gaya dan beban berlawanan
• Lengan gaya lebih pendek daripada
lengan beban
• Pengungkit jenis ini banyak terdapat
pada tubuh manusia, contoh:
articulatio humero-ulnaris ; gaya
berupa otot-otot flexor seperti m.
brachialis, sedangkan beban ialah
berat lengan bawah
10
Peranan otot rangka
Agonis
• Otot yang menghasilkan gerak yang dikehendaki
Antagonis
• Otot yang bekerja berlawanan dengan agonis
Stabilizer
• Otot-otot yang bekerja pada satu segmen sehingga
pergerakan pada segmen yang berdekatan dapat terjadi
Neutralizer
• Otot yang mengeliminir gerakan yang tak diinginkan dari
otot lain 11
Muscles perform a variety roles in movement. In
arm abduction, the deltoid is the agonist, since it
is responsible for the abduction movement. The
latissimus dorsi is the antagonistic muscle, since
it is resists abduction. There are also muscles
stabilizing in the region so the movement can
occur. Here, the trapezius is shown stabilizing
and holding the scapula in place. Last there may
be some neutralizing action: the teres minor may
neutralize via external rotation any internal
rotation produced by the latissimus dorsi.

KERJA OTOT
Concentric
• Otot berkontraksi disertai dengan pemendekan
otot
Eccentric
• Otot berkontraksi disertai dengan pemanjangan
otot.
Isometric
• Otot berkontraksi disertai sedikit perubahan
panjang otot / tidak ada perubahan panjang
otot. Contoh: menahan bahu tetap pada posisi 13

GERAK
• GERAK DASAR
berjalan, berlari, melompat, melempar, menangkap
• GERAK YANG BERTUJUAN
menulis, bersepeda, dll
• GERAK ADAPTIF
penggunaan prothese, tongkat penyangga

BERJALAN
Definition:
• Gait circle: movement of single limb heel
strike to heel strike again.
• Step: from heel strike of one foot to heel strike
of the opposite foot
• Stride: from heel strike of one foot to the
subsequent heel strike of the same foot
• Stance phase:
– 60% of gait circle.
– Part of gait when foot is in contact with ground
– Initial contact to preswing.
– Devided 5 phases: Initial contact, Foot flat, Mid
stance, Terminal stance, Pre swing.
• Swing Phase:
• Part of gait with foot in air, advancing forward
• Devided 3 phases: Intitial swing, Mid swing,
Terminal swing.
BERJALAN
23
Determinant of Gait:
• Pelvic rotation
• Pelvic tilt
• Flexion at the knee
• Foot and ankle motion
• Knee motion
• Lateral movement of the pelvis
(Saunders, Enman & Eberhart)
27
BERJALAN
PELVIC ROTATION
• Saat kaki melayang, pelvis berputar ke
depan.
• Saat kaki melangkah, pusat gravitasi
pindah ke depan.
28
BERJALAN
TILTING
29
BERJALAN
FOOT AND ANKLE MOTION
• Kaki dan pergelangan kaki membantu
melancarkan jalur pusat gravitasi pada
saat foot strike (heel strike)
KNEE MOTION
• Fleksi lutut toe off
• Shock absorber
30
BERJALAN
LATERAL MOVEMENT OF THE PELVIS
• Setiap tahap lateral movement
• Femur aduksi pelvis
• Tibia sedikit valgus
31
BERJALAN
* = low activity
** = moderate activity
For stair climbing as a whole,
there is less contribution from *** = high activity
the hip muscles than in walking
or running. 35
